前立腺の少数転移癌における転移指向療法を伴うエンザルタミドおよびADTの期間
これは、エンザルタミドを標準治療の放射線およびホルモン療法に追加して生活の質を改善することを調査するための、多施設、研究者主導、2群の無作為化試験です。
参加者は、標準治療の放射線およびホルモン(ADT)療法(Arm 1)または標準治療の放射線およびホルモン(ADT)療法と経口エンザルタミドを8か月間(Arm 2)受けます。 参加者は、最大 5 年間、クリニックまたはリモートで定期的にフォローアップされます。

- 年齢 > 18 歳

- 前立腺腺癌の組織学的診断
- ECOGパフォーマンスステータス0-2
ステージ IV の去勢感受性の異時性転移性前立腺がん。 -研究登録から6か月以内に1〜10個の転移と診断された
- 従来の画像診断 (CT または骨スキャンまたは MR) で最大 1 つの転移沈着物
- 追加の転移はPSMA PETのみで検出可能
- すべての疾患部位は放射線療法の影響を受けやすく、安全に治療することができます
- 患者は ADT の継続使用を拒否する
除外基準:
- -患者をADT、エンザルタミド、およびSBRTに適さないものにする重大な併存疾患
- -非黒色腫皮膚がんおよび上皮内がんを除く、過去5年以内の悪性腫瘍の病歴 非治癒的に管理された
- -サルベージ全身療法の以前の使用
- 脊髄圧迫の証拠

他の:Arm 1 - Standard of Care
Participants in this group will receive standard of care radiation treatment and ADT.
Participants will start ADT first.
Androgen deprivation therapy will be delivered via injection every 3-4 months as per standard of care (for 8-9 months total).
At the same time, or within 3 months of starting ADT, participants will begin radiation treatment.
The radiation treatment course will consist of 2-5 sessions of radiation daily or every other day (the number of sessions will be determined by the physician depending on the location of cancer, number of cancer spots, etc.).

Participants in this group will receive standard of care radiation treatment and ADT, plus abiraterone.
Participants will start ADT first.

At the same time, or within 3 months of starting ADT, participants will begin radiation treatment.
The radiation treatment course will consist of 2-5 sessions of radiation daily or every other day (the number of sessions will be determined by the physician depending on the location of cancer, number of cancer spots, etc.).
In addition, participants will take abiraterone orally (by mouth) daily for 8-9 months.
Participants will start the first pill within a month of enrollment.
This pill can be taken around the same time of day with or without food.

Participants in this group will receive standard of care radiation treatment and ADT, plus abiraterone and atorvastatin.
Participants will start ADT first.
Androgen deprivation therapy will be delivered via injection every 3 or 4 months as per standard of care (for 8-9 months total).
At the same time, or within 3 months of starting ADT, participants will begin radiation treatment.
The radiation treatment course will consist of 2-5 sessions of radiation daily or every other day (the number of sessions will be determined by the physician depending on the location of cancer, number of cancer spots, etc.).
In addition, participants will take abiraterone orally (by mouth) daily for 8-9 months.
Participants will start the first pill within a month of enrollment.
This pill can be taken around the same time of day with or without food.
Participants will also take atorvastatin once daily by mouth for 2 years.
This may also be taken with or without food.
