도시 환경 및 소아 천식(URECA)
가난한 도시 지역에서 자란 소수 민족 어린이는 천식 발병률이 가장 높으며 이 질병의 급성 악화로 인한 이환율도 더 높습니다.

상세 설명
이 연구의 목적은 환경 요인(예: 도심 가정 먼지의 구성 요소)이 도심 어린이의 면역 체계 발달 및 천식 증상에 영향을 미치는 방식을 결정하는 것입니다. 이 연구는 피험자가 출생부터 10세까지 세 기간으로 나뉩니다. 각 연령층은 서로 다른 목표와 끝점을 탐색합니다.
연구 목적/가설:
0~3세 피험자:
- 도심의 환경적 요인은 면역 체계의 발달에 악영향을 미쳐 3세까지 사이토카인 조절 장애, 알레르기 및 재발성 쌕쌕거림을 촉진합니다.
- 바이러스성 하기도 감염이 있고 3세까지 사이토카인 조절 장애가 발생한 소아는 6세까지 천식 발병 위험이 증가합니다.
4~7세 피험자:
- 어린 시절의 특정 도시 노출에 의해 유발되는 독특한 면역 발달 패턴이 있으며, 이러한 면역 발달 패턴은 1) 항바이러스 반응의 손상 및 2) Th2 유사 반응의 강조(예: 바퀴벌레 특유의 Interleukin-13(IL-13)). 이러한 면역 발달 변화의 임상적 효과는 3-4세까지 빈번한 바이러스 유발 천명 및 알레르기 감작이며, 이러한 특성은 상승적으로 7세에 천식 위험을 증가시킵니다.
7~10세 피험자:
- 시너지 효과를 내는 환경 노출(바퀴벌레 알레르겐, 실내 오염 물질[환경 담배 연기(ETS) 및 이산화질소(NO2)], 미생물 노출 부족) 및 가족 특성(스트레스, 선천적 면역과 관련된 유전적 요인)의 고유한 조합이 있습니다. 도시 지역의 천식 발병, 지속성 및 이환율. 이러한 노출 및 특성은 중요한 성장 기간 동안 면역 발현 및 폐 발달에 영향을 미쳐 특정 천식 표현형을 초래합니다.
10~16세 피험자:
- 출생부터 청소년기에 이르기까지 가난한 도시 지역에서 자라는 소수민족 어린이의 천명, 천식 및 아토피 표현형을 확인합니다.

- 천식, 건초열 또는 습진이 있는 경우(또는 유아의 아버지가 이러한 질병이 있는 경우)
- 현재 미국 정부 빈곤 수준 미만의 가구가 최소 20% 포함된 사전 선택된 지역에 거주
- 분만 시 임신 34주 이상
