Descrição detalhada
Estes são os seguintes objetivos secundários:
- Determinar se a irradiação do SNC pode ser omitida com segurança no contexto da terapia sistêmica utilizada no protocolo.
- Identificar se infusões intravenosas prolongadas (24 horas) de HDMTX produzem maior acúmulo de poliglutamato de metotrexato (MTXPG) do que infusões curtas (4 horas) 42 horas após 1 gm/m2 de HDMTX, estratificado para linhagem (linhagem T vs B) e ploidia (linhagem B hiperdiploide vs não hiperdiploide).
- Determinar se infusões intravenosas prolongadas (24 horas) de HDMTX produzem maiores efeitos antileucêmicos do que infusões curtas (4 horas), com base na inibição da síntese de purina de novo em blastos de medula óssea e na diminuição de blastos circulantes durante a "janela" de 4 dias antes do início da terapia de indução de remissão convencional.

Detalhes do Plano de Tratamento
O tratamento consistirá em três fases principais, Indução da Remissão, Consolidação e Continuação. O tratamento com uma Janela Upfront HDMTX para fins de pesquisa será opcional.
Todos os pacientes receberão terapia IT no dia 1, a dose depende da idade.
Janela Inicial de Metotrexato de Alta Dose
HDMTX (1 g/m2) como uma infusão de 4 horas versus uma infusão de 24 horas. O resgate de leucovorina será dado.
Indução de remissão
Prednisona 40 mg/m2/dia PO Dias 5 - 32 Vincristina 1,5 mg/m2/semana IV Dias 5, 12, 19, 26 Daunorrubicina 25 mg/m2/semana IV Dias 5, 12 L-asparaginase 10.000 Unidade/m2/dose IM Dias 6, 8, 10, 12, 14, 16 (19, 21, 23) Ciclofosfamida 1000 mg/m2/dose IV Dia 26 Citarabina 75 mg/m2/dose IV Dias 27-30, 34-37 6-Mercaptopurina 60 mg /m2/dose PO Dias 26-39 Imatinib 40 mg/m2 bid para doentes Ph positivos a partir do Dia 22 da indução. A terapia intratecal será administrada no dia 1 e 19, dependendo da idade da dose. Pacientes com alto risco de recidiva do SNC receberão tratamentos IT adicionais nos dias 8 e 26.
Tratamento de consolidação
Dose alvo de metotrexato de alta dose dependendo do status de risco, dias 1, 15, 29 e 43 e mercaptopurina 50 mg/m2/dia, dias 1-56.
Tratamento de reintensificação para pacientes com doença de alto risco:
Pacientes com doença de alto risco terão a opção de transplante de células-tronco hematopoiéticas (HSCT) e podem receber 1-2 ciclos adicionais de tratamento de reintensificação antes de maximizar a morte antileucêmica antes do transplante.

Reindução I e II
Esta fase do tratamento será iniciada nas semanas 7 e 17 após o exame da medula óssea confirmar a remissão completa. O tratamento de reindução será administrado duas vezes: semanas 7 a 9 e semanas 17 a 19 para todos os pacientes.

Mercaptopurina 75 mg/m2 PO, diariamente x 7 dias, Dias 1-7 Metotrexato 40 mg/m2 IV ou IM, Dia 1 Ciclofosfamida 300 mg/m2 IV, Dia 1 Citarabina 300 mg/m2 IV, Dia 1 Dexametasona 12 mg/m2 (std/alto risco) ou 8 mg/m2 (baixo risco) PO diariamente (tid) x 5, Dia 1-5 Vincristina 2,0 mg/m2 IV push (máx. 2 mg), Dia 1
O mesmo tratamento (semanas 21-28) será repetido por um total de 6 vezes (até a semana 68). Após a semana 68, todos os pacientes receberão 6MP diariamente e MTX semanal com pulsos de dexametasona e vincristina a cada 4 semanas até a semana 100, após a qual serão administrados apenas 6MP e metotrexato. O tratamento intratecal será administrado a cada 8 semanas apenas para pacientes com alto risco de recidiva do SNC após a semana 48 e será descontinuado após a semana 96. A terapia de continuação será descontinuada após 120 semanas em meninas e após 146 semanas em meninos
Os pacientes que atendem aos critérios de LLA de alto risco são candidatos ao transplante alogênico de células-tronco hematopoiéticas. No entanto, se a opção for recusada pelos pacientes ou responsáveis, ou o procedimento for considerado inadequado pelo médico assistente e pelo investigador principal, o paciente permanecerá no estudo e continuará recebendo quimioterapia

Critério de inclusão:
- Diagnóstico de LLA não de células B por imunofenotipagem, conforme determinado pelo padrão de reatividade a um painel de anticorpos monoclonais com citometria de fluxo, bem como morfologia e coloração citoquímica.
- Faixa etária: 1 a 18 anos (inclusive).
Critério de exclusão:
• Tratado anteriormente com quimioterapia por uma semana ou mais.
