BAKGRUNN: Legemidler som brukes i cellegiftbehandling virker på forskjellige måter for å stoppe tumorceller i å dele seg slik at de slutter å vokse eller dør. Strålebehandling bruker høyenergi røntgenstråler for å skade tumorceller. MÅL:

Bestem sviktfri overlevelse (FFS) hos pasienter med nylig diagnostisert lavrisiko rabdomyosarkom av embryonal eller botryoid subtype som oppfyller kriteriene for gruppe I etter behandling med daktinomycin og vinkristin med eller uten strålebehandling.
Bestem FFS-raten hos disse pasientene som oppfyller kriteriene for gruppe II etter behandling med daktinomycin, vinkristin og cyklofosfamid med eller uten strålebehandling.
Bestem FFS-frekvensen hos pasienter med ectomesenchymomas som inneholder rabdomyosarcomatous elementer (embryonal histiotype) som får en av de ovennevnte behandlingene.
Bestem nye molekylære markører spesifikke for embryonale og botryoide tumorhistologier som er av diagnostisk og prognostisk betydning hos pasienter behandlet med disse regimene.

OVERSIGT: Pasienter blir tildelt 1 av 2 grupper, avhengig av histologi og sykdomssted.

Gruppe I (gunstig tumorsted, negative lymfeknuter, stadium 1, klinisk gruppe I, IIA eller III (kun bane), node negativ [N0] ELLER ugunstig tumorsted, negative eller ukjente lymfeknuter, stadium 2, klinisk gruppe I) : Pasienter får vinkristin IV over 1 minutt ukentlig i 8 uker og Dactinomycin IV over 1 minutt hver 3. uke i 4 doser. Behandlingen gjentas hver 12. uke i 4 kurer. Radioterapi gis til pasienter med klinisk gruppe II eller III sykdom i uke 3-8.
Gruppe II (gunstig tumorsted, positive lymfeknuter, stadium 1, klinisk gruppe III (kun bane), nodepositiv [N1] ELLER gunstig tumorsted unntatt bane, eventuelle lymfeknuter, stadium 1, klinisk gruppe III ELLER ugunstig tumorsted, stadium 2, klinisk gruppe II ELLER ugunstig tumorsted, stadium 3, klinisk gruppe I eller II): Pasienter får vinkristin og daktinomycin som i gruppe I. Pasienter får også cyklofosfamid IV over 30-60 minutter og filgrastim (G-CSF) eller sargramostim ( GM-CSF) subkutant én gang daglig begynner 24 timer etter fullført kjemoterapi og fortsetter i 10 dager eller til blodtellingen er gjenopprettet. Strålebehandling gis i uke 3-8, 12-17 eller 28-33, hvis det er klinisk indisert som i gruppe I.

Pasientene følges hver 3.-4. måned i 3 år (4 år etter diagnose), hver 6. måned i 1 år, og deretter årlig.

PROSJERT PÅLEGGING: Totalt 254 pasienter for gruppe I vil bli påløpt til denne studien innen 6 år. Det vil påløpe ca. 12 pasienter per år for gruppe II.

SYKDOMENS KARAKTERISTIKA:

Histologisk bekreftet embryonalt (EMB) rabdomyosarkom (RMS) eller botryoid- eller spindelcellevarianter av EMB RMS eller embryonalt ektomesenchymom som oppfyller 1 av følgende kriterier:
- Stadium 1, ingen klinisk gruppe IV: Svulst på gunstig sted (bane, hode og nakke [unntatt parameningeal], genitourinært [ikke blære/prostata], eller galleveier) og ingen metastatisk sykdom
- Stadium 2 eller 3, klinisk gruppe I eller II: Svulst på ugunstig sted (blære/prostata, ekstremitet, kranial parameningeal, trunk, retroperitoneum, bekken, perineal/perianal, intrathorax, gastrointestinal eller lever), ingen grov gjenværende sykdom etter første operasjon , og ingen metastatisk sykdom
Må ha ipsilateral lymfeknutedisseksjon hvis 10 år eller eldre med primær paratestikkelkreft ELLER under 10 år med klinisk positive regionale lymfeknuter
Lav risiko for gjentakelse
