Tempistica della chirurgia primaria per palatoschisi (TOPS)

Questo studio è uno studio controllato randomizzato con un disegno parallelo che si svolge nei centri del Regno Unito, della Scandinavia e del Brasile.

650 neonati con diagnosi di palatoschisi isolata che sono considerati clinicamente idonei per l'operazione a 6 mesi e che soddisfano i criteri di inclusione, saranno inclusi nello studio e randomizzati per ricevere:

  • Chirurgia all'età di 6 mesi, OR
  • Chirurgia all'età di 12 mesi.

L'obiettivo principale è determinare se la chirurgia per palatoschisi, utilizzando una tecnica Sommerlad, all'età di 6 mesi, rispetto alla chirurgia utilizzando la stessa tecnica all'età di 12 mesi, migliora la funzione velofaringea all'età di 5 anni. Tutti i bambini saranno seguiti all'età di 12 mesi, 3 anni e cinque anni per la valutazione degli esiti primari (all'età di 5 anni) e degli esiti secondari.

Criterio di inclusione:

  • Neonati con palatoschisi isolata
  • Idoneo dal punto di vista medico per l'operazione a 6 mesi, corretto per l'età gestazionale
  • Consenso informato scritto per delega
  • Un genitore/tutore madrelingua nel paese di residenza

Criteri di esclusione:

  • Consenso non ottenuto
  • B. Neonati con palatoschisi sindromica (tranne la sindrome di Van der Woude, che può essere inclusa se l'udito non è affetto) o grave ritardo dello sviluppo
  • Ipoacusia neurosensoriale congenita o anomalie dell'orecchio medio;
  • La variazione nella presentazione anatomica è tale che il chirurgo che eseguirà la procedura ritiene che la chiusura di uno stadio con la tecnica Sommerlad sarebbe inappropriata;
  • Palatoschisi sottomucosa (definita dalla classica triade di segni, bifido, ugola, difetto osseo del palato duro, diastasi muscolare, come descritto da Jensen et al (1988).
  • Dove la lingua parlata a casa non è la lingua maggioritaria nel paese di residenza.

Insufficient Velopharyngeal Function
Lasso di tempo: 5 years of age
Measured by the Velopharyngeal Composite Score (VPC sum), which is a sum of scores, based on three components: hypernasality, non-oral errors, and VPI symptoms. Each component is scored between 0 and 2. The sum of the three scores gives the VPC sum (0-6). Scores ≥ 4 on this scale are considered insufficient (higher scores indicate a worse outcome). This outcome is measured as a dichotomous variable (insufficient or not insufficient) with a cut-off threshold of ≥ 4 to indicate insufficient. We report this as a percentage of participants that are assessed as having insufficient velopharyngeal function.
