高リスク軟部肉腫に対する術前補助テモゾロミドベースの陽子線化学放射線療法を調査するパイロット研究
この研究の目的は、軟部肉腫を対象とした治療におけるテモゾロミドと放射線の併用の効果を評価することです。
この研究では、将来の軟部肉腫の治療に役立つ可能性のある情報を得るために、最初の生検と最終手術で切除された腫瘍組織も調べます。 さらに、研究者は、治療を開始する前に、口/頬の内側から正常組織のサンプル(オプション)を検査します(腫瘍組織と比較するため)。

- 患者は、登録前に局所性高リスク軟部肉腫 (STS) の組織学的および X 線写真による証明が必要です。
この研究の目的では、高リスク STS は次の危険因子のうち少なくとも 2 つによって定義されます。
- MRIまたはCTで腫瘍の最大直径が5cm以上
- 組織学グレード 3 または 4 (AJCC、第 7 版)
- 埋没筋膜の深部にある腫瘍
- 原発部位への放射線療法を行っていない局所再発肉腫
- 年齢 > 55 歳
- 腫瘍は、断面画像によって定義されるように、潜在的に切除可能であると考えられなければなりません。
- 以前の手術またはホルモン療法から少なくとも 2 週間が経過している必要があります。 患者は、抗がん療法による以前の治療によるグレード 1 以下の急性毒性を有していなければなりません (最新の治療前に記録されたベースライン状態に戻っている)。 以前の治療による持続的で安定した慢性毒性がグレード 1 以下の患者が対象となります。
- 同意時の年齢は 18 歳以上。
- 医師は余命が3か月を超えると記録した。
患者は、以下に定義されているように、治験治療前の 4 週間以内に正常な臓器および骨髄機能を有している必要があります。
- ヘモグロビン ≥ 9 g/dL
- 白血球 ≥3,000/マイクロリットル (mcL)
- 好中球絶対数 ≥1,500/mcL
- 血小板 ≥100,000/mcL
- 総ビリルビン ≤ 制度上の正常上限値
- アスパラギン酸アミノトランスフェラーゼ (AST/SGOT) およびアラニン アミノトランスフェラーゼ (ALT/SGPT) < 2.5 x 制度上の正常上限値
- クレアチニンが正常値の上限(ULN)の1.5倍以内
- 妊娠の可能性のある女性は、研究参加前、研究参加期間中および治療後12か月間、適切な避妊(ホルモンまたはバリア法による避妊、禁欲)を行うことに同意しなければなりません。 患者は、この試験での治療を開始する前14日以内に血清妊娠検査が陰性でなければなりません。 性的に活発な男性は、適切な避妊法を使用しなければならず、この研究期間中に治療を受けている間は子供を産むべきではありません。
- 研究関連の手順または研究治療の前に、治験審査委員会(IRB)のスタンプが押された、研究固有のインフォームドコンセント文書を理解し、署名する能力。
除外基準:
- 治験エージェントの受け入れ。
- -この研究での治療を開始する前の4週間以内に細胞傷害性薬剤による治療および/または生物学的薬剤による治療。
- 転移性疾患の証拠。
- 患部への以前の放射線治療。
- 進行中または活動性の感染症、症候性うっ血性心不全、不安定狭心症、制御されていない不整脈、および/または研究要件の遵守を制限する精神疾患/社会的状況を含むがこれらに限定されない、制御されていない併発疾患。
- 治験薬には催奇形性または流産促進作用の可能性があるため、妊婦はこの研究から除外されています。 これらの薬剤による母親の治療に続発して授乳中の乳児に有害事象が起こる可能性は不明ですが、潜在的なリスクがあるため、母親が治療を受けている場合は母乳育児を中止する必要があります。
- HIV 陽性患者は、骨髄抑制療法で治療された場合に致死的感染症のリスクが高く、抗レトロウイルス療法と治験薬との間に潜在的な薬物動態学的相互作用があるため、対象外となります。
- 研究期間中はすべてのハーブ薬および/または代替薬を中止する必要があります。これらには、Hydrastis canadensis (ゴールデンシール)、Uncaria tomentosa (キャッツクロー)、または Echinacea angustifolia が含まれますが、これらに限定されません。
- 免疫抑制剤または慢性ステロイドによる治療が必要。
- テモゾロミドの成分またはダカルバジン(DTIC)に対する過敏反応の病歴。

陽子線照射を週 5 日、5 週間行う。標準手術は化学放射線療法の完了後 4 ~ 6 週間後に行われる。
