Chemioradioterapia protonica per i sarcomi dei tessuti molli ad alto rischio (SA02)
Uno studio pilota che indaga la chemioradioterapia protonica neoadiuvante basata su temozolomide per i sarcomi dei tessuti molli ad alto rischio
L'obiettivo di questo studio è valutare gli effetti della combinazione di temozolomide e radiazioni nel trattamento previsto dei sarcomi dei tessuti molli.
Questo studio esaminerà anche il tessuto tumorale che è stato rimosso durante la biopsia iniziale e l'intervento chirurgico finale per informazioni che potrebbero aiutare a trattare il sarcoma dei tessuti molli in futuro. Inoltre, gli investigatori esamineranno un campione del tessuto normale (facoltativo) dall'interno della bocca/guancia (per confrontarlo con il tessuto tumorale) prima di iniziare il trattamento.

Descrizione
Criterio di inclusione:
- I pazienti devono avere una prova istologica e radiografica di sarcoma localizzato dei tessuti molli ad alto rischio (STS) richiesta prima della registrazione.
Ai fini di questo studio, STS ad alto rischio è definito da almeno due dei seguenti fattori di rischio:
- Tumore ≥ 5 cm di diametro massimo alla RM o alla TC
- Istologia grado 3 o 4 (AJCC, 7a edizione)
- Tumore in profondità alla fascia che investe
- Sarcoma localmente ricorrente senza precedente radioterapia al sito primario
- Età >55 anni
- Il tumore deve essere considerato potenzialmente resecabile come definito dall'imaging in sezione trasversale.
- Devono essere trascorse almeno 2 settimane da qualsiasi precedente intervento chirurgico o terapia ormonale. I pazienti devono presentare tossicità acute ≤ grado 1 di qualsiasi trattamento precedente con modalità antitumorali (tornati allo stato basale come indicato prima del trattamento più recente). Sono ammissibili i pazienti con tossicità croniche persistenti e stabili derivanti da un precedente trattamento ≤ grado 1.
- Età ≥18 anni al momento del consenso.
- Il medico ha documentato un'aspettativa di vita superiore a 3 mesi.
I pazienti devono avere una normale funzionalità degli organi e del midollo entro 4 settimane prima del trattamento in studio come definito di seguito:
- emoglobina ≥ 9 g/dL
- leucociti ≥3.000/microlitro (mcL)
- conta assoluta dei neutrofili ≥1.500/mcL
- piastrine ≥100.000/mcL
- bilirubina totale ≤ limite superiore istituzionale della norma
- aspartato aminotransferasi (AST/SGOT) e alanina aminotransferasi (ALT/SGPT) < 2,5 x limite superiore istituzionale del normale
- creatinina entro 1,5 volte il limite superiore della norma (ULN)
- Le donne in età fertile devono accettare di utilizzare una contraccezione adeguata (metodo di controllo delle nascite ormonale o di barriera; astinenza) prima dell'ingresso nello studio e per la durata della partecipazione allo studio e per 12 mesi dopo il trattamento. Le pazienti devono avere un test di gravidanza su siero negativo entro 14 giorni prima dell'inizio del trattamento in questo studio. Anche gli uomini sessualmente attivi devono utilizzare un metodo contraccettivo appropriato e non devono generare un figlio durante la terapia durante questo studio.
- Capacità di comprendere e disponibilità a firmare un documento di consenso informato specifico dello studio e timbrato dall'Institutional Review Board (IRB) prima di qualsiasi procedura correlata alla ricerca o trattamento dello studio.
Criteri di esclusione:
- Ricevere eventuali agenti investigativi.
- Trattamento con agenti citotossici e/o trattamento con agenti biologici nelle 4 settimane precedenti l'inizio del trattamento in questo studio.
- Evidenza di malattia metastatica.
- Precedente radioterapia al sito interessato.
- Malattie intercorrenti non controllate incluse, ma non limitate a, infezione in corso o attiva, insufficienza cardiaca congestizia sintomatica, angina pectoris instabile, aritmia cardiaca incontrollata e/o malattie psichiatriche/situazioni sociali che limiterebbero la conformità ai requisiti dello studio.
- Le donne in gravidanza sono escluse da questo studio perché gli agenti sperimentali possono avere potenziali effetti teratogeni o abortivi. Poiché esiste un rischio sconosciuto ma potenziale di eventi avversi nei lattanti secondari al trattamento della madre con questi agenti, l'allattamento al seno deve essere interrotto se la madre è in trattamento.
- I pazienti HIV positivi non sono ammissibili perché questi pazienti sono ad aumentato rischio di infezioni letali se trattati con terapia soppressiva del midollo e la potenziale interazione farmacocinetica tra la terapia antiretrovirale e gli agenti sperimentali.
- Tutti i farmaci a base di erbe e/o alternativi devono essere interrotti durante lo studio, questi includono ma non sono limitati a: Hydrastis canadensis (goldenseal), Uncaria tomentosa (artiglio di gatto) o Echinacea angustifolia.
- Requisito per il trattamento con agenti immunosoppressori o steroidi cronici.
- Una storia di reazione di ipersensibilità a uno qualsiasi dei componenti della temozolomide o alla dacarbazina (DTIC).

Proton Chemioradioterapia seguita da intervento chirurgico.
Temozolomide cinque giorni alla settimana durante la radioterapia per 5 settimane.
Radiazione protonica cinque giorni alla settimana per 5 settimane La chirurgia standard avrà luogo 4-6 settimane dopo il completamento della chemioradioterapia.

ACTIVE_COMPARATORE: Chemioradioterapia Temozolomide
Chemioradioterapia Temozolomide: 75 mg/m2 cinque giorni alla settimana durante la radioterapia per 5 settimane.
La temozolomide deve essere assunta per via orale 1 ora prima di ogni sessione di radioterapia durante i giorni feriali (dal lunedì al venerdì).
La dose sarà determinata utilizzando l'area di superficie corporea (BSA) calcolata all'inizio del trattamento concomitante.
La BSA sarà calcolata dall'altezza ottenuta alla visita di pretrattamento e dal peso ottenuto prima del primo giorno di trattamento.
Il trattamento concomitante durerà fino alla fine della radioterapia.
