背景:肾移植(KTx)受者需要终身接受免疫抑制治疗,这会增加严重感染的风险。 血流感染(BSI)在移植后很常见,并且与高死亡率和高发病率相关。 对所有KTx受者进行预防性抗生素治疗并未带来总体益处,但尚未评估针对高BSI风险的KTx受者采用靶向抗生素的个性化预防性治疗策略。

背景 据估计,丹麦有超过50万成年人患有慢性肾脏病。 慢性肾脏病可能进展,其最严重表现是肾衰竭。 由于较其他治疗方案(包括透析)具有更长的预期寿命,肾移植(KTx)是肾衰竭患者的首选治疗方法
(1)。 丹麦近3500人依靠移植肾生活,每年有250-300人接受肾移植。 然而,肾移植存在同种异体移植物排斥的风险,超过20%的肾移植受者会发生排斥反应,可能导致移植失败(2)。 为降低排斥风险,肾移植受者需终身服用免疫抑制药物,这反过来又增加了感染风险(3)。
感染在肾移植后很常见,且可能是严重并发症。 因此,肾移植后第一年任何感染的发生率超过80%,感染是导致死亡的主要原因(4,5)。 血流感染(BSI)因高死亡率而特别受关注,且与移植失败风险增加相关(6,7)。 研究者先前发现,肾移植受者在移植后第一年的BSI累积发生率为11.8%,其中30天BSI常继发于尿路感染,主要由肠杆菌目细菌引起(6,8,9)。 尿路感染是肾移植受者最常见的感染并发症,超过20%的肾移植受者有复发性尿路感染(10)。 研究者和他人先前发现尿路感染、高龄、合并症、急性排斥反应和中性粒细胞减少是移植后BSI的风险因素(7,8)。 为预防肾移植受者的BSI,有人建议进行菌尿筛查。 然而,一项纳入112名肾移植受者的随机临床试验显示,普遍筛查菌尿和系统治疗无症状菌尿对肾盂肾炎发生率无影响(11)。 此外,美国移植学会(AST)感染性疾病实践社区建议将肾移植受者无症状菌尿的筛查限制在移植后第一个月,因缺乏疗效证据(12)。 总之,普遍筛查菌尿无效,迫切需要预防肾移植受者BSI的新方法。 我们将通过基于风险的方法来解决这一知识缺口,识别并治疗高风险个体。

干预 肾移植后第一年的BSI主要由肠杆菌目细菌引起(6,8,9),在我们中心,超过65%的BSI由肠杆菌目引起(图2B, 8)。 丹麦的首选治疗是覆盖革兰阴性菌的β-内酰胺类抗生素,如匹美西林。 尽管超广谱β-内酰胺酶(ESBL)问题日益严重,但我们中心超过80%的肠杆菌目对匹美西林敏感。 因此,在本研究中,研究者选择匹美西林作为干预措施。 干预时长基于Rigshospitalet 2010-2019年肾移植BSI发生率确定。 干预将在移植后一个月开始,因为许多肾移植受者在移植后立即接受预防性抗生素治疗,且第一个月病原体组成更多样,对匹美西林敏感的细菌较少(8)。
在移植后一个月,150名高风险受者将按1:1随机分配至:
- 干预组(n=75),将在移植后第1-6个月每天一次接受400毫克匹美西林。
- 对照组(n=75),将在移植后第1-6个月每天一次接受安慰剂。
结局
主要结局是移植后第1-6个月期间的肠杆菌目BSI。 次要结局包括以下内容,除非另有说明,均在移植后6个月评估:
- 严重结局:全因死亡率、住院以及移植失败或移植功能(以eGFR衡量)下降50%。
- 感染结局:肠杆菌目尿路感染以及所有尿路感染和BSI的发生率。
- 与抗生素使用相关的结局:多重耐药菌或艰难梭菌(C. diff)感染的发生率。
- 根据国际协调会议-良好临床实践(ICH-GCP)指南定义的严重不良事件。
- 患者相关结局:患者报告的生活质量(14)。
使用长期抗生素的一个担忧是耐药性的发展。 作为探索性结局,研究者将确定匹美西林对尿液微生物组和耐药组的影响,以进一步评估微生物组和耐药性的亚临床变化。
参与者与研究设计(图1) 试验资格在移植后一个月评估,届时高风险受者被纳入并随机分组。
研究参与者将在纳入后随访5个月(直至移植后6个月)。
研究参与者将进行三次研究访视:纳入时、两个月后和五个月后。
所有三次访视时都将收集带耐药模式的尿培养。 此外:
- 纳入时,参与者将回答关于生活质量的问卷(14)。此外,检查患者记录。
- 两个月时,评估研究药物的依从性和潜在副作用。
- 五个月时,所有参与者将回答与生活质量(14)、依从性和副作用相关的问卷。
在基线和5个月访视时,将检查患者记录以获取移植功能、药物、排斥事件、微生物学(BSI、尿路感染、多重耐药菌和艰难梭菌)以及全因死亡率和住院情况。
微生物组与耐药组 在基线和5个月访视时,将收集5毫升中段尿用于评估微生物组和耐药组。 将使用Qiagen Biostic Bacteremia Kit提取宏基因组DNA并进行鸟枪法宏基因组测序。 DNA提取和Illumina文库制备后,测序将在挪威奥斯陆的挪威测序中心使用Illumina NovaSeqXPlus进行。 宏基因组读数将按描述处理(15)。 肠道和尿液微生物组的多样性和功能表征:研究者将使用鸟枪法宏基因组测序评估匹美西林对尿液微生物组全局分类组成的影响。 分类学分析将使用MetaPhlAn进行,功能和通路分析将使用HUMAnN3进行。 微生物α多样性将使用四个指数量化:丰富度、香农指数、辛普森指数和基尼指数。 β多样性将使用加权和非加权UniFrac距离度量进行评估,如前所述(16)。

样本量与功效 研究者开发了一个预测模型,包含年龄、性别、诱导免疫抑制治疗类型以及肾移植后第一个月的尿路感染。 该模型可识别一个高风险肾移植受者亚组,其肾移植后第一年BSI发生率为25%,而低风险组为6%。 在干预期(肾移植后2-6个月),高风险组的BSI发生率为23%。 抗生素预防通常非常有效,基于先前肾移植受者的BSI数据(8),研究者预期在移植后2-6个月BSI减少76%。 作为比较,使用Bactrim可使耶氏肺孢子菌感染减少85%。 假设高风险受者在肾移植后2-6个月的BSI发生率为23%,α为0.05(双侧),研究者将能够以0.80的功效(1-β)检测到76%的降低,每组需61名受者(总n=122)。 考虑到脱落以及一年患者和移植存活率分别为98%和96%(8,15),研究者目标为每组纳入75名受者(总计150名)。
