Darmmicrobioom en depressie
De rol van het darmmicrobioom als bepalende factor voor depressie bij patiënten met multiple sclerose
Het doel van dit project is om te bepalen of een specifiek darmmicrobioom of van de darm afgeleide metabolieten geassocieerd zijn met depressie bij patiënten met Multiple Sclerose (pwMS). Mechanistisch gezien veronderstellen de onderzoekers verder dat depressie bij pwMS verband houdt met een verminderde overvloed aan darmbacteriën met GABA-producerende activiteiten en/of met ontstekingsremmende eigenschappen. Om te bepalen of de aanwezigheid van depressie bij pwMS verband houdt met een specifiek darmmicrobioom, van de darm afgeleide metabolieten of immuunprofielen van perifeer bloed. De onderzoekers zullen een cross-sectioneel onderzoek uitvoeren in klinisch stabiele pwMS die zijn aangeworven in het John L. Trotter MS Center. De onderzoekers zullen de aanwezigheid van depressie evalueren met behulp van de depressieschaal Quality of Life in Neurological Disorders (Neuro-Qol), een van de 13 schalen in de Neuro-Qol die onlangs is ontwikkeld door de NIH met behulp van moderne psychometrische technieken en gevalideerd in pwMS.
In totaal zullen 120 pwMS worden gerekruteerd: 60 met en 60 zonder depressie op basis van de Neuro-Qol-depressieschaal. Tijdens het studiebezoek zal elke deelnemer worden gevraagd om een ontlastingsmonster te verstrekken voor microbioomanalyses en een bloedmonster voor immunofenotypering van perifeer bloed. Potentiële confounders zullen worden verzameld en behandeld als covariaten in de analyses. Deze omvatten: 1) mate van invaliditeit (EDSS); 2) behandeling met antidepressiva en DMT's; 3) een 4-daags voedingsdagboek om de samenstelling van het dieet te evalueren; 4) gewicht en lengte om de BMI te berekenen; 5) vermoeidheid; 6) niveau van fysieke activiteit; 7) slaapkwaliteit.

DOEL 1. Om te bepalen of de aanwezigheid van depressie bij pwMS verband houdt met een specifiek darmmicrobioom, van de darm afgeleide metabolieten of immuunprofielen van perifeer bloed.

DOEL 1A. Om te bepalen of depressie zal correleren met specifieke darmmicrobioom- of darm-afgeleide metabolietenprofielen in pwMS.
Ontlastingsmonsters zullen worden verwerkt voor microbioomsequencing en metaboloomkarakterisering.
DOEL 1B. Om te bepalen of depressie zal correleren met een specifiek perifeer bloed immuun-inflammatoir profiel bij pwMS.
Van elke deelnemer zal een perifeer bloedmonster worden genomen om het volgende uit te voeren: 1) fenotypering van immuuncellen van perifeer bloed om de belangrijkste subsets van immuuncellen en hun activering te karakteriseren; 2) intracellulaire cytokineproductie om profielen van cytokineproductie van bloedlymfocyten en monocyten te bestuderen.
DOEL 2. De productie van GABA in pwMS met of zonder depressie kwantificeren en de interactie tussen darmmicrobioom en immuunsysteem in vitro bepalen.
Hiertoe zullen de onderzoekers functionele studies uitvoeren om het potentieel van de darmmicrobiota van pwMS met of zonder depressie te evalueren om GABA te produceren en immuun-inflammatoire reacties te moduleren.
DOEL 2A. Om GABA-waarden te kwantificeren in hele ontlasting, specifieke bacteriële isolaten van ontlasting en bloed van pwMS met of zonder depressie.
De onderzoekers zullen de GABA-waarden in de ontlasting en het bloed van pwMS evalueren. Daarnaast zullen de onderzoekers de GABA-productie meten door Bacteroides ssp geïsoleerd uit de darmmicrobiota van pwMS.
Doel 2B. Evalueren van de effecten van hele ontlasting en specifieke bacteriesoorten van pwMS op het fenotype van bloedimmuuncellen en de productie van cytokines.
De onderzoekers zullen testen hoe het hele darmmicrobioom of specifieke bacteriën (geïdentificeerd in doel 1 als geassocieerd met depressie bij pwMS) de immuuncelfunctie kunnen moduleren. Mononucleaire cellen uit perifeer bloed (PBMC) van gezonde donoren zullen worden gekweekt in conditioneringsmedia van hele ontlasting of interessante bacteriën geïsoleerd uit pwMS. PBMC-fenotype en cytokineproductie na blootstelling in vitro zullen worden gekarakteriseerd door middel van flowcytometrie.

- Leeftijd ≥18 jaar
- Diagnose van RRMS of progressieve MS op basis van de herziene McDonald-criteria uit 2017
- Onbehandeld of op een van de MS DMT's zolang deze de afgelopen 3 maanden klinisch stabiel zijn geweest
- Geen voorgeschiedenis van antibioticabehandeling in de 3 maanden voorafgaand aan het studiebezoek en de monsterafname
- Geen andere auto-immuunziekten, chronische stofwisselingsziekten (bijv. diabetes) of aandoeningen (bijv. zwangerschap) die zouden interfereren met de parameters die we gaan meten in de ontlasting en bloedmonsters.
