Detaillierte Beschreibung
Die idiopathische Lungenfibrose (IPF) ist eine fortschreitende und irreversible fibrotische Lungenerkrankung mit variablem Krankheitsverlauf. Die meisten Patienten mit IPF haben einen relativ langsamen klinischen Verlauf, aber bei bis zu 15 % der Patienten kommt es jedes Jahr zu einer akuten Exazerbation der IPF (AE-IPF), definiert als akute Verschlechterung oder Entwicklung von Dyspnoe und neuer beidseitiger Milchglasanomalie und/ oder Konsolidierung auf hochauflösender Computertomographie (HRCT). Eine kürzlich durchgeführte epidemiologische Untersuchung japanischer Patienten mit IPF zeigte, dass die häufigste Todesursache AE-IPF war.
Das Ergebnis von AE-IPF ist sehr schlecht. Die gemeldete 1-Monats-Sterblichkeitsrate liegt bei etwa 60 %, und die gemeldete Sterblichkeitsrate im Krankenhaus liegt zwischen 50 und 60 %.
AE-IPF fehlt eine wirksame pharmazeutische Behandlung. Aktuelle Leitlinien empfehlen, dass die meisten Patienten mit AE-IPF mit Kortikosteroiden behandelt werden sollten, aber keine kontrollierten Studien unterstützen diese Empfehlung. Die International Working Group hat kürzlich eine überarbeitete Definition und Diagnosekriterien für AE-IPF vorgeschlagen. Frühere diagnostische Kriterien. empfahlen den strikten Ausschluss anderer Ursachen einer akuten Verschlechterung der Atemwegserkrankung, aber neue Kriterien haben es Ärzten ermöglicht, Patienten mit ausgelöstem AE zusätzlich zu idiopathischem AE-IPF einzubeziehen.
In Studien zur Behandlung und den Ergebnissen von AE-IPF erhielten fast alle Patienten zusätzlich zu Kortikosteroiden empirische Antibiotika, obwohl kontrollierte Studien fehlten, die den Nutzen einer empirischen Behandlung zeigten.
Azithromycin ist ein Makrolid mit immunmodulatorischen Eigenschaften und entzündungshemmenden Wirkungen. Frühere Berichte haben die Wirksamkeit von Makroliden bei Patienten mit schweren Erkrankungen wie schwerer Lungenentzündung und akuter Lungenverletzung beschrieben. Bis 2011 war Erythromycin das einzige Makrolid, das in Japan durch intravenöse Injektion verwendet werden konnte. Dennoch hat Erythromycin viele Nebenwirkungen und Arzneimittelwechselwirkungen, sodass wir Erythromycin nicht routinemäßig intravenös in der täglichen klinischen Praxis angewendet haben. Bei Verdacht auf AE-IPF setzten wir Chinolon-basierte Antibiotika ein. In Japan ist seit September 2011 intravenöses Azithromycin für die klinische Anwendung zugelassen. Azithromycin ist sicherer und einfacher anzuwenden als Erythromycin, und seit der Veröffentlichung von Walkeys Bericht verwenden wir Azithromycin seit Juli 2012 routinemäßig für Patienten mit akuter respiratorischer Insuffizienz. Wir berichteten zuvor, dass intravenöses Azithromycin mit verbesserten Ergebnissen bei Patienten mit AE einer chronisch fibrosierenden interstitiellen Pneumonie verbunden war. Dieser Bericht hatte jedoch zwei große Einschränkungen: die sehr kleine Anzahl von Patienten, die mit Azithromycin behandelt wurden, und die Einbeziehung von Patienten mit unspezifischer interstitieller Pneumonie und chronischer Hypersensitivitäts-Pneumonie.

Beschreibung
Einschlusskriterien:
- Patienten kommen für die Aufnahme in Frage, wenn bei ihnen eine leichte Exazerbation der IPF diagnostiziert und sie in die Brustabteilung der Assiut-Universität eingeliefert werden und eine Beatmungsunterstützung ohne invasive mechanische Beatmung benötigen.
Ausschlusskriterien:
- Alter: weniger als 18 Jahre.
- Patienten mit anderen Schweregraden als leicht Akute Exazerbation von IPF
- Patienten mit MSCT mit einem radiologischen Muster anstelle von UIP
- Instabile Patienten benötigen eine mechanische Beatmung oder eine stationäre Aufnahme auf der Intensivstation
- Patienten mit Endorganversagen.

Patients will receive conventional therapy and Add-on Azithromycin 500 mg single daily dose for five days then Patients will receive conventional treatment and early adjunctive azithromycin according to the study treatment protocol.
Azithromycin will be administered as a 500 mg oral dose three times weekly during longitudinal follow-up according to tolerability and standard clinical safety monitoring.
This arm represents the early inflammatory intervention strategy targeting inflammatory disease behavior in idiopathic pulmonary fibrosis.
