術後せん妄 (PD) と術後認知機能障害 (POCD) はどちらも、心臓手術後の高齢患者に見られるよく知られた合併症です。 PD および POCD の病因は不明ですが、脳虚血が依然として主要な候補です。 全身酸素化レベルの低下を相関させる試み(すなわち、 SpO2) と PD/POCD の発症は、これまでのところ残念な結果となっています。
私たちは、脳組織酸素飽和度 (SctO2) を継続的に監視する非侵襲的技術である脳酸素濃度測定により、相関関係が存在するかどうかという疑問に答えることができると考えています。
SctO2 の閾値を設定および操作できる絶対脳酸素濃度計 (FORE-SIGHT) を利用できると、脳酸素化と神経認知合併症の発症との関係を評価する機会が得られます。
私たちは、以下を評価するために十分な検出力を備えた、120 人の患者を対象とした無作為化マスク試験を提案します。
- 脳酸素化の欠損と術後最初の 5 日のある時点での PD の発生との間に関連性はありますか?

アクティブコンパレータ:大脳の飽和度の低下、すなわち、 SctO2 < 60 % (5 分間)
脳の不飽和が確立されると、研究担当者は研究患者の脳内の酸素レベルの最適化を試みます。

SctO2 レベルが 60 % を下回った場合、次の介入プロトコルが適用されます。
まず、患者の頭の位置が適切な位置であるかどうかを確認し、顔に充血がないかどうかを観察します。
次に、PaCO2 を 40 ~ 50 mmHg、MAP を 60 ~ 80 mmHg に維持する努力が行われます。
心拍数は 2.0 ~ 2.5 L/min/m2 に維持されます。
ヘマトクリットは 20 % 以上である必要があります。
この目的には、赤血球または血液濃縮が使用されます。
