La risposta cardiorespiratoria acuta alla limitazione del flusso sanguigno rispetto ai regimi di allenamento tradizionale (CaRe BFR): 4 studi crossover randomizzati
I ricercatori ipotizzano che i regimi di esercizio BFR si traducano in un modello di risposta cardiorespiratoria acuta diverso rispetto ai regimi di esercizio tradizionali. Inoltre, i ricercatori ipotizzano che questi modelli differiscano tra partecipanti sani e partecipanti con BPCO.
Per quanto riguarda l'obiettivo secondario, i ricercatori ipotizzano che il BFR si traduca in risposte pressorie inferiori rispetto all'allenamento tradizionale sia nei partecipanti sani che in quelli con BPCO.

Gli investigatori realizzano un progetto monocentrico con 4 studi incrociati randomizzati.
CaRe-BFR Sano e BPCO:
L'allenamento della forza viene eseguito bilateralmente su una macchina Leg Press (Leg Press VR2, Cybex International Inc. Medway, MA, USA) utilizzando la metodologia dei tre set, che si è dimostrata la più efficace nel migliorare la forza muscolare.16 Ogni serie viene eseguita fino al cedimento muscolare, che dovrebbe essere raggiunto entro 8-12 ripetizioni. Il ritmo di allenamento è impostato su 1-0-1-0 (ovvero un secondo per la fase concentrica, nessuna pausa, un secondo per la fase eccentrica, nessuna pausa) e scandito esternamente da un metronomo. Il carico di allenamento è l'80% dell'1RM. Il riposo tra le serie è di 1 minuto e i partecipanti rimangono
L'allenamento della forza BFR viene eseguito bilateralmente su una macchina Leg Press (Leg Press VR2, Cybex International Inc. Medway, MA, USA) in conformità con le linee guida applicative basate sull'evidenza per applicare la metodologia più efficace per migliorare la forza muscolare.17 L'esercizio consiste in un totale di 75 ripetizioni durante 4 serie. Il ritmo di allenamento è impostato su 1-0-1-0 (ovvero un secondo per la fase concentrica, nessuna pausa, un secondo per la fase eccentrica, nessuna pausa) e scandito esternamente da un metronomo. La serie 1 copre 30 ripetizioni e le serie successive 15 ripetizioni ciascuna. Il carico di allenamento è il 30% dell'1RM. I polsini sono montati bilateralmente nella parte più prossimale della coscia e gonfiati al 70% dell'AOP individuale. Il riposo tra le serie è di 1 minuto e i partecipanti rimangono seduti con i polsini gonfiati.
-La resistenza viene eseguita bilateralmente su un cicloergometro fisso (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Germania) in un'impostazione a intervalli con fasi di carico di 1 set con 3 intervalli della durata di 2 minuti ciascuno; il carico di allenamento è il 65% del picco di lavoro (PWR). Tra gli intervalli, le pause sono standardizzate a 1 minuto. Prima dell'incontro di allenamento, viene eseguito un ciclo di riscaldamento al 30% di PWR per 5 minuti per raggiungere uno stato stazionario in FC e RER.
Il BFR di resistenza viene eseguito bilateralmente su un ergometro da bicicletta stazionario (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Germania) in un'impostazione a intervalli con fasi di carico di 1 serie con 3 intervalli della durata di 2 minuti e 18 secondi ciascuno (corrispondenti al lavoro complessivo svolto in EN); il carico di allenamento è il 50% del PWR mentre il flusso sanguigno dell'arto è limitato al 50% dell'AOP. Tra gli intervalli, le pause sono standardizzate a 1 minuto (cuffia gonfiata). Prima dell'incontro di esercizio, viene eseguito un ciclo di riscaldamento (senza BFR) al 30% di PWR per 5 minuti per raggiungere uno stato stazionario in FC e RER.
Misure:
Durante l'esercizio, gli investigatori raccolgono dati respiro per respiro (Ergostik, Geratherm Respiratory GmbH, Bad Kissingen, Germania). Inoltre, vengono raccolti i dati continui di SpO2 e FC tramite una sonda del lobo dell'orecchio e la risposta BP immediatamente dopo i set di esercizi

- Dolore durante l'esercizio di qualsiasi origine
- Gravidanza
- Storia di evento tromboembolico nell'arto inferiore
- Pressione arteriosa sistolica a riposo <100 mmHg

- Età ≥ 18 anni
- BPCO diagnosticato secondo le linee guida GOLD12
Criteri di esclusione:

- Esacerbazione acuta o recente (entro le ultime 6 settimane) di BPCO

BFR-CYC: Three intervals of 2 minutes at 50% PWR, separated by 1-minute recovery periods, with bilateral thigh cuffs inflated to 50% LOP during exercise intervals. Control: Time- and structure-matched protocol (3 × 2 minutes) without blood flow restriction at 65% PWR, with identical recovery periods. |
BFR-CYC: Three intervals of 2 minutes at 50% PWR, separated by 1-minute recovery periods, with bilateral thigh cuffs inflated to 50% LOP during exercise intervals.

Both COPD patients and healthy participants will complete two resistance exercise conditions in a randomized, crossover design:
Control: Bilateral leg press at 40% 1RM, 3 sets of 15 repetitions, without blood flow restriction. |
BFR-RE: Unilateral right-leg press at 40% of one-repetition maximum (1RM) with cuffs inflated to 80% LOP.

Patients with COPD will complete three cycling exercise conditions in a randomized, crossover design:

BFR-CYC: Continuous cycling at 50% peak work rate (PWR) with bilateral thigh cuffs inflated to 50% limb occlusion pressure (LOP).
A maximum duration of 30 minutes is applied as a safety limit.

Healthy participants will complete two BFR resistance exercise conditions in a randomized, crossover design:

Participants will perform unilateral right-leg press exercise at 40% 1RM using a standardized protocol of 4 sets (30-15-15-15 repetitions) with fixed rest intervals. The protocol will be completed as prescribed (not to volitional exhaustion) unless predefined safety criteria require early termination. |

Maximal voluntary contraction (MVC) of the knee extensors will be assessed before and immediately after unilateral leg press exercise performed with blood flow restriction at 40% and 80% limb occlusion pressure (LOP).
MVC will be measured using an isometric setup, and voluntary activation will be evaluated using the twitch interpolation technique with supramaximal electrical stimulation.
The primary outcome is the change in MVC from pre- to post-exercise, reflecting exercise-induced neuromuscular fatigue.
