導入
アテローム性動脈硬化性心血管疾患(ASCVD)は、人口の罹患率と死亡率の主な原因の 1 つです。 動脈性高血圧症(AHT)は、他の心血管リスク(CVR)因子とともに、大小の動脈の構造と機能に異常を生じさせ、血管の早期老化を引き起こし、最終的には標的臓器損傷(TOD)の発症につながります。 ASCVD の前兆であることは明らかですが、十分に診断されていません。 ただし、すべての患者が TOD を発症するわけではなく、同じ期間または同じ強度で発症するわけでもありません。 さらに、最初に低から中程度の CVR を示した患者は、TOD の存在下ではすでに中程度から高の CVR であるとみなされ、より厳しい治療目標が設定されています [1、2]。
血管生体力学は主に動脈の機械的特性と挙動の研究に焦点を当てています。 不利な動脈生体力学は、アテローム性動脈硬化症および動脈硬化の発生と進行に関連しています。 これらのプロセスは、内皮下腔での酸化リポタンパク質の蓄積、炎症経路の活性化と増幅、および動脈線維症と石灰化の進行によって媒介される一連の血管損傷を表します。 これらの現象の核心は、動脈壁への有害分子に対する血管内皮の透過性の増加と濾過能力の喪失を特徴とする内皮機能不全の発生にあります[3、4]。
酸化ストレスは、反応性種の生成と、前者に有利な抗酸化システムとの間の不均衡として定義され、内皮機能不全の発症、酸化リポタンパク質の沈着、およびこれらと単核貪食システムとの間の炎症促進性相互作用において重要な役割を果たします。 活性酸素種 (ROS) などの蓄積は、脂質の過酸化およびグリコ酸化現象を誘発し、最終的に高度な脂質酸化および糖化最終生成物 (それぞれ ALE および AGE) の形成につながります。 ALE と AGE はタンパク質に結合して機能的および構造的異常を引き起こす可能性があり、また、とりわけ死経路を活性化することによって細胞シグナル伝達にも影響を与える可能性があります [5、6]。
好ましくない酸化還元状態と糖化レベルは、特定の亜集団、特に糖尿病 (DM) 患者における老化、変性疾患、蓄積された血管損傷に関連しています。 しかし、抗酸化システムの糖化とグリコ酸化が動脈生体力学に及ぼす影響や、中程度から高い CVR を持つ一見健康な患者における標的臓器損傷の発症についてはほとんど知られていません。 私たちは、カタラーゼ、スーパーオキシドジスムターゼ、グルタチオンペルオキシダーゼに焦点を当て、血清糖化レベル、グリコ酸化状態、およびいくつかの抗酸化システム要素の機能構造バランスと動脈生体力学的異常と TOD との具体的な関係を研究することを目的としています [4,7,8]。
材料と方法
プロジェクトの設計とセットアップ
これは、サンティアゴ デ コンポステーラ大学病院の内科に属する高血圧および心血管リスク部門が推進する、観察的で両方向的な多施設共同プロジェクトです。 候補患者は、サンティアゴ・デ・コンポステーラとバルバンサの統合管理組織(EOXI)に所属し、心血管リスクの診断、管理、フォローアップのための相談を求める個人となります[9]。
参加者。 包含基準と除外基準
相談を受けた患者の総プールから、ガイドライン参照スコアに基づいて中程度から高の CVR 心血管リスク プロファイルを持つ 18 歳以上の個人のみが事前に選択されます。 除外基準には、現在の臨床診療ガイドラインに従ったDMの存在、喫煙習慣(現在の摂取量および採用前6か月以内の摂取量と定義)、危険なアルコール摂取(女性および男性の場合、1日あたり10gおよび20gを超える摂取量と定義)が含まれます。 、それぞれ)、および確立された心血管疾患[10-14]。
動脈生体力学的マーカーと TOD の評価
高周波プローブを備えた Mindray Z60 デバイス (Mindray®、広東省、中国) を使用して、総頸動脈 (CCA)、内頸動脈 (ICA)、および下肢の遠位動脈の両側超音波探査を実行します。 次の測定値と計算が抽出されます。
- 運動モード (M モード) では、CCA の中間セグメントの伸張性と張力パラメータを計算するために、動脈半径、動脈圧、総壁厚を同時に測定します [15,16]。
- 二次元モード (B モード) では、CCA の遠位領域における内膜中膜厚さ (IMT) の自動測定と、コレステロール プラークの検出および特性評価が行われます [17]。
- スペクトル ドップラー モードでは、CCA と ICA の中間セグメントにおける流速と血管抵抗指数に関する体積時間 (流量) 関数の形態学的記述が行われます [18]。
- エラストグラフィーモジュールを備えた B モードでは、CCA の中間セグメントの局所的な剛性度を直接評価します [19]。
- ECG 同期を備えたスペクトル ドップラー モードでは、頸動脈-大腿部の脈波伝播速度 (c-f PWV) を評価します [20]。
- スペクトル ドップラー モードでは、足首上腕指数 (ABI) を評価します [21]。
また、Topcon TRC-NW6S デバイス (Electronics®、東京、日本) を使用した網膜検査を使用して、網膜微小血管系の初期損傷も評価します。
酸化ストレスマーカーと抗酸化システム活性構造の評価
酸化還元状態を評価するには、参照されたプロトコルに従って、血漿および尿中の脂質酸化(脂質過酸化)およびタンパク質のレベルが、それぞれチオバルビツール酸反応性物質(TBARS)および還元されたチオールを定量することによって決定されます[22、23]。 どちらの方法でも、分光測光法による定量が可能です。 分析は、Asys UVM-340 分析装置 (Biochrom®、ケンブリッジ、英国) を使用して行われます。 グルタチオンペルオキシダーゼ、カタラーゼ、およびスーパーオキシドジスムターゼの酵素機能は、参照されたプロトコールに従って、Asys UVM-340 アナライザー (Biochrom®、ケンブリッジ、英国) を使用して酵素活性として測定されます [24、25]。 糖化およびグリコ酸化パターンに基づく抗酸化酵素の構造特徴は、SDS-PAGE、クロマトグラフィー、質量分析などのプロテオミクスを使用して評価されます [26-28]。
糖化および糖酸化状態の測定
糖化ヘモグロビンの割合は、ヘモグロビン ネクスト アナライザーを備えた高速液体クロマトグラフィー (HPLC) を使用して評価されます (A. Menarini Diagnostics®、イタリア、フィレンツェ)。 フルクトサミンレベルは、Advia 2400分析装置(Siemens Healthcare Diagnostics Inc.(登録商標)、タリータウン、米国)を用いたジアザイム糖化血清タンパク質酵素法(ダイアザイム、ケント、英国)によって測定される。 糖化アルブミンの割合は、フルクトサミンとアルブミンのレベルから推定されます[29、30]。 この臨床検査評価は中央病院検査部門によって実行されます。
グリコ酸化の一般的なレベルは、AGE リーダー (DiagnOptics BV®、フローニンゲン、オランダ) を使用して皮膚内の終末糖化生成物 (AGE) を測定することによって定量化されます。 AGE は主に、糖酸化プロセス中に生じるタンパク質に結合した糖化付加物を表します。 AGE Reader は、さまざまな AGE の蛍光特性を使用して、皮膚の自己蛍光 (SAF) を非侵襲的に評価するために開発されました [31]。
心血管リスクの結果
治療に必要な診察数(NNT)と動脈生体力学の異常および標的臓器損傷(TOD)を検出するための探索時間を測定し、治療強化を正当化するCVRの増加を決定します。 心血管系および脳血管系のイベント、入院、診察、死亡および障害も評価されます[32]。
倫理的配慮と優れた臨床実践
この研究は、ヘルシンキ宣言の倫理原則およびガリシア(スペイン)保健サービス(SERGAS)の研究における優良実践基準に従って実施されます。 参加に同意したすべての患者から書面によるインフォームドコンセントが得られます。 プロトコールは、サンティアゴ・ルゴ市の研究倫理委員会によって承認されました(コード 2021/401 および 2023/302)。
作業計画とプロジェクトの流れ
第一段階では、基準を満たし参加に同意した患者を対象とする予定です。 臨床人体計測評価、動脈生体力学、TOD 検出、およびサンプル収集は、単一の医療行為で行われます。 生体力学、酸化ストレス、糖化およびグリコ酸化マーカーは、生体力学的異常および動脈 TOD がある患者とない患者の両方で定量化されます。 この段階の目的は、生体力学的パラメータと動脈 TOD に基づいて患者グループ間の差異を評価することです。 患者のサブグループでは、糖化およびグリコ酸化レベルに関する抗酸化酵素の構造機能の違い、および動脈生体力学的異常および TOD との関係も評価されます。 このフェーズには、プロジェクトの初期結果の達成に加えて、予備データの抽出、収集、処理、分析が含まれます。

数学的分析とモデリング
SPSS 22.0 統計ソフトウェア パッケージ (SPSS Inc.®、イリノイ州シカゴ、EE.UU) が使用されます。 量的変数の正規性の評価を含む、記述統計的アプローチが実行されます。 一変量統計は、変数の種類に応じて特定のテストでグループを比較するために使用されます。 関連する結果が見つかった場合は、線形相関研究が実施されます。 単変量解析の後、予測因子と結果の関連性を説明するとともに、生体力学的異常、TOD、心血管系の結果のリスクを予測する多変量モデルが構築されます。 生体力学的パラメータ、TOD、および心血管の結果に長期的に影響を与える可能性のある要因を評価するために、関節モデリングアプローチおよび時系列分析に関連する特定の数学的手順が適用されます。 サンプルサイズの計算は、変数のタイプと分布に応じた特定の統計検定に基づき、常に中程度の効果量、95% 信頼区間 (95%CI)、および少なくとも 80% 検出力を考慮します [33-35]。

GlycOxiTOD Registry Cohort
Adults enrolled in the GlycOxiTOD observational registry during evaluation for increased cardiovascular risk.
The cohort includes participants undergoing cardiovascular risk assessment and evaluation of vascular or other target-organ damage.
Participants may be followed through clinical visits and medical-record review to assess changes in cardiovascular risk factors, target-organ damage, treatments, health care use, and cardiovascular events.
No treatment is assigned by the registry.

Multimodal cardiovascular phenotyping may include clinical and anthropometric assessment; office and 24-hour ambulatory blood pressure monitoring; vascular ultrasound and arterial-function measurements; blood and urine biomarkers related to metabolism, glycation, oxidative stress, and inflammation; skin advanced glycation assessment; retinal imaging; and peripheral or cervical photoplethysmography. Selected measurements may be repeated to assess technical feasibility, agreement with reference methods, repeatability, reproducibility, and signal or image quality of non-invasive devices and analytical procedures.
Assessments depend on the applicable protocol or nested study, and not all participants undergo every procedure.

An ankle-brachial index greater than 1.40 is non-compressible.
Components are recorded separately.
Photoplethysmography-derived measures, investigational image-texture features, and device-estimated pulse wave velocity are excluded from the reference outcome.
A participant is positive if any criterion is met; negative only when carotid/femoral imaging, validated pulse wave velocity, and ankle-brachial index are all evaluable and below threshold; otherwise indeterminate.

Performance will be summarized using the area under the receiver operating characteristic curve with a 95% confidence interval. Sensitivity, specificity, positive and negative predictive values, and likelihood ratios may be reported at prespecified thresholds. Feature selection, signal preprocessing, quality criteria, and model version will be locked before confirmatory validation. Exploratory model development and confirmatory validation analyses will be reported separately. |

Analyses are limited to participants with comparable evaluable assessments at both time points.
