Efficacia comparativa dei farmaci antipsicotici nei pazienti con schizofrenia (CATIE Schizophrenia Trial)

Questo studio sarà composto da 1600 pazienti con schizofrenia per i quali potrebbe essere indicato un cambio di terapia per motivi di limitata efficacia o tollerabilità. Tutti i pazienti riceveranno un trattamento psicosociale attraverso la partecipazione allo studio. Ai partecipanti alla ricerca e ai loro familiari verranno offerti interventi psicosociali diretti a migliorare la comprensione della malattia da parte del paziente e della famiglia, ridurre il peso della malattia in famiglia, massimizzare l'aderenza al trattamento, ridurre al minimo le ricadute, migliorare l'accesso a una gamma di servizi riabilitativi basati sulla comunità e migliorare la conservazione dello studio.
Fase I: i pazienti verranno assegnati in modo casuale a una delle cinque condizioni di trattamento per un massimo di 18 mesi:
- 320 iniziano il trattamento in doppio cieco con perfenazina (PER)
- 320 iniziano il trattamento in doppio cieco con olanzapina (OLZ)
- 320 iniziano il trattamento in doppio cieco con quetiapina (QUET)
- 320 iniziano il trattamento in doppio cieco con risperidone (RIS)
- 220 iniziano il trattamento in doppio cieco con ziprasidone (ZIP)
Fase IA: 100 pazienti sottoposti a screening e trovati affetti da discinesia tardiva che sarebbero altrimenti idonei per lo studio saranno assegnati in modo casuale a uno dei quattro farmaci atipici nella Fase IA.
Fase IB: i pazienti che falliscono il trattamento con perfenazina nella Fase I verranno assegnati in modo casuale a olanzapina, quetiapina o risperidone nella Fase IB.
Fase II: i pazienti che interrompono il trattamento antipsicotico atipico assegnato inizialmente nella Fase I, IA o IB per qualsiasi motivo non amministrativo procederanno al secondo trattamento assegnato (terzo per i pazienti di Fase IB) e saranno seguiti fino al resto del la loro partecipazione di 18 mesi, come segue:
- I pazienti originariamente assegnati a uno dei nuovi antipsicotici atipici che interrompono a causa di fallimento di efficacia saranno assegnati in modo casuale al trattamento in doppio cieco con uno degli altri due nuovi antipsicotici atipici (OLZ, RIS, QUET) che non avevano ricevuto in precedenza (50% ) o con clozapina in aperto (50%).
- I pazienti originariamente assegnati a uno dei nuovi antipsicotici atipici che interrompono a causa del fallimento della tollerabilità saranno assegnati in modo casuale al trattamento in doppio cieco con uno degli altri nuovi antipsicotici atipici (OLZ, RIS, QUET) che non avevano ricevuto in precedenza (50%) o con ziprasidone (50%). Fino all'attivazione di ziprasidone, tutti i pazienti verranno assegnati a uno degli altri antipsicotici atipici.
La Fase II durerà almeno 6 mesi, anche se ciò significa che i partecipanti rimarranno nello studio per più di 18 mesi
Fase III: ai pazienti che interrompono la Fase II verrà raccomandato un trattamento aperto con il regime preferito in base alla loro storia di trattamento nello studio. Le opzioni di trattamento includono clozapina, antipsicotici atipici più recenti (olanzapina, risperidone, quetiapina, ziprazidone e aripiprazolo), flufenazina decanoato, perfenazina e doppia terapia antipsicotica utilizzando due di questi farmaci.
Nota: tutti i trattamenti saranno in doppio cieco nelle fasi di trattamento I e II ad eccezione della clozapina.

- Intolleranza o mancata risposta a uno dei trattamenti
- Diagnosi di disturbo schizoaffettivo, ritardo mentale, disturbo pervasivo dello sviluppo, delirio, demenza, amnesia
- Primo episodio di schizofrenia
- Donne attualmente in stato di gravidanza o che allattano
