特発性肺線維症 (IPF) は、進行性かつ不可逆的な線維性肺疾患であり、疾患経過はさまざまです。 ほとんどの IPF 患者は比較的ゆっくりとした臨床経過をたどりますが、毎年最大 15% の患者が IPF の急性増悪 (AE-IPF) を経験しています。または高解像度コンピューター断層撮影 (HRCT) の統合。 日本人 IPF 患者の最近の疫学調査では、最も一般的な死因は AE-IPF であることが示されました。
AE-IPFの結果は非常に悪いです。 報告されている 1 か月の死亡率は約 60% であり、報告されている院内死亡率は 50 ~ 60% です。

アジスロマイシンは、免疫調節特性と抗炎症効果を持つマクロライドです。 以前の報告では、重度の肺炎や急性肺損傷などの深刻な状態の患者におけるマクロライドの有効性が説明されています。 2011年まで、日本で静脈内注射で使用できるマクロライドはエリスロマイシンだけでした。 それでも、エリスロマイシンには多くの副作用と薬物相互作用があるため、毎日の臨床診療で日常的に静脈内エリスロマイシンを使用することはありませんでした. AE-IPFが疑われる場合には、キノロンベースの抗生物質を使用しました。 静注用アジスロマイシンは、日本では 2011 年 9 月から臨床使用が承認されています。 アジスロマイシンはエリスロマイシンよりも安全で使いやすく、Walkey のレポートの発行以来、2012 年 7 月から急性呼吸不全の患者に日常的にアジスロマイシンを使用しています。 静脈内アジスロマイシンは、慢性線維性間質性肺炎のAE患者の転帰の改善と関連していることを以前に報告しました。 しかし、その報告には 2 つの大きな制限がありました。アジスロマイシンで治療された患者の数が非常に少ないことと、非特異的間質性肺炎および慢性過敏性肺炎の患者が含まれていることです。

説明
包含基準:
- 患者は、IPFの軽度の悪化と診断され、アシュート大学の胸部部門に入院し、侵襲的な人工呼吸なしで人工呼吸器のサポートが必要な場合、登録の資格があります。
除外基準:
- 年齢:18歳未満。
- 軽度以外の重症度の患者 IPFの急性増悪
- UIPではなく放射線学的パターンを伴うMSCT患者
- 不安定な患者には人工呼吸器または RICU 入院が必要
- 末端臓器不全の患者。

Patients will receive conventional therapy and Add-on Azithromycin 500 mg single daily dose for five days then Patients will receive conventional treatment and early adjunctive azithromycin according to the study treatment protocol.
Azithromycin will be administered as a 500 mg oral dose three times weekly during longitudinal follow-up according to tolerability and standard clinical safety monitoring.
This arm represents the early inflammatory intervention strategy targeting inflammatory disease behavior in idiopathic pulmonary fibrosis.
