ARRIÈRE-PLAN:
L'étude sur la santé des infirmières a commencé en 1976, lorsque 121 700 infirmières autorisées vivant dans onze États ont rempli un questionnaire postal comprenant des éléments sur leurs antécédents médicaux, leur régime alimentaire et d'autres facteurs de risque de cancer. L'Institut national du cancer a financé les trois premières années de l'étude et les années suivantes sur les données relatives au cancer. En 1980, le National Heart, Lung, and Blood Institute a commencé à financer la partie cardiovasculaire de l'étude. Des questionnaires de suivi ont ensuite été administrés tous les deux ans. Des échantillons de sang ont été prélevés sur un sous-échantillon de femmes entre 1989 et 1990 et stockés pour analyse de l'ADN germinal et des biomarqueurs circulants.
La composante cardiovasculaire est actuellement financée jusqu'en mars 2022.
NARRATIF DE CONCEPTION :
En 1976, 121 700 infirmières âgées de 30 à 55 ans ont rempli et renvoyé le questionnaire initial de l'étude sur la santé des infirmières et constituent la cohorte de l'étude sur la santé des infirmières. La cohorte a été suivie au moyen de questionnaires envoyés tous les deux ans. Au total, 109 413 participants ont répondu au questionnaire de 1978. De plus, 400 femmes qui ont répondu en 1976 ont déclaré ne pas vouloir continuer l'étude et 390 femmes sont décédées. En 1980, la première évaluation alimentaire majeure a été ajoutée au questionnaire. Dans l'intervalle 1978-1980, il y a eu 624 décès, 300 femmes ont refusé de participer, environ 99 000 ont rempli le questionnaire complet, y compris le questionnaire alimentaire, et 4 000 autres ont rempli un questionnaire abrégé, pour un total d'environ 103 000 réponses. En 1982, la réponse globale était de 101 174. Les quelque 19 000 non-répondants vivants ont été suivis par téléphone, soit 71 % des non-répondants. Ainsi, à la fin de 1982, des informations courantes étaient disponibles sur 116 150 ou 95,4 % de la cohorte initiale. Un sous-ensemble de 32 826 femmes ont fourni des échantillons de sang entre 1989 et 1990. Le génotypage de 1 186 femmes (382 atteintes de maladie coronarienne et 804 témoins appariés) a été réalisé à l'aide de la plateforme Affymetrix 6.0.
Les décès dans la cohorte étaient généralement signalés par les plus proches parents ou les autorités postales. À la fin de chaque cycle d'envoi, l'Index national des décès a été recherché pour les noms des non-répondants qui pourraient être décédés. En comparant les décès constatés à partir de sources indépendantes, l'étude a constaté environ 98% des décès. Des certificats de décès ont été obtenus auprès des services de statistiques de l'état civil pour confirmer tous les décès signalés. Pour tous les certificats de décès indiquant une éventuelle maladie cardiovasculaire, l'autorisation d'obtenir des informations complémentaires a été demandée aux membres de la famille. Le décès confirmé par maladie coronarienne nécessitait des informations supplémentaires en plus du certificat de décès, telles que des rapports d'autopsie, des modifications de l'ECG et des enzymes de l'infarctus du myocarde avant le décès, des douleurs thoraciques classiques immédiatement avant le décès, un infarctus du myocarde ou une angine de poitrine documentés, ou un cathétérisme cardiaque antérieur montrant maladie.
Les données ont été recueillies sur la date de naissance, le poids, le tabagisme, le statut ménopausique et l'utilisation provisoire actuelle de l'hormonothérapie ménopausique. Des données ont également été recueillies sur les cas incidents d'infarctus du myocarde non mortel, d'accident vasculaire cérébral, d'embolie pulmonaire et d'angine de poitrine. Ces items figuraient sur chaque questionnaire. De plus, les paramètres nutritionnels ont été évalués par un questionnaire auto-administré semi-quantitatif de fréquence alimentaire en 1986 puis en 1990.
L'étude a été renouvelée en 1993 avec une attention particulière accordée aux antioxydants alimentaires, à d'autres facteurs nutritionnels, à l'activité physique, à la distribution régionale des graisses, à la thérapie œstrogénique et progestative ménopausique et aux marqueurs biochimiques, notamment les lipides plasmatiques et les apoprotéines. Le suivi des événements de maladies cardiovasculaires non mortelles est supérieur à 92 % pour les 121 700 participants d'origine. Les événements cardiovasculaires mortels sont documentés par des certificats de décès et confirmés et classés par l'examen des dossiers hospitaliers, des rapports d'autopsie et des entretiens avec les plus proches parents. Les recherches de l'Index national des décès pour tous les non-répondants garantissent l'identification des décès restants, ce qui se traduit par un suivi de la mortalité qui est complet à plus de 98 %.
L'étude a été renouvelée au cours de l'exercice 2002 pour poursuivre le suivi jusqu'en mars 2007. L'un des principaux objectifs de ce renouvellement était de comparer la capacité prédictive de plusieurs marqueurs biochimiques et génétiques de l'inflammation et de l'activation endothéliale des maladies coronariennes par rapport aux accidents vasculaires cérébraux chez la femme : protéine C-réactive (CRP), E-sélectine, molécule d'adhésion intercellulaire-1, l'endothéline-1 et les polymorphismes des gènes CRP et E-sélectine. L'étude poursuit également l'enquête sur les déterminants du mode de vie des maladies cardiovasculaires, y compris l'hormonothérapie ménopausique (dose, formulation et durée d'utilisation) et la consommation d'alcool (dose et type de boisson), dans la cohorte complète, et les interactions de ces expositions avec les facteurs ci-dessus. biomarqueurs et avec de nouveaux marqueurs génétiques (polymorphismes des gènes de la prothrombine et de l'alcool déshydrogénase-3). Les génotypes de 1 186 femmes typés dans le cadre d'une étude d'association à l'échelle du génome des maladies coronariennes ont été utilisés pour étudier les facteurs génétiques associés aux maladies cardiovasculaires et l'interaction des gènes et de l'alimentation sur le risque cardiovasculaire. Les cycles de renouvellement ultérieurs comprenaient l'étude des adipokines plasmatiques et des prédicteurs métabolomiques, y compris les métabolites de la flore intestinale tels que le N-oxyde de triméthylamine, la choline et la L-carnitine, de la maladie coronarienne. Le cycle de financement actuel comprend un profilage métabolomique complet des cas et des contrôles de maladies coronariennes et le développement de scores de risque métabolomique pour les maladies coronariennes, avec un financement jusqu'en mars 2022.

La coronaropathie est définie comme un infarctus du myocarde non mortel et un décès par maladie coronarienne.
Les informations sur le résultat sont évaluées par l'examen des dossiers médicaux des événements signalés sur des questionnaires biennaux ou par des recherches dans l'Index national des décès, et la date de l'événement coronarien ou du décès coronarien est déterminée à partir de ces sources.
