Zeitpunkt der primären Operation bei Gaumenspalten (TOPS)
Bei dieser Studie handelt es sich um eine randomisierte kontrollierte Studie mit parallelem Design, die in Zentren im Vereinigten Königreich, Skandinavien und Brasilien durchgeführt wird.
650 Säuglinge mit der Diagnose einer isolierten Gaumenspalte, die nach 6 Monaten als medizinisch für eine Operation geeignet gelten und die Einschlusskriterien erfüllen, werden in die Studie aufgenommen und randomisiert und erhalten entweder:
- Operation im Alter von 6 Monaten, ODER
- Operation im Alter von 12 Monaten.
Das Hauptziel besteht darin, festzustellen, ob eine Operation einer Gaumenspalte mit einer Sommerlad-Technik im Alter von 6 Monaten im Vergleich zu einer Operation mit derselben Technik im Alter von 12 Monaten die velopharyngeale Funktion im Alter von 5 Jahren verbessert. Alle Säuglinge werden im Alter von 12 Monaten, 3 Jahren und fünf Jahren zur Beurteilung der primären Ergebnisse (im Alter von 5 Jahren) und sekundären Ergebnissen nachuntersucht.

Säuglinge werden im Alter von 12 Monaten, 3 Jahren und fünf Jahren nachuntersucht. Bei jedem Besuch wird Folgendes beurteilt:

Beschreibung
Einschlusskriterien:
- Säuglinge mit isolierter Gaumenspalte
- Mit 6 Monaten medizinisch operativ geeignet, korrigiert um das Gestationsalter
- Schriftliche Einverständniserklärung des Bevollmächtigten
- Ein Elternteil/Betreuer ist Muttersprachler im Wohnsitzland
Ausschlusskriterien:
- Einwilligung nicht eingeholt
- B. Säuglinge mit syndromaler Gaumenspalte (außer Van-der-Woude-Syndrom, das eingeschlossen werden kann, wenn das Gehör nicht beeinträchtigt ist) oder schwerer Entwicklungsverzögerung
- Angeborener Schallempfindungsschwerhörigkeit oder Mittelohranomalien;
- Die Abweichungen in der anatomischen Darstellung sind so groß, dass der Chirurg, der den Eingriff durchführt, der Ansicht ist, dass ein einstufiger Verschluss mit der Sommerlad-Technik ungeeignet wäre;
- Submuköse Gaumenspalte (definiert durch die klassische Zeichentrias, Bifid, Zäpfchen, knöcherner Defekt des harten Gaumens, Muskeldiastase, wie von Jensen et al. (1988) beschrieben.
- Wenn die zu Hause gesprochene Sprache nicht die Mehrheitssprache im Wohnsitzland ist.

Insufficient Velopharyngeal Function
Zeitfenster: 5 years of age
|
Measured by the Velopharyngeal Composite Score (VPC sum), which is a sum of scores, based on three components: hypernasality, non-oral errors, and VPI symptoms.
Each component is scored between 0 and 2. The sum of the three scores gives the VPC sum (0-6).
Scores ≥ 4 on this scale are considered insufficient (higher scores indicate a worse outcome).
This outcome is measured as a dichotomous variable (insufficient or not insufficient) with a cut-off threshold of ≥ 4 to indicate insufficient.
We report this as a percentage of participants that are assessed as having insufficient velopharyngeal function.
