Akuutti sydän- ja hengityselimistön vaste verenkiertorajoitetuille ja perinteisille harjoitusohjelmille (CaRe BFR) (CaRe-BFR)
Akuutti sydän- ja hengityselimistön vaste verenkiertorajoitettuun verrattuna perinteiseen harjoitusohjelmaan (CaRe BFR): 4 satunnaistettua crossover-tutkimusta
Tutkijat olettavat, että BFR-harjoitusohjelmat johtavat erilaiseen akuuttiin kardiorespiratoriseen vastemalliin verrattuna perinteisiin harjoitusohjelmiin. Lisäksi tutkijat olettavat, että nämä mallit eroavat terveiden osallistujien ja keuhkoahtaumatautia sairastavien osallistujien välillä.
Toissijaisen tavoitteen osalta tutkijat olettavat, että BFR johtaa alhaisempiin verenpainevasteisiin verrattuna perinteiseen harjoitteluun sekä terveillä että COPD-potilailla.

Tutkijat toteuttavat yksikeskisen projektin, jossa on neljä satunnaistettua crossover-tutkimusta.
CaRe-BFR terve ja COPD:
Voimaharjoittelu suoritetaan kahdenvälisesti Leg Press -laitteella (Leg Press VR2, Cybex International Inc. Medway, MA, USA) käyttäen kolmen sarjan metodologiaa, joka on osoittautunut tehokkaimmaksi lihasvoiman lisäämisessä.16 Jokainen sarja suoritetaan lihasten vajaatoimintaan asti, joka tulisi saavuttaa 8-12 toiston sisällä. Harjoitusrytmi asetetaan arvoon 1-0-1-0 (eli yksi sekunti samankeskiselle vaiheelle, ei taukoa, yksi sekunti epäkeskiselle vaiheelle, ei taukoa) ja ulkoisesti tahdistetaan metronomilla. Harjoituskuorma on 80 % 1RM:stä. Lepo sarjojen välillä on 1 minuutti ja osallistujat jäljellä
Voimaharjoittelu BFR suoritetaan kahdenvälisesti Leg Press -laitteella (Leg Press VR2, Cybex International Inc. Medway, MA, USA) näyttöön perustuvien sovellusohjeiden mukaisesti tehokkaimman menetelmän soveltamiseksi lihasvoiman parantamiseen.17 Harjoitus koostuu yhteensä 75 toistosta 4 sarjan aikana. Harjoitusrytmi asetetaan arvoon 1-0-1-0 (eli yksi sekunti samankeskiselle vaiheelle, ei taukoa, yksi sekunti epäkeskiselle vaiheelle, ei taukoa) ja ulkoisesti tahdistetaan metronomilla. Sarja 1 sisältää 30 toistoa ja seuraavat sarjat 15 toistoa. Harjoituskuorma on 30 % 1RM:stä. Hihansuut on asennettu molemmin puolin reiden proksimaaliisimpaan osaan ja täytetty 70 %:iin yksittäisestä AOP:sta. Lepo sarjojen välillä on 1 minuutti ja osallistujat jäävät istumaan hihansuut täytettyinä.
-Kestävyys suoritetaan kahdenvälisesti paikallaan olevalla polkupyöräergometrillä (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Saksa) intervalliasetuksella kuormitusvaiheilla 1 sarja, jossa on 3 jaksoa, jotka kestävät kumpikin 2 minuuttia; harjoituskuormitus on 65 % huipputyötahdista (PWR). Välien välissä tauot on vakioitu 1 minuuttiin. Ennen harjoitusottelua suoritetaan lämmittelypyöräily 30 % PWR:llä 5 minuutin ajan vakaan tilan saavuttamiseksi HR- ja RER-arvoissa.
Endurance BFR suoritetaan kahdenvälisesti paikallaan olevalla polkupyöräergometrillä (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Saksa) intervalliasetuksella 1 sarjan kuormitusvaiheilla, joissa on 3 jaksoa, jotka kestävät kumpikin 2 minuuttia ja 18 sekuntia (sopii kokonaistyöhön FI); harjoituskuormitus on 50 % PWR:stä, kun taas raajan verenvirtaus on rajoitettu 50 prosenttiin AOP:sta. Välien välissä tauot on standardoitu 1 minuuttiin (mansetti täytettynä). Ennen harjoitusottelua suoritetaan lämmittelypyöräily (ilman BFR) 30 % PWR:llä 5 minuutin ajan, jotta saavutetaan vakaa tila HR- ja RER-arvoissa.
Mitat:
Harjoittelun aikana tutkijat keräävät hengitys-hengitystietoja (Ergostik, Geratherm Respiratory GmbH, Bad Kissingen, Saksa). Lisäksi jatkuvat SpO2- ja HR-tiedot korvaliuskan kautta sekä verenpainevaste välittömästi harjoitussarjojen keräämisen jälkeen

COPD-potilaiden mukaan ottaminen ja poissulkeminen määritellään seuraavilla kriteereillä.

Control: Bilateral leg press at 40% 1RM, 3 sets of 15 repetitions, without blood flow restriction. |

Patients with COPD will complete three cycling exercise conditions in a randomized, crossover design:

BFR-CYC: Continuous cycling at 50% peak work rate (PWR) with bilateral thigh cuffs inflated to 50% limb occlusion pressure (LOP).
A maximum duration of 30 minutes is applied as a safety limit.

Healthy participants will complete two BFR resistance exercise conditions in a randomized, crossover design:

Participants will perform unilateral right-leg press exercise at 40% 1RM using a standardized protocol of 4 sets (30-15-15-15 repetitions) with fixed rest intervals. The protocol will be completed as prescribed (not to volitional exhaustion) unless predefined safety criteria require early termination. |

Aikaikkuna: Pre-exercise and immediately post-exercise (within minutes after completion of the exercise protocol)

Maximal voluntary contraction (MVC) of the knee extensors will be assessed before and immediately after unilateral leg press exercise performed with blood flow restriction at 40% and 80% limb occlusion pressure (LOP).
MVC will be measured using an isometric setup, and voluntary activation will be evaluated using the twitch interpolation technique with supramaximal electrical stimulation.
The primary outcome is the change in MVC from pre- to post-exercise, reflecting exercise-induced neuromuscular fatigue.
