조직학적으로 확인된 방광, 요도, 상부관 또는 림프절 양성(LN+) 질환의 근육 침윤성 요로상피암; 요로상피암이 우세한 한 허용되는 변이 조직학(편평 분화의 양은 얼마든지 허용됨); 신경 내분비 암종의 모든 구성 요소는 제외됩니다.
근육 침윤성 방광 종양의 경요도 절제술, 상부 관 절제술 또는 근치 방광 절제술/신장 절제술/요관 절제술/신장요관 절제술/방광전립샘절제술 또는 요도 절제술로 얻은 파라핀 조직 샘플을 이용할 수 있어야 합니다. 결과가 계층화에 사용되므로 이 표본 제출은 등록 전에 필수입니다. 근치/완전 수술(근치 방광절제술/신장절제술/요관절제술/신장요관절제술/방광전립샘절제술 및 LN 해부)에서 채취한 표본이 경요도 절제술보다 선호됩니다(가능한 경우).
환자는 다음 세 범주 중 하나에 속해야 합니다.
- 신보강 화학요법을 받았고 외과적 절제 시 병기가 >= pT2 및/또는 N+ OR인 환자
- 시스플라틴 적격이 아닌 환자(다음 기준 중 >= 1에 따름: Eastern Cooperative Oncology Group [ECOG] 수행 상태 2, 크레아티닌 청소율 < 60 mL/min, 등급 >= 2 청력 손실, 등급 >= 2 신경병증 , 또는 New York Heart Association 클래스 III 심부전 및 외과적 절제 시 병기가 >= pT3 또는 pN+임) 또는
- 의사와의 정보에 입각한 논의를 바탕으로 보조 시스플라틴 기반 또는 기타 전신 화학 요법을 거부하고 수술 절제 시 병리학적 단계가 >= pT3 또는 pN+인 환자
AJCC(American Joint Committee on Cancer) 병기 결정의 7판이 활용됩니다. 환자는 근치 방광 절제술(남성의 경우 방광전립선 절제술) 및 림프절 절제술(방광 원발의 경우), 또는 신장 절제술, 신요관 절제술 또는 요관 절제술(상부 종양의 경우) 또는 요도 절제술(근치 방광 절제술에 추가하여 동시에 또는 과거에)을 받았어야 합니다 > = 사전 등록 4주 전 =< 16주 전; 최종 요법으로 부분 방광 절제술을 받은 환자는 자격이 없습니다.
수술 마진에 육안 암 없음; 미세한 침윤성 요로상피암 양성 절제면이 허용됩니다. 마진의 암종(CIS)은 음의 마진으로 간주됩니다.
수술 후 잔여 암이나 전이의 증거가 없습니다. 상부 요로상피암 환자는 사전 등록 전 3개월 이내에 방광경검사 음성이어야 합니다. 방광이 제거된 경우 방광경 검사가 필요하지 않습니다.
단면 영상에서 전이성 질환 없음(또는 전이성 질환에 대해 "관련된" 방사선 소견)(고형 종양의 반응 평가 기준[RECIST] 버전 [v]1.1 기준에 따름)
