Androgenní deprivační terapie a radioterapie s vysokými dávkami s nebo bez radioterapie celé pánve u nepříznivého středního nebo příznivého vysokého rizika rakoviny prostaty: Randomizovaná studie fáze III
Odůvodnění: Androgeny mohou způsobit růst buněk rakoviny prostaty. Androgenní deprivační terapie může zastavit tvorbu androgenů nadledvinkami. Radiační terapie využívá vysokoenergetické rentgenové záření k zabíjení nádorových buněk.
ÚČEL: Tato randomizovaná studie fáze III studuje androgenní deprivační terapii a radiační terapii při léčbě pacientů s rakovinou prostaty.

Hlavní
- Prokázat, že profylaktická, neoadjuvantní, androgen-deprivační terapie (NADT) a radiační terapie celé pánve (WPRT) povede ke zlepšení celkového přežití (OS) pacientů s „nepříznivým“ středně rizikovým nebo „příznivým“ vysoce rizikovým karcinomem prostaty ve srovnání s NADT a vysokodávkovou radiační terapií prostaty (P) a semenných váčků (SV) využívající RT s modulovanou intenzitou (IMRT) nebo RT s externím paprskem (EBRT) s vysokou dávkou (HDR) nebo trvalou prostatický (radioaktivní semeno) implantát (PPI) boost.

- Ukažte, že profylaktická WPRT zlepšuje biochemickou kontrolu.
- Určete přežití bez vzdálených metastáz (DM).
- Určete příčinu-specifické přežití (CSS).
- Porovnejte akutní a pozdní nežádoucí účinky léčby u pacientů užívajících NADT a WPRT oproti NADT, P a SV RT.
- Zjistěte, zda se kvalita života související se zdravím (HRQOL), měřená pomocí Expanded Prostate Cancer Index Composite (EPIC), významně zhoršuje s rostoucí agresivitou léčby (tj. rameno 2, NADT + WPRT).
- Zjistěte, zda je agresivnější léčba (rameno 2, NADT + WPRT) spojena s větším nárůstem únavy (PROMIS Fatigue Short Form) od výchozího stavu do posledního týdne léčby a větším nárůstem cirkulujících zánětlivých markerů (IL-1, IL- la, IL-6, tumor nekrotizující faktor (TNF)-alfa a C-reaktivní protein).
- Prokažte přírůstkový zisk OS a CSS pomocí agresivnější terapie, která převáží jakékoli nevýhody v primárních generických doménách HRQOL (tj. mobilita, sebepéče, obvyklé aktivity, bolest/nepohodlí a úzkost/deprese).
- Zjistěte, zda změny v únavě od výchozí hodnoty do dalších tří časových bodů (týden před RT, poslední týden léčby a 3 měsíce po léčbě) jsou spojeny se změnami v cirkulujících cytokinech, náladě, spánku a denních aktivitách ve stejných časových bodech .
- Sbírejte tkáňové bloky zalité v parafínu, plazmu, plnou krev a moč pro plánované a budoucí analýzy translačního výzkumu.
PŘEHLED: Toto je multicentrická studie. Pacienti jsou stratifikováni podle skupin se středním až vysokým rizikem, jak je uvedeno v Charakteristiky onemocnění v tomto abstraktu, typu zesílení radioterapie (IMRT vs brachyterapie [Low-dose rate (LDR) pomocí PPI nebo HDR]) a délky trvání androgen- deprivační terapie (krátkodobá [6 měsíců] vs. dlouhodobá [32 měsíců]). Pacienti jsou randomizováni do 1 ze 2 léčebných ramen.
Všichni pacienti dostávají neoadjuvantní androgen-deprivační terapii zahrnující bicalutamid perorálně (PO) jednou denně nebo flutamid PO třikrát denně po dobu 6 měsíců a léčbu agonisty/antagonisty luteinizačního hormonu uvolňujícího hormonu (LHRH) zahrnující leuprolidacetát, goserelinacetát, buserelin, triptorelin nebo degarelix subkutánně (SC) nebo intramuskulárně (IM) každé 1 až 3 měsíce počínaje 2 měsíci před radioterapií a pokračovat po dobu 6 nebo 32 měsíců.
Radioterapie začíná do 8 týdnů po zahájení injekce agonisty/antagonisty LHRH.
- Rameno I: Pacienti podstupují vysokodávkovou radioterapii prostaty a semenných váčků pomocí radioterapie s modulovanou intenzitou (IMRT)* nebo 3D-konformní radiační terapie (3D-CRT)* jednou denně, 5 dní v týdnu, po dobu přibližně 9 týdnů. Pacienti mohou také podstoupit brachyterapii s permanentním implantátem prostaty (PPI) nebo vysokodávkovou brachyterapii (jako radioizotop lze použít jód I 125 nebo palladium Pd 103).
- Rameno II: Pacienti podstupují radioterapii celé pánve (WPRT)* (3D-CRT nebo IMRT) jednou denně, 5 dní v týdnu, po dobu přibližně 9 týdnů. Pacienti mohou také podstoupit brachyterapii jako v rameni I.
POZNÁMKA: * Pacienti podstupující brachyterapeutický implantát dostávají 5 týdnů IMRT, 3D-CRT nebo WPRT.
Pacienti mohou podstoupit odběr vzorků krve a moči pro korelační studie. Mohou být také odebrány vzorky primární nádorové tkáně.
Pacienti mohou na začátku a pravidelně během léčby vyplnit dotazníky EPIC (Expanded Prostate Cancer Index Composite), PROMIS-Fatigue Short Form a EuroQol (EQ-5D) kvality života (QOL). Pacienti, kteří se účastní části QOL studie, musí také souhlasit s pravidelným odběrem krve.
Po ukončení studijní terapie jsou pacienti sledováni každé 3 měsíce po dobu 1 roku, každých 6 měsíců po dobu 3 let a poté každý rok.

CHARAKTERISTIKA ONEMOCNĚNÍ:
Patologicky (histologicky nebo cytologicky) prokázaná diagnóza adenokarcinomu prostaty do 180 dnů od registrace se středním až vysokým rizikem recidivy podle jedné z následujících kombinací:
- Gleasonovo skóre 7-10 + T1c-T2b (palpace) + prostatický specifický antigen (PSA) < 50 ng/ml (zahrnuje středně a vysoce rizikové pacienty)
- Gleasonovo skóre 6 + T2c-T4 (palpace) + PSA < 50 ng/ml NEBO
- Gleasonovo skóre 6 + >= 50 % (pozitivní) biopsie + PSA < 50 ng/ml
- Gleasonovo skóre 6 + T1c-T2b (palpace) + PSA > 20 ng/ml Pacienti dříve diagnostikovaní s nízkým rizikem rakoviny prostaty podstupující aktivní dohled, u kterých byla provedena rebiopsie a bylo zjištěno, že mají nepříznivé středně rizikové onemocnění nebo příznivé vysoce rizikové onemocnění podle protokolu kritéria jsou způsobilá pro zařazení do 180 dnů od postupu opakované biopsie.
- Anamnéza a/nebo fyzikální vyšetření (zahrnující minimálně digitální rektální vyšetření prostaty a vyšetření kosterního systému a břicha) do 90 dnů před registrací
Klinicky negativní lymfatické uzliny zjištěné zobrazením (CT nebo MR pánve a/nebo břicha), (ale ne odběrem vzorků uzlin nebo disekcí) během 90 dnů před registrací
- Pacienti s lymfatickými uzlinami nejednoznačnými nebo pochybnými zobrazením jsou způsobilí, pokud jsou uzliny ≤ 1,5 cm
- Stav pacientů po negativní disekci lymfatických uzlin není způsobilý
Žádné známky kostních metastáz (M0) na kostním skenu během 120 dnů před registrací (Na F PET/CT je přijatelná náhrada)
- Nejednoznačné nálezy kostního skenu jsou povoleny, pokud jsou prosté filmy (nebo CT nebo MRI) negativní na metastázy
- Výchozí hodnota PSA v séru provedená testem schváleným FDA (např. Abbott, Hybritech) během 120 dnů před registrací
Vstup do studie PSA by neměl být získán během následujících časových období:
- Desetidenní období po biopsii prostaty
- Po zahájení hormonální terapie
- Do 30 dnů po vysazení finasteridu
- Do 90 dnů po vysazení dutasteridu
CHARAKTERISTIKA PACIENTA:
- Stav výkonu Zubrod 0-1
- Absolutní počet neutrofilů (ANC) ≥ 1 500/mm³
- Počet krevních destiček ≥ 100 000/mm³
- Hemoglobin (Hgb) ≥ 8,0 g/dl (přijatelná je transfuze nebo jiný zákrok k dosažení Hgb ≥ 8,0 g/dl)
Žádná předchozí invazivní (kromě nemelanomové rakoviny kůže) malignita, pokud neprobíhá onemocnění po dobu minimálně 3 let (1 095 dní) a není v pánvi
- např. je přípustný karcinom in situ dutiny ústní; pacienti s předchozí anamnézou rakoviny močového měchýře však nejsou povoleni
- Žádná předchozí hematologická (např. leukémie, lymfom nebo myelom) malignita
- Bez předchozí radikální operace (prostatektomie) nebo kryochirurgie pro karcinom prostaty
- Žádné předchozí ozařování pánve, brachyterapie prostaty nebo bilaterální orchiektomie
- Žádná předchozí hormonální terapie, jako jsou agonisté LHRH (např. leuprolid, goserelin, buserelin, triptorelin) nebo antagonisté LHRH (např. degarelix), antiandrogeny (např. flutamid, bicalutamid, cyproteron acetát), estrogeny (např. DES) nebo chirurgická kastrace (orchiektomie)
- Předchozí farmakologická androgenní ablace u karcinomu prostaty je povolena pouze v případě, že začátek androgenní ablace (jak agonisty LHRH, tak perorálního antiandrogenu) je ≤ 45 dní před datem registrace.
Žádná závažná aktivní komorbidita, definovaná jako některá z následujících:
- Nestabilní angina pectoris a/nebo městnavé srdeční selhání vyžadující hospitalizaci během posledních 6 měsíců
- Transmurální infarkt myokardu během posledních 6 měsíců
- Akutní bakteriální nebo plísňová infekce vyžadující v době registrace intravenózní antibiotika
- Exacerbace chronické obstrukční plicní nemoci nebo jiné respirační onemocnění vyžadující hospitalizaci nebo vylučující studijní terapii v době registrace
- Jaterní insuficience vedoucí ke klinické žloutence a/nebo koagulačním poruchám nebo těžké jaterní dysfunkci
Syndrom získané imunodeficience (AIDS) na základě současné definice Centra pro kontrolu nemocí (CDC).
- Požadavky specifické pro protokol mohou také vyloučit imunokompromitované pacienty
- Pro vstup do tohoto protokolu není vyžadováno testování na HIV
- Žádní pacienti, kteří jsou sexuálně aktivní a nejsou ochotni/schopni používat lékařsky přijatelné formy antikoncepce
- Žádná předchozí alergická reakce na hormony zahrnuté v tomto protokolu
PŘEDCHOZÍ SOUČASNÁ TERAPIE:
- Viz Charakteristika onemocnění
- Žádná předchozí radikální operace (prostatektomie) nebo kryochirurgie pro rakovinu prostaty
- Žádné předchozí ozařování pánve, brachyterapie prostaty nebo bilaterální orchiektomie
- Žádná předchozí hormonální terapie, jako jsou agonisté hormonu uvolňujícího luteinizační hormon (LHRH) (např. leuprolid, goserelin, buserelin, triptorelin) nebo antagonisté LHRH (např. degarelix), antiandrogeny (např. flutamid, bicalutamid, cyproteron acetát), estrogeny (např. diethylstilbestrol (DES)) nebo chirurgická kastrace (orchiektomie)

Aktivní komparátor: Prostate/Seminal Vesicle Radiotherapy + ADT + Prostate Boost (Arm 1)
Participants receive androgen deprivation therapy (ADT), consisting of an oral anti androgen plus a luteinizing hormone-releasing hormone (LHRH) agonist or antagonist, for a total duration of 4, 6, or 32 months.
Approximately 8-10 weeks after starting the LHRH agent, participants undergo external beam radiation therapy (EBRT) to the prostate and entire seminal vesicles, delivered as 45 Gy in 25 fractions using three-dimensional conformal radiotherapy (3D CRT) or intensity modulated radiotherapy (IMRT).
A boost to the prostate and proximal seminal vesicles follows, using either IMRT (34.2 Gy in 19 fractions, with protocol specified dose reduction options when indicated) or brachytherapy delivered as low dose rate (LDR) or high dose rate (HDR) implant per protocol.

Participants receive ADT as in Arm 1.
Approximately 8-10 weeks after starting the LHRH agent, participants undergo external beam radiation therapy (EBRT) to the whole pelvis-including prostate, seminal vesicles, and pelvic lymph nodes-delivered as 45 Gy in 25 fractions using three-dimensional conformal radiotherapy (3D CRT) or intensity modulated radiotherapy (IMRT).
A boost is then administered as specified for Arm 1.

Change score is calculated by subtracting baseline T-score from later T-score, with a positive change score indicating increased fatigue.
