Studie van de invloed van proprioceptief hergewichtsvermogen op de biomechanica van de onderste extremiteit tijdens functionele taken en ongeplande zijwaartse manoeuvres
Blessures aan de voorste kruisband (VKB) komen vaak voor bij handbal, vooral bij jonge spelers. Recent onderzoek benadrukte de implicatie van het centrale zenuwstelsel als een potentiële risicofactor voor ACL-ruptuur.
Het vermogen om proprioceptieve signalen dynamisch te herwegen op basis van houdingsomstandigheden is cruciaal voor evenwichtscontrole.
Het doel van deze studie is daarom om de invloed van proprioceptieve herweging op biomechanische determinanten van ACL-belastingen tijdens functionele taken en ongeplande zijwaartse manoeuvres te onderzoeken.

Teamhandbal is een traumatische sport, vooral als het gaat om blessures aan de voorste kruisband (VKB). Jonge vrouwen zijn kwetsbaarder omdat ze 3 tot 5 keer meer kans hebben op een VKB-ruptuur dan mannen.
Verschillende anatomische, biomechanische en sensomotorische risicofactoren zijn duidelijk geïdentificeerd, maar de implicatie van het centrale zenuwstelsel is onlangs benadrukt. Het is inderdaad aangetoond dat personen die lijden aan ACL-rupturen een verminderde functionele connectiviteit vertoonden tussen hersengebieden die verantwoordelijk zijn voor houdingsregulatie en sensomotorische verwerking. Vanwege de onverwachte situaties die zich voordeden tijdens spelsituaties, wordt de rol van de hersenen (d.w.z. neurale controle) nu bepleit om sensomotorische fouten te verklaren die leiden tot blessures tijdens complexe taken, zoals het doen alsof van een tegenstander. Spiervibratie is een betrouwbaar hulpmiddel om proprioceptieve integratie tijdens houdingsregulatie te beoordelen. Het vermogen om van de ene proprioceptieve cue naar de andere over te schakelen wanneer de houdingsomstandigheden veranderen, is cruciaal. Dit dynamische herwegingsproces maakt het mogelijk om een optimale houdingscontrole te verkrijgen. Recente onderzoeken hebben echter aangetoond dat dit proces veranderd is bij symptomatische populaties, oudere patiënten of zelfs bij vermoeidheid. Om preciezer te zijn, sommige individuen lijken in staat om proprioceptieve afhankelijkheid te verschuiven, terwijl andere dat niet doen. Voor zover wij weten, hebben geen studies het verband onderzocht tussen proprioceptieve herweging en biomechanische determinanten van ACL-belastingen tijdens functionele taken. Het doel van deze studie is dus om de biomechanica van de onderste ledematen te vergelijken tijdens onverwachte zijwaartse snijmanoeuvres en verticale sprong met één been bij jonge handbalspelers op basis van hun vermogen om proprioceptieve signalen te herwegen.

Beschrijving
Inclusiecriteria:
- Leeftijd van 15 tot 25 jaar
- Intensieve handbaltraining gedurende ten minste twee jaar, het beheersen van het technische gebaar van ongeplande zijwaartse snijmanoeuvre
- Trainingsvolume van minimaal 5 uur per week
- Ondertekening van de toestemming (deelnemers en ouders voor minderjarigen)
Uitsluitingscriteria:
- Recente osteoarticulaire pathologie (d.w.z. minder dan drie maanden) van de onderste ledematen, al dan niet traumatisch
- Ongeschikt om toestemming te geven of te weigeren deel te nemen aan het onderzoek
- Voor de hand liggende evenwichtsstoornis of invaliderende neurologische pathologie
- Pijn van het bewegingsapparaat (gewricht, pees of spier) permanent of tijdens inspanning
- Vermoeidheid (beoordeling met behulp van de Borg-schaal) tijdens het klinisch onderzoek (> 6) voorafgaand aan het uitvoeren van het sportieve gebaar
- Bekende huidallergie voor elk kleefproduct

De proefpersoon bevindt zich in unipodale ondersteuning (slechts één voet op de grond) op de geteste onderste ledemaat in het midden van het platform.
Drie lijnen die een "Y" vormen, worden gerangschikt volgens de onderste ledemaat die de leiding heeft in drie richtingen: anterieur (ANT), posteromediaal (PM) en posterolateraal (PL).
Het doel is dan om de langst mogelijke afstand in alle drie de richtingen te bereiken met de punt van de voet in reliëf alvorens terug te keren naar de startpositie.
De proefpersoon krijgt 4 trainingspogingen per richting op elke onderste ledemaat, daarna worden er 3 pogingen geregistreerd om het gemiddelde te behouden.

De proefpersoon valt van een trede en landt op één been, springt dan zo hoog mogelijk en stabiliseert zich weer op hetzelfde been.
De hoogte van de trede is 30 cm.
De proefpersoon zal 3 opeenvolgende sprongen maken met strikte naleving van de instructies: laat je vallen tot het niveau van de markering op de grond en stuiter zo hoog mogelijk terwijl je zo min mogelijk tijd op de grond doorbrengt.
Tijdens het tweede contact met de grond moet de proefpersoon gedurende 3 seconden stabiliseren, zodat de instructies en metingen reproduceerbaar zijn.

Het doel is om een onverwachte speelsituatie te creëren, dicht bij de dagelijkse acties van de proefpersonen in de praktijk van handbal. De proefpersoon zal een zijwaartse snijmanoeuvre maken voor een tegenstander, gesimuleerd door een dummy die tijdens de gebruikelijke training wordt gebruikt. De proefpersoon zal in een rechte lijn sprinten en vervolgens bij het krachtenplatform een snelle richtingsverandering maken aan de zijkant van zijn schietende arm of in een rechte lijn blijven rennen. Een willekeurig lichtsignaal zal de speler aangeven in welke richting hij zijn manoeuvre moet uitvoeren. Er zal een computerreconstructie van de kinematica en dynamiek (kniemoment) worden uitgevoerd.

De proefpersoon zal worden gevraagd om roerloos in bipodale (beide voeten op de grond) steun op een stabiele en onstabiele ondergrond (schuim) te staan.
Een peesvibratie (80Hz) wordt willekeurig toegepast op het onderwerp in de achillespezen of paravertebrale spieren.
Deze trilling zal een verandering van proprioceptieve informatie in het getrilde gebied veroorzaken, wat leidt tot een verstoring van het houdingsevenwicht.
Aldus wordt, volgens de mate van verplaatsing van het drukpunt (CoP), de proprioceptieve weegverhouding (dRPW) berekend om daaruit het gewicht af te leiden dat door het CZS wordt toegekend aan de verschillende proprioceptieve inputs tijdens de houdingstaak.

De proefpersoon krijgt 4 trainingspogingen per richting op elke onderste ledemaat, daarna worden er 3 pogingen geregistreerd om het gemiddelde te behouden.

De waarden zullen het mogelijk maken om biomechanische kenmerken te vergelijken volgens het proprioceptieve profiel (plastische proefpersonen versus rigide proefpersonen).

Een dRPW van 1 geeft 100% gebruik van informatie uit de enkel aan, terwijl een dRPW van 0 100% gebruik van informatie uit de heup aangeeft.
Zo is het mogelijk om een evolutie van deze dRPW te berekenen tijdens de overgang van een stabiele naar onstabiele bodem.
De "plastic" onderwerpen verminderen hun dRPW op onstabiele grond (vergeleken met stabiel) ("normaal" gedrag).
"Stijve" onderwerpen behouden (of verhogen zelfs) hun dRPW wanneer ze over onstabiele grond gaan.
