Próba Setmelanotydu w nabytej otyłości podwzgórzowej

Podwójnie ślepe, randomizowane, kontrolowane placebo badanie fazy III oceniające skuteczność i bezpieczeństwo stosowania setmelanotydu u pacjentów z nabytą otyłością podwzgórzową
Celem tego badania jest sprawdzenie, jak dobrze Setmelanotyd działa na poprawę redukcji masy ciała, uczucia głodu i jakości życia u pacjentów w wieku 4 lat i starszych z nabytą otyłością podwzgórzową (HO).
Aby określić, jak dobrze działa setmelanotyd i jak jest bezpieczny, pacjenci z HO będą codziennie przyjmować zastrzyk z setmelanotydu lub placebo i przeprowadzać oceny próbne przez okres do 60 tygodni.

- Udokumentowane dowody nabytej otyłości podwzgórzowej (HO)
- Wiek 4 lata i więcej
- Przyrost masy ciała związany z uszkodzeniem podwzgórza i BMI ≥30 kg/m2 u pacjentów w wieku ≥18 lat lub BMI ≥95 percentyla dla wieku i płci u pacjentów w wieku od 4 do <18 lat
- Zgodzić się na stosowanie wysoce skutecznej metody antykoncepcji w trakcie badania i przez 90 dni po jego zakończeniu
Kluczowe kryteria wykluczenia:
- Rozpoznanie zespołu Pradera-Williego (PWS) lub szybko rozwijającej się otyłości z hipowentylacją, dysregulacją podwzgórza, układu autonomicznego, zespołem guza neuroendokrynnego (ROHHADNET)
- Utrata masy ciała >2% w ciągu ostatnich 3 miesięcy u pacjentów w wieku ≥18 lat lub zmniejszenie BMI o >2% u pacjentów w wieku od 4 do <18 lat
- Operacja lub zabieg bariatryczny w ciągu ostatnich 2 lat
- Diagnoza ciężkich zaburzeń psychicznych; wszelkie myśli, próby lub zachowania samobójcze
- Obecna, klinicznie istotna choroba płuc, serca, metaboliczna lub onkologiczna
- Istotne zmiany dermatologiczne związane z czerniakiem lub zmianami przedczerniakowymi skóry (z wyłączeniem nieinwazyjnych zmian podstawnokomórkowych lub płaskonabłonkowych)
- Historia lub bliska rodzinna historia raka skóry lub czerniaka
- Udział w jakimkolwiek badaniu klinicznym z badanym lekiem/wyrobem w ciągu 3 miesięcy przed podaniem pierwszej dawki próbnej
- Wcześniej włączony do badania klinicznego z udziałem setmelanotydu lub jakiejkolwiek wcześniejszej ekspozycji na setmelanotyd
- Niezdolność do przestrzegania schematu wstrzyknięć raz dziennie (QD).
- W przypadku kobiet w ciąży i/lub karmiących piersią
- Pacjenci z otyłością, którą można przypisać innym schorzeniom genetycznym lub zespołowym (np. PPL [POMC, PCSK1, LEPR łącznie], BBS) przed uszkodzeniem podwzgórza.
- W przypadku otrzymywania hormonalnej terapii zastępczej dawka pozostaje stabilna przez co najmniej 2 miesiące przed badaniem przesiewowym

BMI Z-Score calculated for participants <18 years old only is a measure of relative weight adjusted for child's age, sex and height at the time of data collection.

A Z-score of 0 is equal to the mean of a reference population (i.e., healthy, age and sex-matched individuals).

Participants ≥12 years of age who were able to self-report were administered the Symptoms of Hyperphagia: Patient (Version 1.0).
The questionnaire consisted of 4 items related to the frequency of a participant's hunger symptoms on a scale (Never, 1 or 2 times, 3 or more times) over the past 24 hours.
The scores on this scale range from 0 to 2 with higher scores indicating more hyperphagia.
Daily composite score was derived as the sum of daily answered questions divided by the number of answered questions per day.
The weekly average of the daily composite scores equals to the sum of daily composite scores divided by the number of days with a composite score within the 7 identified days prior to the visit.

The IWQOL-Lite-Clinical Trials (administered to participants ≥18 years of age) is a validated 20-item self-report measure of obesity-specific quality of life questionnaire.
It assessed 2 primary domains of obesity-related health-related quality of life (HRQoL): physical (7 items) and psychosocial (13 items).
Each item was rated on a scale from 0 (worst) to 100 (best), with higher scores indicating better levels of functioning.
It provided composite scores for each domain, as well as a total score, all ranging from 0 (worst) to 100 (best).
Higher scores reflect better levels of functioning and quality of life.
LSM and SE were calculated using ANCOVA model.

Results below represent the total score, which is rated on a scale from 0 (worst) to 100 (best), with higher scores indicating better levels of functioning.
