Pozornost a pohyb očí u Parkinsonovy choroby
Zkoumání úlohy pozornosti v percepčním a kognitivním důsledcích Parkinsonovy choroby
Cílem této observační a intervenční studie je pochopit, jak terapeutická hluboká stimulace mozku (DBS) ovlivňuje pozornost, vnímání a poznání u účastníků s Parkinsonovou chorobou (PD) a esenciálním třesem (ET). Hlavní otázky, na které je třeba odpovědět, jsou:
- Mění zhoršená kontrola pozornosti a pohybu očí v PD, jak jsou sociální podněty vnímány a interpretovány?
- Zlepšuje terapeutické DBS nebo zhoršuje pozornost a percepční deficity pro sociální podněty v PD a ET?
- Může být DBS optimalizována tak, aby obnovila normální kontrolu pozornosti v PD a přitom zůstat účinnou terapií pro jiné aspekty poruchy.
- Co přispívají části mozku zaměřeného DBS k kontrole pozornosti?
Pomocí fotoaparátu pro sledování očí budou vyšetřovatelé studovat, jak účastníci s PD a ET vnímají a vnímají výrazy emocí obličeje před a po zahájení terapie DBS ve srovnání se skupinou zdravých účastníků bez ET, PD nebo DBS. Účastníci s PD a ET uvidí a hodnotí výrazy morfované obličeje na obrazovce počítače ve třech podmínkách:
- Před zahájením terapie DBS (přibližně 1 hodinu).
- V operačním sále, během standardního postupu k implantátu DBS elektrod, zatímco účastník je vzhůru (ne více než 15 minut).
- Po zahájení terapie DBS s krátkými experimentálními změnami úrovně a frekvence DBS (přibližně 1 hodinu).

Parkinsonova choroba (PD) je druhá nejběžnější neurodegenerativní porucha související s věkem, přičemž přibližně 90 000 nových případů je diagnostikována ročně v Severní Americe. Ačkoli motorické příznaky definují poruchu, PD také vede k kognitivním a emocionálním změnám, jako jsou potíže při rozpoznávání výrazů obličeje a regulace pozornosti, které často zastíní problémy s motorem v průběhu onemocnění. Cílem této studie je lépe porozumět spojení mezi pozorností, pohybem očí a emocionálním vnímáním v PD, testování hypotézy, která narušená pozornost vede ke změně vnímání emocí obličeje. Studie také zkoumá, jak hluboká stimulace mozku (DBS) subthalamického jádra (STN) ovlivňuje tyto procesy a poskytne kritické vhled do kognitivních a percepčních důsledků léčby.
Tento výzkum se zabývá kritickou mezerou v porozumění nemotorickým příznakům Parkinsonovy choroby zkoumáním, jak interaguje pozornost, pohyb očí a vnímání. Zjištění poskytnou důkazy o tom, zda kognitivní a emocionální symptomy v PD pramení ze zhoršené kontroly pozornosti a nabízí nový rámec pro léčbu těchto deficitů. Studie navíc vrhne světlo na to, jak různé frekvence DBS ovlivňují vnímání a poznání, což potenciálně vedou personalizované stimulační strategie ke zmírnění motorických i nemotorických příznaků. Získané poznatky mohou ovlivnit budoucí terapie pro PD a prosazovat vědecké znalosti i péči o pacienty.
Účastníci budou rozděleni do tří skupin: PD podstupující DBS, Essential Tremor (ET) podstupující DBS (jako srovnávací skupina pro ne-PD DBS efekty) a zdravé kontroly odpovídající věku a pohlaví. Účastníci dokončí úkoly hodnocení obličeje (hodnocení tváří jako šťastné, neutrální nebo smutné) a vizuální vyhledávací úkoly (nalezení tváří mezi distraktory), zatímco jejich pohyby očí jsou sledovány.
Cílem první studie je měřit, jak změněná pozornost ovlivňuje vnímání emocí obličeje v PD sledováním pohybů očí, zatímco účastníci sledují a kategorizují emoční podněty obličeje. Druhým cílem je charakterizovat mozkovou aktivitu v STN související s pozorností a percepčními procesy během probuzení DBS chirurgii zachycením mikroelektrodových záznamů (MER) nervové aktivity, zatímco účastníci vnímají emocionální tváře, což umožňuje vědcům mapovat roli STN při vedení pozornosti a pohybu očí. Třetím cílem je otestovat, jak stimulace DBS při různých frekvencích (vysoká, nízká nebo vypnuto) ovlivňuje pozornost, pohyb očí a emocionální vnímání tím, že účastníci provádějí vizuální a percepční úkoly při různých nastaveních DBS, aby vyhodnotili, jak stimulace mozku ovlivňuje kognitivní a vnímavé funkce.

Experimentální: Akutní změna DBS
Všichni účastníci jediného ramene této studie podstoupí akutní změnu stimulace DBS za tří podmínek v randomizovaném pořadí v průběhu 1 hodiny.

Účastníci dostanou hlubokou stimulaci mozku dodávanou na klinicky určené terapeutické frekvenci a proudu přibližně 20 minut.
Účastníci dostanou hlubokou stimulaci mozku dodávanou na klinicky určené terapeutické frekvenci a sníží (50%) proud za přibližně 20 minut.
Účastníci budou dostávat hlubokou stimulaci mozku dodávanou na klinicky určeném terapeutickém proudu a sníženy (4 Hz) frekvenci po dobu přibližně 20 minut.

Facial Expression Rating

Participants will rate facial expression along a continuous scale using a computerized slider, following the affective bias task (ABT) of Bijanki et al. (2014).
The scale is anchored with three descriptors, "Very Sad" at slider value 0 (left), "Neutral", at position 0.5 (middle), and "Very Happy" at 1.0 (right).
Responses are compared between two time points: (1) at the initial pre-surgical testing session and post DBS implantation and (2) at the post-implantation session following clinical optimization of therapeutic parameters, 2-3 weeks after surgery.
The comparison will examine both the direction of any bias of the rating, against normative ratings, and the magnitude of average deviation from normative ratings.
Finally, the ratings will be incorporated into a generalized linear model of gaze position (Kovach 2014) to identify the association between perceived facial expression and characteristic fixation patterns.

The location and duration of gaze fixations will be recorded with a remote eye tracking camera.
The location of gaze fixations will be treated as the dependent measure within a generalized linear modeling (GLM) framework for spatial point processes, described in Kovach and Adolphs (2014).
Parameter estimates of the model give the log relative risk of fixation at different locations in the visual scene as a function of the independent measures of the model.
Independent measures include the main effect of (1) Fourier basis functions encoding scene location and its interactions with (2) image rating in the affective bias task of Bijanki et.
al (2014), (3) session and (4) DBS stimulation state.
Measures derived from the GLM model will also include the statistical deviation (e.g.
Kullback-Leibler divergence) from the average distribution of fixations observed in healthy comparison subjects for each image.

During the Aim 2 portion of the study, patients undergoing awake DBS surgery as part of standard clinical care will engage in a subset of the face rating tasks during the surgery.
Invasive recordings will be obtained from DBS target structures, including STN, VIM, and GPi using microelectrodes that are placed as part of standard clinical practice.
Spike sorting will be used to identify firing of individual cells and firing rate will compared to eye movements to identify responses associated with shifts of attention in the targeted deep brain structures .
These measures will be compared across movement-disorders populations using mixed-effects linear modeling and other standard statistical procedures.

Group comparisons of subject-level coherence values across PD, ET, DT, and healthy controls and within-participant comparisons across DBS conditions will be made using mixed-effects linear models, comparing power at baseline and modulation of power within a -0.5 to 0.5 s peri-saccade window.
