Die akute kardiorespiratorische Reaktion auf eine eingeschränkte Durchblutung im Vergleich zu herkömmlichen Trainingsprogrammen (CaRe BFR): 4 Randomisierte Crossover-Studien
Die Forscher gehen von der Hypothese aus, dass BFR-Trainingsprogramme im Vergleich zu herkömmlichen Trainingsprogrammen zu einem anderen akuten kardiorespiratorischen Reaktionsmuster führen. Darüber hinaus stellen die Forscher die Hypothese auf, dass sich diese Muster zwischen gesunden Teilnehmern und Teilnehmern mit COPD unterscheiden.
In Bezug auf das sekundäre Ziel stellen die Forscher die Hypothese auf, dass BFR im Vergleich zu herkömmlichem Bewegungstraining sowohl bei gesunden als auch bei COPD-Teilnehmern zu niedrigeren Blutdruckreaktionen führt.

Die Prüfärzte führen ein monozentrisches Projekt mit 4 randomisierten Crossover-Studien durch.
CaRe-BFR Gesund und COPD:
Krafttraining wird bilateral auf einer Leg Press-Maschine (Leg Press VR2, Cybex International Inc. Medway, MA, USA) unter Verwendung der Drei-Satz-Methode durchgeführt, die sich als am effektivsten zur Steigerung der Muskelkraft erwiesen hat.16 Jeder Satz wird bis zum Muskelversagen ausgeführt, das innerhalb von 8-12 Wiederholungen erreicht werden sollte. Der Trainingsrhythmus wird auf 1-0-1-0 eingestellt (d. h. eine Sekunde für die konzentrische Phase, keine Pause, eine Sekunde für die exzentrische Phase, keine Pause) und von außen durch ein Metronom stimuliert. Die Trainingsbelastung beträgt 80 % des 1RM. Die Pause zwischen den Sätzen beträgt 1 Minute und die Teilnehmer bleiben
Krafttraining BFR wird bilateral auf einer Leg Press-Maschine (Leg Press VR2, Cybex International Inc. Medway, MA, USA) gemäß evidenzbasierten Anwendungsrichtlinien durchgeführt, um die effektivste Methode zur Steigerung der Muskelkraft anzuwenden.17 Die Übung besteht aus insgesamt 75 Wiederholungen in 4 Sätzen. Der Trainingsrhythmus wird auf 1-0-1-0 eingestellt (d. h. eine Sekunde für die konzentrische Phase, keine Pause, eine Sekunde für die exzentrische Phase, keine Pause) und von außen durch ein Metronom stimuliert. Satz 1 umfasst 30 Wiederholungen, die folgenden Sätze je 15 Wiederholungen. Die Trainingsbelastung beträgt 30 % des 1RM. Die Manschetten werden beidseitig am proximalsten Teil des Oberschenkels angebracht und auf 70 % des individuellen AOP aufgepumpt. Die Pause zwischen den Sätzen beträgt 1 Minute und die Teilnehmer bleiben mit aufgeblasenen Manschetten sitzen.
-Ausdauer wird bilateral auf einem stationären Fahrradergometer (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Deutschland) im Intervallsetting mit Belastungsphasen von 1 Satz mit 3 Intervallen à 2 Minuten durchgeführt; Die Trainingsbelastung beträgt 65 % der Peak Work Rate (PWR). Zwischen den Intervallen sind Pausen auf 1 Minute standardisiert. Vor dem Trainingskampf wird ein 5-minütiges Aufwärmradfahren bei 30 % PWR durchgeführt, um einen stabilen Zustand von HF und RER zu erreichen.
Die Ausdauer-BFR wird bilateral auf einem stationären Fahrradergometer (kardiomed 521, proxomed Medizintechnik GmbH, Alzenau, Deutschland) in einem Intervallsetting mit Belastungsphasen von 1 Satz mit 3 Intervallen von je 2 Minuten und 18 Sekunden (entsprechend der Gesamtbelastung) durchgeführt EN); Die Trainingsbelastung beträgt 50 % des PWR, während die Durchblutung der Extremitäten auf 50 % des AOP beschränkt ist. Zwischen den Intervallen sind Pausen auf 1 Minute standardisiert (Manschette aufgepumpt). Vor dem Trainingskampf wird ein Aufwärm-Radfahren (ohne BFR) bei 30 % PWR für 5 Minuten durchgeführt, um einen stabilen Zustand von HF und RER zu erreichen.
Messungen:
Während der Belastung sammeln die Untersucher Atemzug-für-Atemzug-Daten (Ergostik, Geratherm Respiratory GmbH, Bad Kissingen, Deutschland). Darüber hinaus werden kontinuierliche SpO2- und HR-Daten über eine Ohrläppchensonde und die BD-Reaktion unmittelbar nach den Trainingssätzen erfasst

- Systolischer Ruheblutdruck < 100 mmHg

Both COPD patients and healthy participants will complete two endurance exercise conditions in a randomized, crossover design:
BFR-CYC: Three intervals of 2 minutes at 50% PWR, separated by 1-minute recovery periods, with bilateral thigh cuffs inflated to 50% LOP during exercise intervals. Control: Time- and structure-matched protocol (3 × 2 minutes) without blood flow restriction at 65% PWR, with identical recovery periods. |
BFR-CYC: Three intervals of 2 minutes at 50% PWR, separated by 1-minute recovery periods, with bilateral thigh cuffs inflated to 50% LOP during exercise intervals.

Experimental: Resistance Exercise - COPD and Healthy Participants (BFR vs Control, Fixed Protocol)
Both COPD patients and healthy participants will complete two resistance exercise conditions in a randomized, crossover design:
Control: Bilateral leg press at 40% 1RM, 3 sets of 15 repetitions, without blood flow restriction. |
BFR-RE: Unilateral right-leg press at 40% of one-repetition maximum (1RM) with cuffs inflated to 80% LOP.

Patients with COPD will complete three cycling exercise conditions in a randomized, crossover design:

BFR-CYC: Continuous cycling at 50% peak work rate (PWR) with bilateral thigh cuffs inflated to 50% limb occlusion pressure (LOP).
A maximum duration of 30 minutes is applied as a safety limit.

Experimental: Resistance Exercise - Healthy Participants (LOP Comparison Arm)
Healthy participants will complete two BFR resistance exercise conditions in a randomized, crossover design:

Participants will perform unilateral right-leg press exercise at 40% 1RM using a standardized protocol of 4 sets (30-15-15-15 repetitions) with fixed rest intervals. The protocol will be completed as prescribed (not to volitional exhaustion) unless predefined safety criteria require early termination. |

Zeitfenster: Pre-exercise and immediately post-exercise (within minutes after completion of the exercise protocol)

Maximal voluntary contraction (MVC) of the knee extensors will be assessed before and immediately after unilateral leg press exercise performed with blood flow restriction at 40% and 80% limb occlusion pressure (LOP).
MVC will be measured using an isometric setup, and voluntary activation will be evaluated using the twitch interpolation technique with supramaximal electrical stimulation.
The primary outcome is the change in MVC from pre- to post-exercise, reflecting exercise-induced neuromuscular fatigue.
