INVOERING
Atherosclerotische cardiovasculaire ziekte (ASCVD) vertegenwoordigt een van de belangrijkste oorzaken van morbiditeit en mortaliteit in de bevolking. Arteriële hypertensie (AHT), samen met andere cardiovasculaire risicofactoren (CVR), predisponeert voor afwijkingen in de structuur en functie van grote en kleine slagaders, wat leidt tot voortijdige vasculaire veroudering en culmineert in de ontwikkeling van doelorgaanschade (TOD), die een duidelijke maar ondergediagnosticeerde voorloper van ASCVD. Niet alle patiënten ontwikkelen echter TOD, en ook niet in dezelfde tijdsperiode of met dezelfde intensiteit. Bovendien wordt van patiënten die aanvankelijk een lage tot matige CVR vertonen, al aangenomen dat ze een matige tot hoge CVR hebben in de aanwezigheid van TOD, met strengere therapeutische doelstellingen [1,2].
Vasculaire biomechanica richt zich primair op de studie van mechanische eigenschappen en gedrag van slagaders. Ongunstige arteriële biomechanica wordt geassocieerd met de ontwikkeling en progressie van atherosclerose en arteriosclerose. Deze processen vertegenwoordigen een continuüm van vasculaire schade, gemedieerd door de accumulatie van geoxideerde lipoproteïnen in de subendotheliale ruimte, activering en amplificatie van ontstekingsroutes, en de ontwikkeling van arteriële fibrose en verkalking. De kern van deze verschijnselen ligt in het optreden van endotheliale disfunctie, gekenmerkt door verhoogde permeabiliteit en verlies van de filtercapaciteit van het vasculaire endotheel voor schadelijke moleculen naar de arteriële wand [3,4].
Oxidatieve stress, gedefinieerd als een onbalans tussen de productie van reactieve soorten en antioxidantsystemen ten gunste van de eerste, speelt een cruciale rol bij de ontwikkeling van endotheeldisfunctie, de afzetting van geoxideerde lipoproteïnen en pro-inflammatoire interactie tussen deze en het mononucleaire fagocytische systeem. De accumulatie van reactieve zuurstofsoorten (ROS) en andere induceert lipideperoxidatie- en glycoxidatieverschijnselen die culmineren in de vorming van geavanceerde lipoxidatie- en glycatie-eindproducten (respectievelijk ALE's en AGE's). ALE's en AGE's kunnen zich binden aan eiwitten, waardoor functionele en structurele afwijkingen ontstaan, en kunnen ook de cellulaire signalering beïnvloeden door onder andere doodsroutes te activeren [5,6].
Een ongunstige redoxstatus en glycatieniveaus houden verband met veroudering, degeneratieve ziekten en geaccumuleerde vasculaire schade bij bepaalde subpopulaties, met name bij patiënten met diabetes mellitus (DM). Er is echter weinig bekend over de invloed van glycatie en glycoxidatie van het antioxidantsysteem op de arteriële biomechanica en de ontwikkeling van doelorgaanschade bij ogenschijnlijk gezonde patiënten met een matige tot hoge CVR. We streven ernaar de specifieke relatie te bestuderen tussen arteriële biomechanische afwijkingen en TOD met serumglycatieniveaus, glycoxidatiestatus en functie-structuurbalans van sommige antioxidantsysteemelementen, met de nadruk op catalase, superoxide-dismutase en glutathionperoxidase [4,7,8].
MATERIALEN EN METHODES
Projectontwerp en -opstelling
Dit is een observationeel, ambispectief en multicenterproject dat wordt gepromoot door de Afdeling Hypertensie en Cardiovasculair Risico van de afdeling Interne Geneeskunde van het Universitair Ziekenhuis van Santiago de Compostela. Kandidaatpatiënten zijn personen die behoren tot de Organisatiestructuur van Geïntegreerd Management (EOXI) van Santiago de Compostela en Barbanza en die advies zoeken voor de diagnose, het beheer en de follow-up van cardiovasculaire risico's [9].
Deelnemers. Inclusie- en uitsluitingscriteria
Uit de totale pool van geraadpleegde patiënten zullen alleen personen ouder dan 18 jaar met een matig tot hoog CVR cardiovasculair risicoprofiel op basis van richtlijnreferentiescores worden voorgeselecteerd. Uitsluitingscriteria omvatten de aanwezigheid van DM volgens de huidige klinische praktijkrichtlijnen, rookgewoonte (gedefinieerd als huidige consumptie en binnen de 6 maanden voorafgaand aan de rekrutering), risicovolle alcoholconsumptie (gedefinieerd als consumptie van meer dan 10 g en 20 g per dag voor vrouwen en mannen respectievelijk) en vastgestelde hart- en vaatziekten [10-14].
Beoordeling van arteriële biomechanische markers en TOD
We zullen bilaterale echografie-onderzoek uitvoeren van de Common Carotis Artery (CCA), Internal Carotis Artery (ICA) en distale slagaders van de onderste ledematen met behulp van een Mindray Z60-apparaat (Mindray®, Guangdong, China) met hoogfrequente sonde. De volgende metingen en berekeningen worden daaruit gehaald:
- In de bewegingsmodus (M-modus), gelijktijdige meting van de arteriële straal, arteriële druk en totale wanddikte voor de berekening van rekbaarheids- en spanningsparameters in het middensegment van de CCA [15,16].
- In tweedimensionale modus (B-modus), geautomatiseerde meting van de dikte van de intima-media (IMT) in het distale gebied van de CCA, samen met detectie en karakterisering van cholesterolplaque [17].
- In spectrale Doppler-modus, morfologische beschrijving van de volume-tijd (stroom) functie in termen van stroomsnelheden en vasculaire weerstandsindices in het middensegment van de CCA en ICA [18].
- In B-modus met elastografiemodule, directe evaluatie van de lokale stijfheidsgraad in een middensegment van de CCA [19].
- In spectrale Doppler-modus met ECG-synchronisatie, beoordeling van de halsslagader-femorale pulsgolfsnelheid (c-f PWV) [20].
- In spectrale Doppler-modus: beoordeling van de enkel-armindex (ABI) [21].
We zullen ook beginnende schade in de retinale microvasculatuur beoordelen met behulp van retinografie met een Topcon TRC-NW6S-apparaat (Electronics®, Tokyo, Japan).
Beoordeling van oxidatieve stressmarkers en activiteitsstructuur van het antioxidantsysteem
Om de redoxstatus te beoordelen, zullen de niveaus van lipidenoxidatie (lipidenperoxidatie) en eiwitten in plasma en urine worden bepaald door respectievelijk thiobarbituurzuurreactieve stoffen (TBARS) en gereduceerde thiolen te kwantificeren, volgens de protocollen waarnaar wordt verwezen [22,23]. Beide methoden maken kwantificering via spectrofotometrische methoden mogelijk. De analyses zullen worden uitgevoerd met behulp van een Asys UVM-340-analysator (Biochrom®, Cambridge, VK). De enzymatische functie van glutathionperoxidase, catalase en superoxide-dismutase zal worden gemeten als enzymatische activiteit met een Asys UVM-340-analysator (Biochrom®, Cambridge, VK) volgens de protocollen waarnaar wordt verwezen [24,25]. Structuurkenmerken van antioxiderende enzymen gebaseerd op glycatie- en glycoxidatiepatronen zullen worden beoordeeld met behulp van proteomics waaronder SDS-PAGE, chromatografie en massaspectrometrie [26-28].
Bepaling van de glycatie- en glycooxidatiestatus
Het percentage geglyceerd hemoglobine zal worden beoordeeld met behulp van hoogwaardige vloeistofchromatografie (HPLC) met de Hemoglobine Next-analysator (A. Menarini Diagnostics®, Florence, Italië). Fructosamineniveaus zullen worden bepaald met behulp van de enzymatische methode van diazyme-geglyceerd serumeiwit (Diazyme, Kent, VK) op een Advia 2400-analysator (Siemens Healthcare Diagnostics Inc.®, Tarrytown, VS). Het percentage geglyceerde albumine zal worden geschat op basis van fructosamine- en albumineniveaus [29,30]. Deze laboratoriumbeoordeling wordt uitgevoerd door de afdeling Centraal Ziekenhuislaboratorium.
Algemene niveaus van glycoxidatie zullen worden gekwantificeerd door het bepalen van geavanceerde glycatie-eindproducten (AGE's) in de huid met behulp van een AGE Reader (DiagnOptics BV®, Groningen, Nederland). AGE's vertegenwoordigen voornamelijk geglyceerde adducten gebonden aan eiwitten die ontstaan tijdens glycoxidatieprocessen. De AGE Reader is ontwikkeld om de autofluorescentie (SAF) van de huid op niet-invasieve wijze te beoordelen met behulp van de fluorescerende eigenschappen van verschillende AGE’s [31].
Resultaten bij cardiovasculair risico
We zullen het aantal consultaties en onderzoekstijd dat nodig is om te behandelen (NNT) meten om afwijkingen in de arteriële biomechanica en doelorgaanschade (TOD) op te sporen, en zo een verhoogde CVR bepalen die therapeutische intensivering rechtvaardigt. Cardiovasculaire en cerebrovasculaire voorvallen, ziekenhuisopnames, consultaties, overlijden en invaliditeit zullen ook worden beoordeeld [32].
Ethische overwegingen en goede klinische praktijken
Dit onderzoek zal worden uitgevoerd in overeenstemming met de ethische principes van de Verklaring van Helsinki en de normen voor goede praktijken in het onderzoek van de Galicische (Spanje) Gezondheidsdienst (SERGAS). Van alle patiënten die hebben ingestemd met deelname zal schriftelijke geïnformeerde toestemming worden verkregen. De protocollen zijn goedgekeurd door de Research Ethics Committee van Santiago-Lugo (codes 2021/401 en 2023/302).
Werkplan en projectstroom
In een eerste fase zijn we van plan patiënten op te nemen die aan de criteria voldoen en toestemming geven om deel te nemen. Klinisch-antropometrische evaluatie, arteriële biomechanica, TOD-detectie en monsterafname zullen in één medische handeling worden uitgevoerd. Biomechanische, oxidatieve stress-, glycatie- en glycoxidatiemarkers zullen worden gekwantificeerd bij zowel patiënten met als zonder biomechanische afwijkingen en arteriële TOD. Het doel van deze fase is om de verschillen tussen patiëntengroepen te evalueren op basis van biomechanische parameters en arteriële TOD. In een subgroep van patiënten zullen ook verschillen in de structuur-functie van antioxiderende enzymen in termen van glycatie- en glycoxidatieniveaus, en hun relatie met arteriële biomechanische afwijkingen en TOD worden geëvalueerd. Deze fase omvat het extraheren, verzamelen, verwerken en analyseren van voorlopige gegevens, samen met het bereiken van de eerste resultaten van het project.
In een tweede fase,
- Patiënten zonder biomechanische afwijkingen of TOD zullen een follow-up van vijf jaar ondergaan, met jaarlijkse en eindevaluaties van biomechanische afwijkingen, TOD en cardiovasculaire uitkomsten. Het doel van deze fase is om verschillen in de voorgaande markers tussen patiëntengroepen te evalueren op basis van biomechanische parameters en arteriële TOD. In een subgroep van patiënten zullen ook verschillen in de structuur-functie van antioxiderende enzymen in termen van glycatie- en glycoxidatieniveaus, en hun relatie met arteriële biomechanische afwijkingen en TOD worden beoordeeld. Deze fase omvat het extraheren, verzamelen, verwerken en analyseren van voorlopige gegevens, samen met het bereiken van de eindresultaten van het project.
- Patiënten met biomechanische afwijkingen en/of TOD zullen een follow-up van 5 jaar ondergaan, met jaarlijkse en eindevaluaties van TOD-progressie en cardiovasculaire uitkomsten. Het doel van deze fase is om de verschillen in de voorgaande markers tussen patiëntengroepen op basis van arteriële TOD en cardiovasculaire uitkomsten te evalueren. In een subgroep van patiënten zullen verschillen in de structuur-functie van antioxiderende enzymen in termen van glycatie- en glycoxidatieniveaus, en hun relatie met TOD en cardiovasculaire uitkomsten, worden beoordeeld. Deze fase omvat het extraheren, verzamelen, verwerken en analyseren van voorlopige gegevens, samen met het bereiken van de eindresultaten van het project.
Wiskundige analyse en modellering
Er zal gebruik worden gemaakt van het statistische softwarepakket SPSS 22.0 (SPSS Inc.®, Chicago, IL, EE.UU). Er zal een beschrijvende statistische benadering worden uitgevoerd, inclusief beoordeling van de normaliteit voor kwantitatieve variabelen. Univariate statistieken zullen worden gebruikt om groepen te vergelijken met specifieke tests, afhankelijk van het type variabele. Als er relevante resultaten worden gevonden, zullen lineaire correlatiestudies worden uitgevoerd. Na univariate analyse zullen multivariate modellen worden geconstrueerd, zowel verklarend voor de associatie van voorspellers met de uitkomsten, als voorspellend voor het risico op biomechanische afwijkingen, TOD en cardiovasculaire resultaten. Om factoren te beoordelen die biomechanische parameters, TOD en cardiovasculaire resultaten in de loop van de tijd kunnen beïnvloeden, zullen specifieke wiskundige procedures gerelateerd aan gezamenlijke modelleringsaanpak en tijdreeksanalyse worden toegepast. Berekeningen van de steekproefgrootte zullen gebaseerd zijn op specifieke statistische tests op basis van het type variabele en de verdeling, waarbij altijd rekening wordt gehouden met een gematigde effectgrootte, een betrouwbaarheidsinterval van 95% (95% BI) en een power van ten minste 80% [33-35].

Inclusiecriteria:
- Patiënten van 18 jaar of ouder.
- Matig tot hoog cardiovasculair risico geschat door SCORE2OP.
- Ondertekende schriftelijke toestemming voor deelname aan het onderzoek.
Uitsluitingscriteria:
- Afwezigheid van de huidige rookgewoonte en in de afgelopen 6 maanden.
- Alcoholgebruik met een hoog risico (respectievelijk meer dan 10 en 20 g/dag bij vrouwen en mannen).
- Aanwezigheid van diabetes mellitus.
- Gevestigde hart- en vaatziekten, waaronder hartfalen, ischemische hartziekte, hartklepziekte, atriale fibrillatie, perifere aderziekte en cerebrovasculaire ziekte.

GlycOxiTOD Registry Cohort
Adults enrolled in the GlycOxiTOD observational registry during evaluation for increased cardiovascular risk.
The cohort includes participants undergoing cardiovascular risk assessment and evaluation of vascular or other target-organ damage.
Participants may be followed through clinical visits and medical-record review to assess changes in cardiovascular risk factors, target-organ damage, treatments, health care use, and cardiovascular events.
No treatment is assigned by the registry.

Multimodal cardiovascular phenotyping may include clinical and anthropometric assessment; office and 24-hour ambulatory blood pressure monitoring; vascular ultrasound and arterial-function measurements; blood and urine biomarkers related to metabolism, glycation, oxidative stress, and inflammation; skin advanced glycation assessment; retinal imaging; and peripheral or cervical photoplethysmography. Selected measurements may be repeated to assess technical feasibility, agreement with reference methods, repeatability, reproducibility, and signal or image quality of non-invasive devices and analytical procedures.
Assessments depend on the applicable protocol or nested study, and not all participants undergo every procedure.

|
Number and proportion of participants with baseline vascular target-organ damage or subclinical vascular disease, defined by at least one of the following: carotid plaque according to Mannheim criteria; femoral plaque according to prespecified ultrasound criteria; mean carotid intima-media thickness of 0.9 mm or greater; carotid-femoral pulse wave velocity greater than 10 m/s, or validated brachial-ankle pulse wave velocity greater than 14 m/s; or resting ankle-brachial index of 0.90 or lower.
An ankle-brachial index greater than 1.40 is non-compressible.
Components are recorded separately.
Photoplethysmography-derived measures, investigational image-texture features, and device-estimated pulse wave velocity are excluded from the reference outcome.
A participant is positive if any criterion is met; negative only when carotid/femoral imaging, validated pulse wave velocity, and ankle-brachial index are all evaluable and below threshold; otherwise indeterminate.

Number of affected vascular domains among: structural atherosclerotic damage (carotid or femoral plaque, or mean carotid intima-media thickness of 0.9 mm or greater); large-artery stiffness (carotid-femoral pulse wave velocity greater than 10 m/s or validated brachial-ankle pulse wave velocity greater than 14 m/s); and lower-extremity arterial disease (ankle-brachial index of 0.90 or lower).
The score ranges from 0 to 3. It is calculated only when all three domains are evaluable; otherwise, affected and evaluable domains are reported separately.
An ankle-brachial index greater than 1.40 is classified as non-compressible and not normal.

Participants without sufficient renal data are classified as indeterminate.

Performance will be summarized using the area under the receiver operating characteristic curve with a 95% confidence interval. Sensitivity, specificity, positive and negative predictive values, and likelihood ratios may be reported at prespecified thresholds. Feature selection, signal preprocessing, quality criteria, and model version will be locked before confirmatory validation. Exploratory model development and confirmatory validation analyses will be reported separately. |

Technical validity, agreement, repeatability, and reproducibility of selected non-invasive cardiovascular devices and measurement procedures.
Device-specific analyses may include mean difference or measurement bias versus an established reference method, limits of agreement, intraclass correlation coefficients, coefficients of variation, intraobserver and interobserver variability, proportion of interpretable measurements, acquisition time, and reasons for technical failure.
Results are reported separately for each device and measurement.

Progression is defined as development of a new affected vascular domain, an increase of at least one point in the 0-to-3 vascular damage burden score, or a prespecified adverse change in a quantitative vascular measure within an approved nested study.
Analyses are limited to participants with comparable evaluable assessments at both time points.
