Intratekal immunoglobulin til behandling af voksne patienter med stivkrampe: et randomiseret kontrolleret 2x2 faktorielt forsøg

Detaljeret beskrivelse
Forskerne vil udføre et randomiseret delvist blindet kontrolleret 2x2 faktorielt forsøg. For det første vil voksne indlagt på intensivafdelingen på Hospitalet for Tropiske Sygdomme Ho Chi Minh City blive randomiseret til at modtage enten humant (3000 IE) eller heste (21.000 enheder) intramuskulært antitoksin. For det andet vil deltagerne blive randomiseret til at modtage enten standardbehandling med intramuskulært antitoksin alene eller med tilsætning af 500 IE intratekal humant antitoksin. Patienter med tidligere antitoksinbehandling og patienter med kontraindikationer mod lumbalpunktur eller antitoksinbehandling vil blive udelukket.
Alle patienter vil modtage anden standard stivkrampebehandling, som skønnes nødvendig af de behandlende læger. Spasmer vil blive behandlet med benzodiazepiner som førstelinjebehandling. Patienter med spasmer, der ikke er kontrolleret med benzodiazepiner, vil modtage trakeostomi, lammelser, magnesiumsulfat og mekanisk ventilation. Hjertefrekvens, BP, temperatur og dagligt stofforbrug vil blive registreret under hele intensivopholdet. Patienter vil blive fulgt efter udskrivning fra hospitalet indtil 240 dage for invaliditet/dødsfald.

Beskrivelse
Inklusionskriterier:
- Alle voksne patienter (≥16 år) med en klinisk diagnose af generaliseret stivkrampe indlagt på intensivafdelingen (ICU) på Hospital for Tropical Diseases, Ho Chi Minh City, Vietnam
Ekskluderingskriterier:
- Forudgående administration af antitoksin under denne episode
- Kontraindikation til brug af humant eller hesteantitoksin
- Kontraindikation til lumbalpunktur
- Modtager allerede mekanisk ventilation eller forventes at kræve dette, før intratekal injektion kan gives
- Graviditet
- Informeret samtykke ikke opnået

For det andet vil deltagerne blive randomiseret til at modtage tilsætningen af 500 IE intratekal humant antitoksin.

Krav om mekanisk ventilation under intensivophold
Tidsramme: Under intensivophold i gennemsnit 3 uger
|
Kriterier for mekanisk ventilation er iltmætning (SpO2) <90%; eller partialtryk af oxygen/fraktion af indåndet oxygen (PaO2/FiO2) <250; eller overdreven spasmer, der nødvendiggør muskellammelse. Disse kriterier er vejledende, og den endelige beslutning om at ventilere en patient ligger hos den enkelte patientansvarlige læge. |
