Évaluation de l'impact de l'application de photobiomodulation post-exercice sur les performances, la récupération et l'état comportemental dans la population d'opérateurs spéciaux formés
Les forces d'opérations spéciales (SOF) s'entraînent continuellement pour maintenir des performances optimales. Ainsi, ils sont presque toujours en état de convalescence et ont besoin de thérapies non invasives pour faire face à la charge de travail éprouvante. La thérapie par photobiomodulation (PBMT) est un traitement non invasif dans lequel un laser de faible intensité est appliqué sur le corps pour améliorer la guérison, la récupération et les performances. La réadaptation et le reconditionnement rapides d'optimisation humaine tactique de l'armée (THOR3) offrent une voie cohérente pour la mise en œuvre du PBMT en tant que modalité. Des études menées auprès d'athlètes ont montré des avantages en matière de performance et de récupération grâce à l'application focale du PBMT avant et après l'entraînement. Bien qu'il existe moins de preuves sur les effets cognitifs/comportementaux potentiels d'une application systématique de PBMT, la fatigue autodéclarée s'est également avérée significativement plus faible dans les groupes avec application focale de PBMT par rapport au placebo. De plus, la recherche sur le PBMT chez les athlètes tactiques militaires en bonne santé est limitée. Le PBMT peut être un outil prometteur pour améliorer les performances physiques en accélérant la récupération musculo-squelettique et psychologique de la population SOF. Les enquêteurs visent à étudier les effets physiologiques et comportementaux de l'application du PBMT après l'exercice sur les performances des opérateurs SOF.
L'intention : Les enquêteurs proposent de mener un essai contrôlé randomisé en simple aveugle avec contrôle fictif pour étudier l'efficacité de la fourniture d'un entraînement physique post-PBMT dans une population SOF.
Les objectifs spécifiques de cette étude sont de :
- Analyser et décrire les effets physiologiques, le cas échéant, de l'application du PBMT après l'exercice chez les opérateurs des forces spéciales suivant une formation dirigée par un entraîneur.
- Analyser et décrire les effets comportementaux, le cas échéant, de l'application du PBMT après l'exercice chez les opérateurs des forces spéciales suivant une formation dirigée par un entraîneur.
- Évaluer l'utilité clinique globale du PBMT focal suite à un entraînement physique dans une SOF de l'armée américaine, population d'athlètes tactiques.

Critère d'intégration:
- Personnel des forces spéciales en service actif (série 18)
- Capable de lire et de comprendre la langue anglaise à des fins de consentement
- Capable de s’engager dans l’intervention et le suivi de l’étude
- Capable de participer à la formation dirigée par un coach THOR3, sans restriction
Critère d'exclusion:
- Obésité (graisse corporelle > 25 %)
- Maladie cardiovasculaire
- Utilisation de certains médicaments (par ex. statines, diurétiques, agents hypertenseurs)
- Femelle
- Tatouage dans la zone de traitement (région du corps)
- Diagnostic de porphyrie (allergie induite par la lumière) ou d'eczéma photosensible
- Utilisation actuelle de médicaments associés à une sensibilité à la chaleur ou à la lumière (par ex. amiodarone, chlorpromazine, doxycycline, hydrochlorothiazide, acide nalidixique, naproxène, piroxicam, tétracycline, thioridazine, voriconazole)

Comparateur factice: Faux PBMT
L'inclusion du PBMT fictif garantira que toutes les procédures de traitement des participants restent les mêmes, à l'exception de l'émission de photons (traitement actif), permettant ainsi que la contribution de toute différence entre les groupes soit créditée à l'utilisation du PBMT.

Sham PBMT sera fourni par un membre formé de l'équipe d'étude.
Sham PBMT sera administré de la même manière que celui indiqué pour le processus PBMT ci-dessus, mais l'appareil restera en mode veille (c'est-à-dire que le mode de traitement ne sera pas activé).
La lumière infrarouge étant invisible à l’œil nu, la seule différence visible entre les modes traitement et veille est la présence de quelques petites lumières oranges (ces lumières sont allumées pendant le mode traitement).

Comparateur actif: Photobiomodulation Treatment (PBMT)
PBMT will be delivered at 40 watts (W).
PBMT will be applied to the quadriceps area.
A study team member will use the quadriceps measurements of the treatment area to calculate the PBMT treatment time (approximately 5-20 minutes) and specified J/cm2.
PBMT treatment will be provided 3 times per week, for 3 weeks.

PBMT will be delivered at 40W (depending on participant skin pigmentation). PBMT will be applied to the quadriceps area. A study team member will use the quadriceps measurements of the treatment area to calculate the PBMT treatment time (approximately 5-20 minutes) and specified J/cm2. PBMT treatment will be provided 3 times per week, for 3 weeks. PBMT will be delivered by a trained study team member using the LightForce® XLi 40W device with the Smart Hand Piece technology, which has a built-in accelerometer in the hand piece that controls the speed of light delivery to the treatment area. The trained team members will use the Smart Hand Piece technology, which assesses the operator's speed and provides real-time visual and sensory feedback calibrated to shut-off when moving too slowly and warns the operator when moving too quickly by vibrating. Treatment is delivered through a flexible optical fiber threaded through the hand piece, which contains a rolling glass massage ball.
