급성 외상성 출혈로 병원에 입원한 환자의 응고병증 관리에서 전혈 대 분획 혈액 제품을 사용한 수혈 요법의 평가
외상성 출혈의 예후는 수혈 요법의 특성과 관련이 있습니다. 혈장을 조기에 대량으로 공급하면 사망률이 50% 감소하고 혈소판을 조기에 대량으로 공급하면 사망률이 20% 감소합니다. 그러나 이 전략은 특히 집단적 비상 상황(공격)의 맥락에서 물류상의 어려움에 직면합니다. 군대에서 널리 문서화한 전혈의 사용은 혈장과 혈소판의 가용성을 향상시키고 체인의 다양한 행위자에 의한 취급을 단순화합니다.

에 결정된 대량 수혈 프로토콜의 시작이 필요한 중증 외상 환자
최소 2개의 위험 신호 점수 요소(병원 전 데이터에 따름):
- 의심되는 골반 골절
- 충격 지수(FC/PAS)> = 1
- 마이크로도즈 헤모글로빈 <13g
- 평균 혈압 <70mmHg
- 병원 전 기관 삽관이 필요합니다.
그리고 환자 도착 시 설정된 혈액 소비 평가(ABC) 점수의 최소 두 가지 기준:
- 관통 외상
- 외상을 위한 집중 복부 초음파 검사(FAST) 에코 양성
- 혈압 <90mmHg
- 호흡수 >120bpm
- 외상 환자 입원 후 6시간 이내에 최소 4개의 PRBC를 수혈해야 할 부상자의 치료를 담당하는 의사(임상 진단)의 예측에 대한 AND/OR
제외 기준:
- 비외상성 출혈
- 대규모 수혈 프로토콜을 시작하기 전에 2개 이상의 PRBC를 수혈한 환자.
- 항응고 치료
- 임신
- 연령 < 18세
- 혈액 제제 투여를 거부하는 환자
- 다른 병원에서 이송된 환자
- 환자 또는 의사가 배치된 병원 전 응급 의료 시스템에 의해 이송되지 않음
- 화상 환자(신체 표면의 ≥30%).
- 사회보장국 소속 없음
