진행성 비편평 NSCLC에 대해 카보플라틴, 파클리탁셀 및 베바시주맙에 이어 베바시주맙, 페메트렉시드 또는 베바시주맙과 페메트렉시드의 조합을 사용한 유지 요법의 무작위 3상 연구
근거: 화학 요법에 사용되는 파클리탁셀 및 카보플라틴과 같은 약물은 종양 세포를 죽이거나 분열을 멈추는 등 종양 세포의 성장을 멈추기 위해 다양한 방식으로 작용합니다. 베바시주맙과 같은 단클론 항체는 다양한 방식으로 종양 성장을 차단할 수 있습니다. 일부는 종양 세포의 성장 및 확산 능력을 차단합니다. 다른 사람들은 종양 세포를 찾아서 죽이는 것을 돕거나 종양을 죽이는 물질을 종양 세포에 옮깁니다. 베바시주맙은 또한 종양으로 가는 혈류를 차단하여 비소세포 폐암의 성장을 멈출 수 있습니다. - 유도 요법(I군): 환자는 제1일에 3시간에 걸쳐 파클리탁셀 IV, 15-30분에 걸쳐 카보플라틴 IV, 30-90분에 걸쳐 베바시주맙 IV를 투여받습니다. 과정은 질병 진행 또는 허용할 수 없는 독성이 없는 경우 4 과정 동안 21일마다 반복됩니다.
유지 요법: 유도 요법 후 완전 반응, 부분 반응 또는 안정적인 질병을 달성한 환자는 3개의 치료군 중 1개로 무작위 배정됩니다. 치료는 유도 화학 요법 투여 마지막 날로부터 6주 이내에 시작됩니다.
- A군: 환자는 제1일에 30-90분에 걸쳐 베바시주맙 IV를 투여받습니다.
- B군: 환자는 1일차에 10분에 걸쳐 페메트렉시드이나트륨 IV를 투여받습니다.
- C군: 환자는 A군과 마찬가지로 베바시주맙을, B군과 마찬가지로 페메트렉시드를 투여받습니다. 모든 군에서 질병 진행이나 허용할 수 없는 독성이 없는 경우 코스는 21일마다 반복됩니다.
일부 환자는 기준선에서 그리고 상관 연구를 위해 연구 중에 주기적으로 혈액 샘플 수집을 받습니다.
연구 요법 완료 후 환자는 2년 동안 3개월마다, 이후 2-5년 동안 6개월마다 추적 관찰됩니다.

포함 기준(1단계 유도 요법):
- 세포학적 또는 조직학적으로 확인된 비소세포폐암(NSCLC)
- 우세한 비편평 조직학(달리 명시되지 않은 NSCLC는 허용됨). 혼합 종양은 우세한 세포 유형으로 분류됩니다.
- M1a 또는 M1b 병기를 포함한 IV 병기 또는 재발성 질환
- 동측 폐엽이 있는 IIIB기(T4NX) 질환은 환자가 병용 화학 요법 또는 방사선 요법의 대상자가 아닌 경우 허용됩니다.
- 이전 보조 화학 요법 이후 최소 12개월
- 이전 방사선 치료 후 최소 2주
- 보조 화학 요법의 일부로 제공된 경우 사전 카보플라틴 허용
- 뇌전이 환자는 뇌에 대한 국소 치료를 받았어야 하며 등록 전 국소 치료 완료 시점으로부터 최소 2주 동안 뇌에서 진행의 증거가 없어야 합니다.

- 파클리탁셀, 페메트렉세드 디소듐 또는 베바시주맙의 사전 사용
- 지난 4주 이내의 주요 객혈
- 활동성 감염, 증후성 울혈성 심부전, 불안정 협심증, 중증 심부정맥, 연구 요건 준수를 제한하는 정신 질환/사회적 상황을 포함하되 이에 국한되지 않는 조절되지 않는 병발성 질환
- 지난 12개월 이내에 동맥 혈전 사건 또는 주요 출혈의 병력
- 등록 전 6주 이내에 개흉술, 개복술, 개두술 또는 심각한 외상과 같은 대수술. 생검 절차 및 흉관 삽입은 이 프로토콜의 목적상 주요 수술로 간주되지 않습니다.
- 등록 후 7일 이내 핵심 생검
- 지난 6개월 이내에 중대한 혈관 질환(예: 외과적 치료가 필요한 대동맥류 또는 최근의 말초 동맥 혈전증)
- 임상적으로 중요한 심혈관 질환
- 지난 6개월 이내에 복강 누공, 위장 천공 또는 복강 내 농양
- 심각한 치유되지 않는 상처, 궤양 또는 골절의 병력
- 폐의 공동 병변
- 임신 또는 간호
- HIV 감염 환자의 동시 항레트로바이러스 요법
