질병을 예방하거나 치료하기 위해 임상의가 처방하는 많은 약물은 예상치 못한 독성 효과를 유발할 수 있습니다. 많은 국가 임상 지침에서는 부작용을 피하기 위해 기본 및 주기적인 실험실 모니터링을 권장합니다. 우리의 SIP(Safety in Prescribing) 프로젝트는 약물 안전성에서 많은 중요한 격차를 발견했습니다. Lab Alert는 Kaiser Permanente의 실험실 모니터링 성능을 개선하여 부작용을 방지하기 위한 전자 도구를 평가합니다. 우리는 고위험 약물을 복용하고 실험실 모니터링을 받지 않은 환자를 등록할 것입니다. 이 2년 연구의 주요 결과는 개입 후 1주 및 3주에 지침 기반 실험실 모니터링을 받는 환자의 비율입니다.
15개의 1차 진료 클리닉이 다음과 같이 무작위로 배정됩니다. 미리 알림은 환자가 약물 사용으로 인해 지침에 지정된 검사실 검사가 필요함을 알려줍니다. 전자 데이터를 사용하여 연구 지정 약물을 복용하는 개입 클리닉에 PCP가 할당된 환자를 식별하고 제외 여부를 선별하여 600명의 환자(개입 그룹당 200명)를 생성합니다. 환자는 평소 클리닉에 할당된 조건에 따라 치료 그룹에 할당됩니다. 개입 및 관찰 기간 후, 개입 부문의 환자를 식별하는 데 사용된 동일한 시간 프레임과 포함 및 제외 기준을 사용하여 일반 치료 클리닉에서 비교 대상 200명의 환자 코호트를 후향적으로 식별합니다. 따라서 약 800명의 환자가 연구에 포함될 것입니다. 기준선, 후속 조치 및 결과 데이터는 전자 기록에서 얻을 수 있습니다.
연구 정의 약물은 연구 팀과 품질 위원회에 의해 확정됩니다. 이러한 약물은 이전 작업을 기반으로 식별되고 합리적인 빈도로 처방되며 일반적으로 실험실 모니터링에 차이가 있고 상당한 독성 위험이 있으며 이전 및 잠재적 부작용으로 인해 참여 HMO에 관심이 있습니다. 우리는 새로운 처방에 대한 기본 모니터링에 초점을 맞출 것으로 예상합니다. 새로운 시작은 인덱스 처방전이 있지만 이전 6개월 동안 해당 약물을 다른 조제하지 않은 환자로 정의됩니다.
