상세 설명
회전근개 파열은 50세 이상 인구의 최대 40%에서 발견되며 나이가 들수록 유병률이 증가합니다. 대부분의 사람들은 무증상 상태를 유지하지만 일부 사람들에게 증상이 나타나는 이유는 알려져 있지 않습니다. 증상이 있는 환자의 경우 파열된 회전근개의 외과적 봉합은 환자의 30-50%에서 실패하지만 대부분의 환자는 봉합 실패에도 불구하고 상당한 통증 감소를 보입니다. 비수술적 치료는 환자의 25-82%에서 성공적인 결과를 보고했지만 어떤 환자 관련 기능이 비수술적 치료의 성공을 예측하는지는 알려져 있지 않습니다. 이 연구 노력의 일반적인 목적은 회전근개 비수술적 치료의 성공(시각 아날로그 통증 척도 감소 및 환자 만족도로 정의됨)을 예측할 수 있는 환자 특징(이력 정보, 신체 검사 결과 및 MRI 기반 병리학)을 식별하는 것입니다. 커프 눈물. 제안된 연구 설계는 수준 1 및 수준 2 연구의 증거 기반 의학(EBM) 체계적 검토에서 파생된 표준 물리 치료 프로그램을 따를 회전근 개 파열 환자의 전향적 코호트 연구입니다.

설명
포함 기준:
• 전층 회전근 개 파열의 MRI 소견이 있는 환자(18-100세)
참고: 회전근개 파열을 평가하기 위한 어깨 MRI를 얻기 위한 기준[부록 D]:
- 중대한 약점(위의 지침에 따라 ≥ 2점)
- ADL 통증 또는 야간 통증 >/= VAS에서 7
- 중대한 충돌(위 지침에 따름) + ≥ 3개월 동안 심각한 증상이 있음.
제외 기준:
- 급성 회전근개 파열(일반적으로 고속 부상으로 인해 발생하며 증상은 1개월 미만)
- 관련 탈구
- 관련 골절
- 전신성 류마티스 질환, 즉 류마티스 관절염 및 전신성 홍반성 루푸스
- 양측 회전근개 파열 동시 치료 중인 환자
- 양식을 작성할 수 없는 환자
- 목이나 견갑골의 통증
- 이전 어깨 수술
견상완관절염(아래 기준 중 1개 이상 충족)
- 상완골 머리 또는 glenoid에 osteophytes> 2mm
- 진정한 AP 또는 겨드랑이 방사선 사진에서 보이는 경화증 또는 낭종 형성과 함께 관절강 협착;
- 견봉과 접촉하는 상완골두
- 유착성 관절낭염
