Detaljeret beskrivelse
Dette forsøg vil bestå af 1600 patienter med skizofreni, for hvem et medicinskifte kan være indiceret på grund af begrænset effekt eller tolerabilitet. Alle patienter vil modtage en vis psykosocial behandling gennem studiedeltagelse. Forskningsdeltagere og deres familiemedlemmer vil blive tilbudt psykosociale interventioner rettet mod at forbedre patientens og familiens forståelse af sygdommen, mindske sygdomsbyrden i familien, maksimere behandlingstilslutning, minimere tilbagefald, forbedre adgangen til en række lokalsamfundsbaserede rehabiliteringstjenester og forbedring af studiefastholdelse.
Fase I: Patienter vil blive tilfældigt tildelt en af fem behandlingstilstande i op til 18 måneder:
- 320 begynder dobbelt-blind behandling med perphenazin (PER)
- 320 begynder dobbeltblind behandling med olanzapin (OLZ)
- 320 begynder dobbeltblind behandling med quetiapin (QUET)
- 320 begynder dobbeltblind behandling med risperidon (RIS)
- 220 begynder dobbeltblindbehandling med ziprasidon (ZIP)
Fase IA: 100 patienter, der er screenet og fundet at have tardiv dyskinesi, som ellers ville være kvalificerede til undersøgelsen, vil blive tilfældigt tildelt et af de fire atypiske lægemidler i fase IA.
Fase IB: Patienter, der mislykkes med behandling med perphenazin i fase I, vil blive tilfældigt tildelt olanzapin, quetiapin eller risperidon i fase IB.
Fase II: Patienter, der afbryder deres oprindelige tildelte atypiske antipsykotiske behandling i fase I, IA eller IB af en hvilken som helst ikke-administrativ årsag, vil fortsætte til deres anden tildelte behandling (tredje for fase IB-patienter) og vil blive fulgt i op til resten af deres 18-måneders deltagelse, som følger:
- Patienter, der oprindeligt blev tildelt et af de nyere atypiske antipsykotika, og som ophører på grund af effektsvigt, vil blive tilfældigt tildelt dobbeltblind behandling med et af de to andre nyere atypiske antipsykotika (OLZ, RIS, QUET), som de ikke tidligere havde fået (50 % ) eller med åbent mærket clozapin (50%).
- Patienter, der oprindeligt blev tildelt et af de nyere atypiske antipsykotika, og som stopper på grund af tolerabilitetssvigt, vil blive tilfældigt tildelt dobbeltblind behandling med et af de andre nyere atypiske antipsykotika (OLZ, RIS, QUET), som de ikke tidligere havde fået (50 %) eller med ziprasidon (50%). Indtil ziprasidon aktiveres, vil alle patienter blive tildelt et af de andre atypiske antipsykotika.
Fase II vil vare mindst 6 måneder, selvom det betyder, at deltagerne bliver i undersøgelsen i mere end 18 måneder
Fase III: Patienter, der afbryder fase II, vil blive anbefalet åben behandling med det foretrukne regime baseret på deres behandlingshistorie i undersøgelsen. Behandlingsmulighederne omfatter clozapin, nyere atypisk antipsykotikum (olanzapin, risperidon, quetiapin, ziprazidon og aripiprazol), fluphenazin decanoat, perphenazin og dobbelt antipsykotisk behandling ved brug af to af disse lægemidler.
Bemærk: Alle behandlinger vil være dobbeltblindede i behandlingsfase I og II undtagen clozapin.
