回旋筋腱板断裂は、50 歳以上の人の最大 40% に見られ、有病率は年齢とともに増加します。 ほとんどの人は無症状のままですが、一部の人が症状を発症する理由は不明です. 症候性の患者では、断裂した回旋腱板の外科的修復は 30 ~ 50% の患者で失敗しますが、大部分の患者は修復の失敗にもかかわらず、痛みが大幅に軽減されます。 非手術治療は、患者の 25 ~ 82% で成功した結果を報告していますが、どの患者関連の特徴が非手術治療の成功を予測するかは不明です。 この研究努力の一般的な目的は、回旋筋の非手術治療の成功 (視覚的アナログ疼痛スケールの減少と患者の満足度によって定義される) を予測する患者の特徴 (病歴情報、身体検査所見、および MRI ベースの病理学) を特定することです。袖口の涙。 提案された研究デザインは、レベル 1 およびレベル 2 研究のエビデンスに基づく医学 (EBM) 系統的レビューから得られた標準的な理学療法プログラムに従う回旋筋腱板断裂患者の前向きコホート研究です。

包含基準:
• 回旋筋腱板全層断裂の MRI 所見を有する患者 (18-100 歳)
注: 回旋腱板断裂を評価するために肩の MRI を取得する基準 [付録 D]:
- 重大な弱点 (上記のガイドラインで 2 ポイント以上)。
- ADL の痛みまたは夜間の痛み >/= VAS で 7
- 重大なインピンジメント (上記のガイドラインによる) + 重大な症状が 3 か月以上続く。
除外基準:
- 回旋筋腱板の急性断裂(通常、高速損傷および 1 か月未満の症状が原因)
- 関連転位
- 関連する骨折
- 全身性リウマチ性疾患、すなわち 関節リウマチおよび全身性エリテマトーデス
- 同時に両側腱板断裂の治療を受けている患者
- フォームに記入できない患者
- 首や肩甲骨からの痛み
- 以前の肩の手術
肩甲上腕関節炎 (以下の基準の 1 つ以上を満たす)
- 上腕骨頭または関節窩に2mmを超える骨棘
- 真のAPまたは腋窩X線写真で見られる硬化または嚢胞形成を伴う関節腔の狭窄;
- 上腕骨頭が肩峰に接触
- 癒着性嚢炎
