ACHTERGROND:
Coronaire hartziekte is de belangrijkste doodsoorzaak in de Verenigde Staten, goed voor bijna 500.000 sterfgevallen per jaar. Studies hebben bevestigd dat een myocardinfarct bij tot wel 80 procent van de patiënten verband houdt met een occlusieve coronaire trombus. Trombolytica van de eerste en tweede generatie (waaronder streptokinase en rt-PA) zijn met succes gebruikt om de myocardiale bloedstroom te herstellen wanneer trombus een infarctgerelateerde kransslagader heeft afgesloten. Er was echter verder klinisch onderzoek nodig om de meest geschikte dosering van het trombolyticum en de wijze van toediening te bepalen, het risico van latere herocclusie, restenose en/of myocardinfarct, de behoefte aan aanvullende therapieën en de waarschijnlijkheid van voordeel of hemorragische complicaties.
In 1983 richtte het National Heart, Lung and Blood Institute de TIMI Study Group op. De groep bestond uit 13 klinische centra (later uitgebreid tot 24), een radiografisch kernlaboratorium, een radionuclide kernlaboratorium en een gegevenscoördinatiecentrum.
ONTWERP VERHAAL:
De TIMI-studie werd uitgevoerd in twee fasen. In Fase I of TIMI I werden in aanmerking komende patiënten gerandomiseerd om ofwel 80 mg recombinant weefseltype plasminogeenactivator (rt-PA) of 1,5 miljoen eenheden streptokinase intraveneus te ontvangen om de relatieve veiligheid en werkzaamheid te bepalen. Na randomisatie kregen patiënten met angiografisch gedocumenteerde stenose van meer dan 50 procent in de infarctgerelateerde slagader dubbelblinde trombolytische therapie, volledige antistolling en conventionele zorg. Patiënten ondergingen vervolgens herhaalde katheterisatie, radionuclide ventriculogram en pre-ontslag, zes weken en zes maanden cardiovasculair onderzoek.
TIMI I werd in februari 1985 stopgezet vanwege statistisch significante verschillen in coronaire reperfusiesnelheden in de behandelingsgroepen; rt-PA bleek het superieure trombolyticum te zijn. Na TIMI Fase I stapte de fabrikant van rt-PA over op een grootschalige productiemethode voor rt-PA, en het nieuwe product bleek trombolytische activiteit en specificiteit te hebben in vitro en bij proefdieren vergelijkbaar met het product vervaardigd door de oude methode. De TIMI-onderzoekers concludeerden echter dat klinische evaluatie noodzakelijk zou zijn voordat TIMI Fase II kan worden gestart.
Zo werden in 1985 TIMI Open Label Phase-onderzoeken gestart met als doel de veiligheid en werkzaamheid van de 'nieuwe' intraveneuze rt-PA vast te stellen. Net als in Fase I was het eindpunt lysis van coronaire trombus binnen 90 minuten na aanvang van de behandeling bij patiënten met gedocumenteerde totale occlusie van de infarctgerelateerde kransslagader. Bijkomende doelen van de studie waren het bepalen van de herocclusiepercentages van infarctgerelateerde slagaders na 18-48 uur, evenals het bepalen van de werkzaamheid van PTCA om de perfusie in infarctgerelateerde slagaders te handhaven en recidiverend myocardinfarct te voorkomen. De TIMI Open Label Phase-onderzoeken stelden vast dat optimale coronaire rekanalisatie en behoud van reperfusie plaatsvond met 150 mg 'nieuw' rt-PA toegediend gedurende zes uur. De daaropvolgende hemorragische complicaties die werden waargenomen met 150 mg rt-PA maakten echter een wijziging van de dosis rt-PA naar 100 mg noodzakelijk.
In TIMI II begon de opname van patiënten in april 1986 en eindigde in juni 1988 met de inschrijving van 3.534 patiënten. Patiënten werden binnen vier uur na het begin van de pijn op de borst, vermoedelijk veroorzaakt door een myocardinfarct, intraveneus behandeld met rt-PA en willekeurig toegewezen aan een invasieve strategie of een conservatieve strategie. Het primaire eindpunt was overleving zonder recidiverend myocardinfarct na zes weken en één jaar follow-up. Er waren 1.681 patiënten toegewezen aan de uitgestelde invasieve strategie waarbij katheterisatie werd uitgevoerd tussen 18 en 48 uur na rt-PA-therapie. Als katheterisatie een subtotale stenose van meer dan 60 procent van de infarctgerelateerde slagader vertoonde die als technisch benaderbaar werd beschouwd, werd angioplastiek geprobeerd. Angioplastiek werd uitgevoerd bij 60,5 procent van de 1.500 patiënten die katheterisatie ondergingen in de invasieve strategiegroep. De resterende 39,5 procent of 593 patiënten hadden geen angioplastiek uitgevoerd. Er waren 1.658 patiënten toegewezen aan een conservatieve strategie waarbij hartkatheterisatie was gereserveerd voor de 587 patiënten die binnen 21 dagen na het infarct spontane of door inspanning geïnduceerde myocardischemie hadden. In totaal onderging 13,5 procent van de patiënten in deze arm coronaire angioplastiek, 7,6 procent onderging een bypassoperatie en 1,1 procent onderging beide procedures; 77 procent van de patiënten in de groep met conservatieve strategie onderging binnen 21 dagen na het infarct geen revascularisatieprocedure.
TIMI IIA, een subtrial van 586 patiënten, onderzocht of onmiddellijke hartkatheterisatie met percutane transluminale coronaire angioplastiek, indien van toepassing, een voordeel zou opleveren ten opzichte van dezelfde procedure die 18 tot 48 uur later werd uitgevoerd. Alle patiënten werden binnen vier uur na het begin van een acuut myocardinfarct behandeld met intraveneuze rt-PA.
