OBIETTIVO PRIMARIO:

I. Stabilire la sperimentazione come fattibile e valutare l'impatto sulla tossicità gastrointestinale acuta nella terza settimana di radioterapia pelvica seguendo un programma ipo-frazionato.

OBIETTIVI SECONDARI:

I. Stimare l'impatto sulla tossicità urinaria acuta. II. Per stimare l'impatto sulla tossicità gastrointestinale segnalata dal paziente. III. Per valutare la qualità acuta della vita dopo il trattamento. IV. Per quantificare la tossicità finanziaria acuta dopo il trattamento. V. Valutare la soddisfazione per il processo decisionale successivo al trattamento. VI. Per valutare la sopravvivenza globale durante 3 anni di follow-up post-trattamento.

CONTORNO:

I pazienti vengono sottoposti a radioterapia ipofrazionata per 3 settimane in assenza di progressione della malattia o tossicità inaccettabile.

Dopo il completamento del trattamento in studio, i pazienti vengono seguiti a 1, 3 e 6 mesi e poi a 1, 2 e 3 anni.

Criterio di inclusione:

  • Soggetti di età >= 18 anni
  • Malignità patologicamente confermata della cervice o dell'endometrio. Sono consentite istologie non epiteliali. È consentito anche l'adenocarcinoma in situ
  • I pazienti devono essere in stato post isterectomia per la gestione iniziale del cancro della cervice o dell'endometrio
  • Il soggetto deve avere una malattia non metastatica come determinato dall'esame clinico o dalla tomografia computerizzata (TC) o dalla tomografia a emissione di positroni (PET)/imaging TC
  • Performance status dell'Eastern Cooperative Oncology Group (ECOG) =< 2
  • Recupero al basale o =< grado 1 Common Terminology Criteria for Adverse Events (CTCAE) versione (v)5.0 da tossicità correlate a qualsiasi precedente terapia antitumorale, a meno che non sia considerato clinicamente non significativo dallo sperimentatore curante

Criteri di esclusione:

  • Precedente irradiazione addominale o pelvica

Change in Toxicity - Bowel Summary Score
Lasso di tempo: from baseline to the week 3 timepoint, up to 6 weeks from the baseline visit

Change in toxicity will be measured by the change in bowel domain score as assessed on the Expanded Prostate Cancer Index Composite (EPIC) instrument. EPIC is a 26-item, self-assessed questionnaire rated on a 5-point Likert scale. 14 items in this assessment create the Bowel Summary score.

Bowel Summary scores are transformed linearly to a 0-100 scale, with higher scores indicating better Health-related Quality of Life (HRQOL) and lower scores indicating worse HRQOL. This outcome measure will report mean change in the Bowel Summary Score. A positive change indicates the scores increased (improved HRQOL) and a negative Change indicates the scores decreased (worse HRQOL).

This outcome measure will report mean EPIC Bowel Summary scores at the week 3 timepoint.

To assess the feasibility of administering a clinical trial to evaluate hypofractionated radiotherapy.

This outcome measure will report the number of evaluable patients and the number of patients who started study treatment but were not deemed evaluable. To be considered evaluable, an eligible patient must have completed 3 weeks of treatment and completed EPIC bowel domain questionnaire at baseline and 3 weeks.
