Harmonogram pierwotnej operacji rozszczepu podniebienia (TOPS)
To badanie jest randomizowanym badaniem kontrolowanym o równoległym projekcie, odbywającym się w ośrodkach w Wielkiej Brytanii, Skandynawii i Brazylii.
650 niemowląt z rozpoznaniem izolowanego rozszczepu podniebienia, uznanych za zdolnych do operacji w wieku 6 miesięcy i spełniających kryteria włączenia, zostanie włączonych do badania i losowo przydzielonych do:
- Operacja w wieku 6 miesięcy, LUB
- Operacja w wieku 12 miesięcy.
Głównym celem jest ustalenie, czy operacja rozszczepu podniebienia techniką Sommerlada w wieku 6 miesięcy w porównaniu z operacją tą samą techniką w wieku 12 miesięcy poprawia funkcję podniebienia i gardła w wieku 5 lat. Wszystkie niemowlęta zostaną poddane obserwacji w wieku 12 miesięcy, 3 lat i 5 lat w celu oceny głównych wyników (w wieku 5 lat) i drugorzędnych wyników.

- Wrodzony niedosłuch czuciowo-nerwowy lub wady ucha środkowego;
- Różnice w obrazie anatomicznym są takie, że chirurg wykonujący zabieg uważa, że zamknięcie jednoetapowe techniką Sommerlada byłoby niewłaściwe;
- Podśluzówkowy rozszczep podniebienia (zdefiniowany przez klasyczną triadę objawów, rozszczep, języczek, ubytek kostny podniebienia twardego, rozstęp mięśniowy, jak opisali Jensen i wsp. (1988).

Insufficient Velopharyngeal Function
Ramy czasowe: 5 years of age
|
Measured by the Velopharyngeal Composite Score (VPC sum), which is a sum of scores, based on three components: hypernasality, non-oral errors, and VPI symptoms.
Each component is scored between 0 and 2. The sum of the three scores gives the VPC sum (0-6).
Scores ≥ 4 on this scale are considered insufficient (higher scores indicate a worse outcome).
This outcome is measured as a dichotomous variable (insufficient or not insufficient) with a cut-off threshold of ≥ 4 to indicate insufficient.
We report this as a percentage of participants that are assessed as having insufficient velopharyngeal function.
