HINTERGRUND:
Die koronare Herzkrankheit ist die häufigste Todesursache in den Vereinigten Staaten und verursacht jedes Jahr fast 500.000 Todesfälle. Studien haben bestätigt, dass ein Myokardinfarkt bei bis zu 80 Prozent der Patienten mit einem verschließenden Koronarthrombus zusammenhängt. Thrombolytika der ersten und zweiten Generation (einschließlich Streptokinase und rt-PA) wurden erfolgreich zur Wiederherstellung des myokardialen Blutflusses eingesetzt, wenn ein Thrombus eine infarktbedingte Koronararterie verschlossen hatte. Es waren jedoch weitere klinische Untersuchungen erforderlich, um die am besten geeignete Dosis und Verabreichungsmethode des Thrombolytikums, das Risiko eines späteren erneuten Verschlusses, einer Restenose und/oder eines Myokardinfarkts, die Notwendigkeit zusätzlicher Therapien und die Wahrscheinlichkeit eines Nutzens oder hämorrhagischer Komplikationen zu bestimmen.
1983 gründete das National Heart, Lung, and Blood Institute die TIMI Study Group. Die Gruppe bestand aus 13 klinischen Zentren (später auf 24 erweitert), einem radiografischen Kernlabor, einem Radionuklid-Kernlabor und einem Datenkoordinierungszentrum.
DESIGN-NARRATIVE:
Die TIMI-Studie wurde in zwei Phasen durchgeführt. In Phase I oder TIMI I wurden geeignete Patienten randomisiert und erhielten entweder 80 mg rekombinanten Gewebetyp-Plasminogenaktivator (rt-PA) oder 1,5 Millionen Einheiten Streptokinase intravenös, um die relative Sicherheit und Wirksamkeit zu bestimmen. Nach der Randomisierung erhielten die Patienten, bei denen eine angiographisch dokumentierte Stenose von mehr als 50 Prozent in der Infarktarterie festgestellt wurde, eine doppelblinde thrombolytische Therapie, eine vollständige Antikoagulation und konventionelle Pflege. Anschließend wurden die Patienten einer wiederholten Katheterisierung, einem Radionuklid-Ventrikulogramm und vor der Entlassung einer sechswöchigen und sechsmonatigen Herz-Kreislauf-Untersuchung unterzogen.
TIMI I wurde im Februar 1985 aufgrund statistisch signifikanter Unterschiede in den Koronarreperfusionsraten in den Behandlungsgruppen abgebrochen; RT-PA erwies sich als das überlegene Thrombolytikum. Nach der TIMI-Phase I wechselte der Hersteller von RT-PA zu einer großtechnischen Produktionsmethode für RT-PA, und es wurde festgestellt, dass das neue Produkt in vitro und bei Versuchstieren eine thrombolytische Aktivität und Spezifität aufwies, die mit dem vom alten Produkt hergestellten Produkt vergleichbar war Methode. Die TIMI-Forscher kamen jedoch zu dem Schluss, dass vor Beginn der TIMI-Phase II eine klinische Bewertung erforderlich sein würde.
Daher wurden 1985 TIMI-Open-Label-Phasenstudien mit dem Ziel initiiert, die Sicherheit und Wirksamkeit des „neuen“ intravenösen rt-PA festzustellen. Wie in Phase I war der Endpunkt die Lyse des Koronarthrombus innerhalb von 90 Minuten nach Beginn der Behandlung bei Patienten mit dokumentiertem Totalverschluss der infarktbedingten Koronararterie. Weitere Ziele der Studie waren die Bestimmung der Wiederverschlussraten infarktbezogener Arterien nach 18–48 Stunden sowie die Bestimmung der Wirksamkeit von PTCA zur Aufrechterhaltung der Durchblutung in infarktbezogenen Arterien und zur Verhinderung wiederkehrender Myokardinfarkte. Die TIMI-Open-Label-Phase-Studien ergaben, dass eine optimale koronare Rekanalisierung und Aufrechterhaltung der Reperfusion mit 150 mg „neuem“ RT-PA über einen Zeitraum von sechs Stunden infundiert wurde. Spätere hämorrhagische Komplikationen, die bei 150 mg RT-PA beobachtet wurden, erforderten jedoch eine Änderung der RT-PA-Dosis auf 100 mg.
In TIMI II begann die Patientenaufnahme im April 1986 und endete im Juni 1988 mit der Aufnahme von 3.534 Patienten. Die Patienten wurden innerhalb von vier Stunden nach Einsetzen der Brustschmerzen, von denen angenommen wurde, dass sie durch einen Myokardinfarkt verursacht wurden, mit intravenösem RT-PA behandelt und nach dem Zufallsprinzip einer invasiven oder konservativen Strategie zugeordnet. Der primäre Endpunkt war das Überleben ohne erneuten Myokardinfarkt nach sechs Wochen und einem Jahr Nachbeobachtung. 1.681 Patienten wurden der verzögerten invasiven Strategie zugeordnet, bei der die Katheterisierung zwischen 18 und 48 Stunden nach der RT-PA-Therapie durchgeführt wurde. Wenn bei der Katheterisierung eine Zwischensummenstenose der Infarktarterie von mehr als 60 Prozent festgestellt wurde, die als technisch behandelbar angesehen wurde, wurde eine Angioplastie versucht. Bei 60,5 Prozent der 1.500 Patienten, die sich in der invasiven Strategiegruppe einer Katheterisierung unterzogen, wurde eine Angioplastie durchgeführt. Bei den restlichen 39,5 Prozent bzw. 593 Patienten wurde keine Angioplastie durchgeführt. Es wurden 1.658 Patienten einer konservativen Strategie zugeordnet, bei der die Herzkatheterisierung den 587 Patienten vorbehalten war, die innerhalb von 21 Tagen nach dem Infarkt eine spontane oder belastungsbedingte Myokardischämie aufwiesen. Insgesamt 13,5 Prozent der Patienten in diesem Arm unterzogen sich einer Koronarangioplastie, 7,6 Prozent einer Bypass-Operation und 1,1 Prozent unterzogen sich beiden Eingriffen; Bei 77 Prozent der Patienten der konservativen Strategiegruppe wurde innerhalb von 21 Tagen nach dem Infarkt kein Revaskularisationsverfahren durchgeführt.
TIMI IIA, eine Teilstudie mit 586 Patienten, untersuchte, ob eine sofortige Herzkatheterisierung mit gegebenenfalls perkutaner transluminaler Koronarangioplastie einen Vorteil gegenüber dem gleichen Eingriff bringen würde, der 18 bis 48 Stunden später durchgeführt wird. Alle Patienten wurden innerhalb von vier Stunden nach Beginn des akuten Myokardinfarkts mit intravenösem rt-PA behandelt.
Das in diesem Datensatz aufgeführte Abschlussdatum der Studie wurde aus dem „Enddatum“ ermittelt, das im PRS-Datensatz (Protocol Registration and Results System) eingegeben wurde.
