Non sono mai stati condotti studi di farmacocinetica (PK) sul DHEA nell'asma in generale e nell'asma grave in particolare.
In molte malattie, la PK nei soggetti con la malattia differisce da quella dei soggetti di controllo e da quelli con altre condizioni.
Pertanto, i ricercatori stanno studiando se i livelli PK di DHEA a rilascio lento sono diversi nei soggetti con asma grave che hanno i fenotipi HSD3B1 AA o AC

Criterio di inclusione:
- Maschio o femmina adulto di età compresa tra i 18 ei 50 anni al momento dell'iscrizione
- Evidenza di asma dimostrata dalla reversibilità del broncodilatatore o dalla risposta alla metacolina durante il rodaggio o dall'evidenza storica di uno dei criteri se il test è stato eseguito secondo lo standard tecnico ERS 2017 o le linee guida ATS 1999 o studi esterni, a condizione che i set completi di flusso i loop di volume sono stati rivisti e approvati dal PI. Questi criteri sono definiti come uno dei seguenti: aumento del FEV1 inferiore o uguale al 12% e 200 ml dopo un massimo di 8 boccate di salbutamolo; metacolina positiva definita come PC20 maggiore o uguale a 16 mg/ml, o PD20 maggiore o uguale a 400 mcg; diagnosi medica documentata di asma grave secondo le linee guida NHLBI; Uso costante di un inalatore ICS/LABA per i 2 mesi precedenti; Non fumatore; le femmine non devono essere in gravidanza o in allattamento; assenza di comorbidità non allergiche; concentrazione basale di DHEA-S < 45 μg/dL nelle femmine e < 90 μg/dL nei maschi; test del genotipo positivo per la variante specifica HSD3B1 AA o AC
Criteri di esclusione:
- Incinta o attivamente cercando di rimanere incinta; allattamento al seno
- test di gravidanza urinario positivo
- Malattia polmonare nota diversa dall'asma
- Dispnea acuta (non correlata all'asma), malattia respiratoria virale o esacerbazione dell'asma entro 4 settimane dallo screening
- Dosaggio sistemico di glucocorticoidi per il mantenimento >10 mg/giorno di prednisone o equivalente
- Pazienti con significative comorbilità non allergiche (ad es. paralisi cerebrale, malattie cardiache, malattie renali, malattie del fegato, ecc.)
- Pazienti con qualsiasi anomalia endocrina centrale o periferica nota come pubertà precoce o diabete
- Pazienti con qualsiasi precedente reazione avversa nota al DHEA
- Fumatore attuale o anamnesi del pacchetto > 5 anni (inclusi dispositivi di svapo/inalazione di nicotina)
- Test di cotinina nelle urine positivo (> 100 mg/mL)
- Uso di prednisone o antibiotici nelle ultime 4 settimane
- Uso di qualsiasi sostanza dopante nelle ultime 2 settimane
- Uso di DHEA nelle ultime 2 settimane
- Uso di androgeni per qualsiasi motivo
- Fenotipo HSD3B1 CC
- Qualsiasi altra condizione o riscontro che possa compromettere la sicurezza del soggetto o la qualità dei dati dello studio, o comunque interferire con il raggiungimento degli obiettivi dello studio, come determinato dal PI
- Amenorrea della menopausa per anamnesi
- PSA positivo (>4 ng/ml) (antigene prostatico specifico)
- Pazienti in trattamento biologico per l'asma
- Diagnosi precedente di disfunzione delle corde vocali, displasia broncopolmonare, fibrosi cistica, broncopneumopatia cronica ostruttiva o altra malattia polmonare
- Pressione arteriosa sistolica > 150 mm Hg e/o pressione arteriosa diastolica > 90 mm Hg
- Frequenze cardiache al di fuori dell'intervallo da 50 a 120 battiti al minuto o con un'irregolarità patologica
- Pazienti affetti da qualsiasi patologia aggiuntiva acuta o cronica che, a giudizio del medico di screening, li renda inadatti allo studio o aumenti i rischi associati allo studio.

Altro: Slow Release DHEA
This is a single-arm, sequential study.
The single arm will start with a one-time 50mg dose visit followed by a washout period.
Next is a 100mg dose visit followed by a washout.
Next is a twice daily 50mg dosing for 3 days, every 12 hours, followed by a washout.
Finally, this is followed by a twice daily 100mg dosing for 3 days, every 12 hours, followed by a washout.
Study cohort will be 9 subjects with asthma.
DHEA dose will be 50 mg via slow release capsules.
Endpoints will be serum DHEA and DHEA-S levels at 10, 20, 30, 60 min & 2, 4, 6, 8, 12h after administration.

The objective is to evaluate the safety and tolerability of slow release DHEA in asthma by measuring the change in physical symptoms from before treatment to after treatment.
Physical symptoms will include reports such as cough, shortness of breath, chest tightness, wheeze, as measured by symptom diary record.

The objective is to evaluate the safety and tolerability of slow release DHEA in asthma patients by performing pulmonary function testing to obtain measurements of FEV (forced expiratory volume) before treatment and after treatment.
All subjects completed the treatment, but not all subjects participated in the pulmonary function testing.
