기본 목표:

I. 경부 절제 후 림프절에 생존 가능한 종양이 있는 환자에서 동시 화학방사선 요법 후 잔류 질환이 의심되는 환자에서 아파티닙/위약 보조 요법을 받은 무병 생존(DFS)을 조사합니다.

2차 목표:

I. 아파티닙/위약의 재발률, 재발 패턴, 이차 원발성 악성종양의 발달, 전체 생존(OS) 및 독성을 평가합니다.

II. 아파티닙에 의한 표적 억제를 평가하고 아파티닙 내성의 바이오마커로서 순환하는 데옥시리보핵산(DNA)을 결정합니다.

개요: 환자는 2개의 치료군 중 1개로 무작위 배정됩니다.

ARM A: 환자는 1-28일에 1일 1회(QD) 아파티닙을 경구로(PO) 투여받습니다.

ARM B: 환자는 1-28일에 위약 PO QD를 받습니다.

양쪽 팔에서 질병 진행 또는 허용할 수 없는 독성이 없는 경우 최대 12개 과정 동안 28일마다 치료를 반복합니다.

연구 치료 완료 후 환자는 2년 동안 3개월마다, 1년 동안 6개월마다, 그 후 2년 동안 12개월마다 추적 관찰됩니다.

  • 동부 협력 종양학 그룹(ECOG) 수행 상태 0 또는 1
  • 환자는 구강, 구인두, 후두, 하인두 또는 p16 양성 알 수 없는 원발 부위의 국소적으로 진행된(III/IV기) HNSCC에 대한 1차 동시 화학방사선요법 후 생존 가능한 종양 세포가 있는 지속적인 1차 및/또는 림프절 질환의 병리학적 증거가 있어야 합니다. 머리와 목 부위; 생존 가능한 종양 세포가 있는 지속적인 원발성 및/또는 림프절 질환은 온전한 세포 구획(즉, haematoxylin 및 eosin(H&E) 염색에서 핵용해, 농농증 또는 핵정맥을 나타내지 않는 세포질 및 핵)
  • 환자는 화학방사선 요법 완료 후 목 절개를 받았어야 하며 치료 후 영상 연구에서 잔류 이상이 있는 결절 수준의 최소 구획 절개를 포함해야 하고/하거나 음성 절제면을 가진 잔류 원발 병변의 완전 절제를 받아야 합니다.
  • 환자는 화학방사선요법 후 원발성 질환 부위에서 완전 반응을 달성했거나 음성 절제면을 가진 잔여 원발성 질환의 완전 절제를 달성해야 합니다.
  • 모든 지속성 림프절 질환은 최소 66Gy의 방사선 요법을 받아야 하고 등록 전에 외과적으로 완전히 절제되어야 하며 외과적 절개는 적절하게 치유되어야 합니다.
  • 피막외 림프절 확장, 신경주위 또는 림프혈관 침범이 있는 환자는 자격이 있습니다.
  • 환자는 등록 당시 화학방사선 요법 완료 후 최소 6주(42일) 이상 36주(252일) 이하여야 합니다.
  • 환자는 삼키는 능력에 관계없이 자격이 있습니다. 삼킴곤란 환자는 위루관을 통해 아파티닙/위약을 투여할 수 있습니다.
  • 절대 호중구 수 >= 1,000/mm^3
  • 혈소판 >= 100,000/mm^3
  • 총 빌리루빈 =< 1.5 x 정상 상한(ULN)
  • 아스파르테이트 아미노 트랜스퍼라제(AST) =< 3 x ULN
  • 알라닌 아미노 트랜스퍼라제(ALT) =< 3 x ULN
  • 계산된 크레아티닌 청소율은 Cockcroft-Gault 공식을 사용하여 >= 50 ml/min이어야 합니다.
