  • 유방의 손상되지 않은 원발성(재발성 아님) 침윤성 암종의 진단

    • IV기 질환
    • 원발성 종양의 확인은 바늘 생검(선호됨)에 의해 이루어져야 합니다.
    • 유방에 만져질 수 있는 잔류 종양 또는 종양 이미지가 있는 한 절개 수술 생검 허용
  • 환자는 자유 절제면이 있는 완전 절제 후 방사선 요법(방사선 요법이 지시된 경우)의 대상자로 판단되어야 합니다.
  • 겨드랑이 절제술을 받지 않은 여성의 경우, 전초림프절 생검은 1-2개의 관련된 림프절의 겨드랑이 림프절 부하를 기록해야 합니다(즉, ACOSOG Z-11 기준이 적용될 수 있음).
  • 이전의 비침습성(Ductal Carcinoma In Situ) 암은 재발이 없는 경우 허용됩니다.
  • 이전 동측 침윤성 암이 5년 이상 이전에 허용된 경우

  • 환자는 원격 전이성 질환과 관련된 적어도 하나의 기관계를 가지고 있어야 합니다.

    • 환자에게 단 하나의 전이성 병변/병소가 있는 경우, 이는 생검으로 입증되어야 하며 전이 부위뿐만 아니라 원발성 유방암의 진단을 확인하는 병리 보고서가 있어야 합니다.
  • 전신 요법을 시작하기 전 지난 6주 이내에 질병 상태를 문서화한 이용 가능한 방사선학적 보고서가 있어야 합니다.
  • 예상 생존 기간이 6개월 이상인 경우 CNS 전이가 허용됨

  • 환자는 최소 16주 동안 최적의 전신 요법을 완료해야 합니다(종양 생물학적 프로필, 환자의 연령 및 폐경기 상태에 적합).

    • 전신 요법이 독성으로 인해 중단되었지만 먼 진행이 없고 최소 12주 동안 요법이 제공된 경우 환자는 계속 적격입니다.
    • 방사선 요법(지시된 경우)은 최종 치료 수술 절차(자유 절제면 재절제 및 액와 절제술 완료 포함) 12주 이내에 시작해야 합니다.

  • 환자는 IV기 유방암 진단 시점(적격 기준 충족 시)부터 최대 30주의 유도 전신 요법 완료 시점까지 언제든지 등록할 수 있습니다.

    • 환자는 전신 요법 시작 후 16-32주 이내에 무작위 배정되어야 합니다.

  • 다음을 포함하여 치료 전 및 무작위 배정 전 4주 +/- 2주 이내에 질병 상태에 대한 방사선학적 기록에 의해 입증된 바와 같이 환자는 전신 치료 시작 이후 질병 진행을 경험하지 않아야 합니다.

    • 새로운 질병 부위 없음
    • 가장 긴 직경에서 기존 사이트의 20% 이상 확대 없음
    • 증상 악화 없음
  • 유도 전신 요법 중 뼈 전이에 방사선 요법이 필요한 환자는 적격입니다.
  • 원발 부위의 국소 질환은 무증상이어야 합니다.
  • 알려진 호르몬 수용체 상태
  • 폐경기 상태가 지정되지 않음
  • 환자는 조사자의 재량 및 기관 지침에 따라 무작위화 4주 +/- 2주 전에 국소 치료를 받을 수 있는 적절한 장기 기능을 가지고 있어야 합니다.
  • 근치적으로 치료된 다른 원발성 암 이후 5년 이상
  • 음성 임신 테스트
  • 가임 환자는 허용되고 효과적인 피임 방법을 사용해야 합니다.
