CATIE – Schizophrenie-Studie
Vergleichende Wirksamkeit antipsychotischer Medikamente bei Patienten mit Schizophrenie (CATIE Schizophrenia Trial)

Detaillierte Beschreibung
Diese Studie wird 1600 Patienten mit Schizophrenie umfassen, bei denen eine Medikationsänderung aufgrund eingeschränkter Wirksamkeit oder Verträglichkeit indiziert sein kann. Alle Patienten erhalten durch die Studienteilnahme eine psychosoziale Behandlung. Den Forschungsteilnehmern und ihren Familienangehörigen werden psychosoziale Interventionen angeboten, die darauf abzielen, das Krankheitsverständnis von Patienten und Familien zu verbessern, die Krankheitslast in der Familie zu verringern, die Therapietreue zu maximieren, Rückfälle zu minimieren, den Zugang zu einer Reihe von gemeindenahen Rehabilitationsdiensten zu verbessern und Verbesserung der Studienretention.
Phase I: Die Patienten werden nach dem Zufallsprinzip für bis zu 18 Monate einer von fünf Behandlungsbedingungen zugewiesen:
- 320 beginnen doppelblinde Behandlung mit Perphenazin (PER)
- 320 beginnen doppelblinde Behandlung mit Olanzapin (OLZ)
- 320 Beginn einer doppelblinden Behandlung mit Quetiapin (QUET)
- 320 beginnen doppelblinde Behandlung mit Risperidon (RIS)
- 220 beginnen doppelblinde Behandlung mit Ziprasidon (ZIP)
Phase IA: 100 gescreente Patienten mit tardiver Dyskinesie, die ansonsten für die Studie in Frage kämen, werden nach dem Zufallsprinzip einem der vier atypischen Medikamente in Phase IA zugeteilt.
Phase IB: Patienten, bei denen die Behandlung mit Perphenazin in Phase I fehlschlägt, werden nach dem Zufallsprinzip Olanzapin, Quetiapin oder Risperidon in Phase IB zugeteilt.
Phase II: Patienten, die ihre ursprünglich zugewiesene atypische antipsychotische Behandlung in Phase I, IA oder IB aus irgendeinem nicht-administrativen Grund abbrechen, werden mit ihrer zweiten zugewiesenen Behandlung (dritter für Patienten der Phase IB) fortfahren und bis zum Rest von weiterverfolgt ihre 18-monatige Teilnahme wie folgt:
- Patienten, die ursprünglich einem der neueren atypischen Antipsychotika zugewiesen wurden und die Behandlung wegen Versagens der Wirksamkeit abbrechen, werden nach dem Zufallsprinzip einer doppelblinden Behandlung mit einem der beiden anderen neueren atypischen Antipsychotika (OLZ, RIS, QUET) zugeteilt, die sie zuvor nicht erhalten hatten (50 % ) oder mit offenem Clozapin (50 %).
- Patienten, die ursprünglich einem der neueren atypischen Antipsychotika zugewiesen wurden und die Behandlung aufgrund von Verträglichkeitsfehlern abbrechen, werden nach dem Zufallsprinzip einer doppelblinden Behandlung mit einem der anderen neueren atypischen Antipsychotika (OLZ, RIS, QUET) zugewiesen, die sie zuvor nicht erhalten hatten (50 %). , oder mit Ziprasidon (50 %). Bis zur Aktivierung von Ziprasidon werden alle Patienten einem der anderen atypischen Antipsychotika zugeordnet.
Phase II wird mindestens 6 Monate dauern, auch wenn dies bedeutet, dass die Teilnehmer länger als 18 Monate in der Studie bleiben
Phase III: Patienten, die Phase II abbrechen, wird eine offene Behandlung mit dem bevorzugten Regime basierend auf ihrer Behandlungshistorie in der Studie empfohlen. Die Behandlungsoptionen umfassen Clozapin, neuere atypische Antipsychotika (Olanzapin, Risperidon, Quetiapin, Ziprazidon und Aripiprazol), Fluphenazin-Decanoat, Perphenazin und eine duale antipsychotische Therapie mit zwei dieser Medikamente.
Hinweis: Alle Behandlungen mit Ausnahme von Clozapin werden in den Behandlungsphasen I und II doppelblind durchgeführt.
