상세 설명
수술 후 섬망(PD)과 수술 후 인지 기능 장애(POCD)는 모두 심장 수술 후 노인 환자에서 볼 수 있는 잘 알려진 합병증입니다. PD 및 POCD의 병인은 알려져 있지 않지만 대뇌 허혈이 주요 후보로 남아 있습니다. 감소된 수준의 전신 산소화(즉, PD/POCD의 발달과 함께 SpO2)는 지금까지 실망스러웠습니다.
우리는 대뇌 조직 산소 포화도(SctO2)를 지속적으로 모니터링하는 비침습적 기술인 대뇌 산소 측정법을 통해 상관 관계가 존재하는지 여부에 대한 질문에 답할 수 있을 것이라고 믿습니다.
SctO2에 대한 임계값을 설정하고 조작할 수 있는 절대 대뇌 산소 농도계(FORE-SIGHT)의 가용성은 대뇌 산소화와 신경인지 합병증의 발달 사이의 관계를 평가할 수 있는 기회를 제공합니다.
우리는 120명의 환자를 대상으로 다음을 평가할 수 있는 무작위, 가려진 시험을 제안합니다.
- 대뇌 산소 결핍과 수술 후 첫 5일 동안 특정 시점의 PD 발생 사이에 연관성이 있습니까?

활성 비교기: 대뇌 불포화반응, 즉; 5분 동안 SctO2 < 60%
대뇌 불포화 상태가 설정되면 연구 인력은 연구 환자의 뇌 내 산소 수준을 최적화하려고 시도합니다.

SctO2 수준이 60% 미만으로 떨어지면 다음 개입 프로토콜이 적용됩니다.
먼저, 환자의 머리 위치가 적절한 위치인지 확인하고 얼굴이 과다한지 관찰합니다.
그런 다음 PaCO2를 40-50mmHg와 60-80mmHg의 MAP 사이로 유지하기 위한 노력을 기울일 것입니다.
심장 지수는 2.0 - 2.5 L/min/m2 사이에서 유지됩니다.
헤마토크릿은 20% 이상이어야 합니다.
적혈구 또는 혈액 농축이 이 목적을 위해 사용됩니다.
