Ostra odpowiedź krążeniowo-oddechowa na schematy treningu fizycznego z ograniczeniem przepływu krwi w porównaniu z tradycyjnymi schematami ćwiczeń (CaRe BFR): 4 randomizowane badania krzyżowe
Badacze postawili hipotezę, że schematy ćwiczeń BFR skutkują innym wzorcem ostrej odpowiedzi krążeniowo-oddechowej w porównaniu z tradycyjnymi schematami ćwiczeń. Ponadto badacze postawili hipotezę, że wzorce te różnią się między zdrowymi uczestnikami a uczestnikami z POChP.
Jeśli chodzi o cel drugorzędowy, badacze postawili hipotezę, że BFR skutkuje niższymi reakcjami na ciśnienie krwi w porównaniu z tradycyjnym treningiem fizycznym zarówno u osób zdrowych, jak i uczestników POChP.

Badacze realizują projekt monocentryczny z 4 randomizowanymi badaniami krzyżowymi.
CaRe-BFR Zdrowy i POChP:
Trening siłowy jest wykonywany obustronnie na maszynie do wyciskania nóg (Leg Press VR2, Cybex International Inc. Medway, MA, USA) przy użyciu metodologii trzech zestawów, która okazała się najbardziej skuteczna w zwiększaniu siły mięśni.16 Każdą serię wykonujemy do wyczerpania mięśniowego, które należy osiągnąć w ciągu 8-12 powtórzeń. Rytm treningu jest ustawiony na 1-0-1-0 (tj. jedna sekunda dla fazy koncentrycznej, bez pauzy, jedna sekunda dla fazy ekscentrycznej, bez pauzy) i zewnętrznie sterowany metronomem. Obciążenie treningowe wynosi 80% 1RM. Odpoczynek pomiędzy seriami wynosi 1 minutę, a uczestnicy pozostają

Medway, MA, USA) zgodnie z wytycznymi aplikacji opartymi na dowodach, aby zastosować najskuteczniejszą metodologię w celu zwiększenia siły mięśni.17 Ćwiczenie składa się łącznie z 75 powtórzeń w 4 seriach. Rytm treningu jest ustawiony na 1-0-1-0 (tj. jedna sekunda dla fazy koncentrycznej, bez pauzy, jedna sekunda dla fazy ekscentrycznej, bez pauzy) i zewnętrznie sterowany metronomem. Seria 1 obejmuje 30 powtórzeń, a kolejne serie po 15 powtórzeń. Obciążenie treningowe wynosi 30% 1RM. Mankiety są zakładane obustronnie w najbardziej proksymalnej części uda i napompowane do 70% indywidualnego AOP. Odpoczynek między seriami wynosi 1 minutę, a uczestnicy pozostają w pozycji siedzącej z napompowanymi mankietami.
-Wytrzymałość jest wykonywana obustronnie na stacjonarnym ergometrze rowerowym (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Niemcy) w ustawieniu interwałowym z fazami obciążenia 1 seria z 3 przerwami trwającymi po 2 minuty każda; obciążenie treningowe wynosi 65% szczytowego wskaźnika pracy (PWR). Pomiędzy interwałami przerwy są standaryzowane do 1 minuty. Przed rozpoczęciem ćwiczeń wykonuje się rozgrzewkę na rowerze przy 30% PWR przez 5 minut, aby osiągnąć stały poziom HR i RER.
Endurance BFR jest wykonywany obustronnie na stacjonarnym ergometrze rowerowym (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Niemcy) w ustawieniu interwałowym z fazami obciążenia 1 seria z 3 interwałami trwającymi po 2 minuty i 18 sekund każda (odpowiednio do ogólnej pracy wykonywanej w PL); obciążenie treningowe wynosi 50% PWR, podczas gdy przepływ krwi w kończynie jest ograniczony do 50% AOP. Pomiędzy przerwami przerwy są standaryzowane do 1 minuty (napompowany mankiet). Przed sesją ćwiczeń wykonuje się rozgrzewkę na rowerze (bez BFR) przy 30% PWR przez 5 minut, aby osiągnąć stały poziom HR i RER.
Wymiary:
Podczas ćwiczeń badacze zbierają dane oddech po oddechu (Ergostik, Geratherm Respiratory GmbH, Bad Kissingen, Niemcy). Ponadto gromadzone są ciągłe dane SpO2 i HR za pomocą sondy płatka ucha oraz odpowiedź BP natychmiast po zebraniu zestawów ćwiczeń

Both COPD patients and healthy participants will complete two endurance exercise conditions in a randomized, crossover design:
BFR-CYC: Three intervals of 2 minutes at 50% PWR, separated by 1-minute recovery periods, with bilateral thigh cuffs inflated to 50% LOP during exercise intervals. Control: Time- and structure-matched protocol (3 × 2 minutes) without blood flow restriction at 65% PWR, with identical recovery periods. |
BFR-CYC: Three intervals of 2 minutes at 50% PWR, separated by 1-minute recovery periods, with bilateral thigh cuffs inflated to 50% LOP during exercise intervals.

Patients with COPD will complete three cycling exercise conditions in a randomized, crossover design:

BFR-CYC: Continuous cycling at 50% peak work rate (PWR) with bilateral thigh cuffs inflated to 50% limb occlusion pressure (LOP).
A maximum duration of 30 minutes is applied as a safety limit.

Healthy participants will complete two BFR resistance exercise conditions in a randomized, crossover design:

Participants will perform unilateral right-leg press exercise at 40% 1RM using a standardized protocol of 4 sets (30-15-15-15 repetitions) with fixed rest intervals. The protocol will be completed as prescribed (not to volitional exhaustion) unless predefined safety criteria require early termination. |

Maximal voluntary contraction (MVC) of the knee extensors will be assessed before and immediately after unilateral leg press exercise performed with blood flow restriction at 40% and 80% limb occlusion pressure (LOP).
MVC will be measured using an isometric setup, and voluntary activation will be evaluated using the twitch interpolation technique with supramaximal electrical stimulation.
The primary outcome is the change in MVC from pre- to post-exercise, reflecting exercise-induced neuromuscular fatigue.
