Projeto de Fotomedicina 14: PBMT para Melhoria de Desempenho em SOF (SOF)
Avaliando o impacto da aplicação de fotobiomodulação pós-exercício no desempenho, recuperação e estado comportamental na população de operadores especiais treinados
As Forças de Operações Especiais (SOF) treinam continuamente para manter o desempenho máximo. Assim, eles estão quase sempre em estado de recuperação e necessitam de terapias não invasivas para lidar com a carga de trabalho desgastante. A terapia de fotobiomodulação (PBMT) é um tratamento não invasivo em que um laser de baixa intensidade é aplicado ao corpo para melhorar a cura, a recuperação e o desempenho. A Reabilitação e Recondicionamento Rápido de Otimização Humana Tática do Exército (THOR3) fornece um caminho consistente para a implementação do PBMT como modalidade. Estudos em atletas demonstraram benefícios de desempenho e recuperação com aplicação focal pré e pós-treino de PBMT. Embora haja menos evidências sobre os potenciais efeitos cognitivos/comportamentais de uma aplicação sistemática de PBMT, a fadiga autorreferida também foi significativamente menor em grupos com aplicação focal de PBMT em comparação com o placebo. Além disso, a pesquisa de PBMT em atletas táticos militares saudáveis é limitada. A intenção: Os investigadores propõem a realização de um ensaio de controle randomizado, cego, com controle simulado para investigar a eficácia do fornecimento de PBMT pós-treinamento físico em uma população SOF.
Os objetivos específicos deste estudo são:
- Analisar e descrever os efeitos fisiológicos, se houver, da aplicação de PBMT pós-exercício em Operadores de Forças Especiais submetidos a treinamento conduzido por treinador.
- Analisar e descrever os efeitos comportamentais, se houver, da aplicação de PBMT pós-exercício em Operadores de Forças Especiais em treinamento conduzido por treinador.
- Avalie a utilidade clínica geral do PBMT focal subsequente ao treinamento físico em uma SOF do Exército dos EUA, população de atletas táticos.

- Capaz de se comprometer com a intervenção e acompanhamento do estudo
- Capaz de participar do treinamento de líder de treinador THOR3, sem restrições

- Obeso (gordura corporal > 25%)
- Doença cardiovascular
- Uso de medicamentos selecionados (por exemplo, estatinas, diuréticos, agentes hipertensivos)
- Fêmea
- Tatuagem na área de tratamento (região corporal)
- Diagnóstico com porfiria (alergia induzida pela luz) ou eczema fotossensível

Comparador Falso: PBMT falso

Comparador Ativo: Photobiomodulation Treatment (PBMT)
PBMT will be delivered at 40 watts (W).
PBMT will be applied to the quadriceps area.
A study team member will use the quadriceps measurements of the treatment area to calculate the PBMT treatment time (approximately 5-20 minutes) and specified J/cm2.
PBMT treatment will be provided 3 times per week, for 3 weeks.

PBMT will be delivered at 40W (depending on participant skin pigmentation). PBMT will be applied to the quadriceps area. A study team member will use the quadriceps measurements of the treatment area to calculate the PBMT treatment time (approximately 5-20 minutes) and specified J/cm2. PBMT treatment will be provided 3 times per week, for 3 weeks. PBMT will be delivered by a trained study team member using the LightForce® XLi 40W device with the Smart Hand Piece technology, which has a built-in accelerometer in the hand piece that controls the speed of light delivery to the treatment area. The trained team members will use the Smart Hand Piece technology, which assesses the operator's speed and provides real-time visual and sensory feedback calibrated to shut-off when moving too slowly and warns the operator when moving too quickly by vibrating. Treatment is delivered through a flexible optical fiber threaded through the hand piece, which contains a rolling glass massage ball.

Scores near 100 indicate very stable sleep duration that supports recovery; 60-80 indicates moderate variability that may mildly reduce readiness; scores below ~60 reflect inconsistent sleep duration likely to lower Readiness.
Higher scores are better outcomes.
