Estabelecimento e transformação clínica do sistema de avaliação de eficácia de medicamentos ADC para câncer de mama com base em imagens moleculares

O objetivo do estudo é construir uma abordagem não invasiva 68Ga-TTP PET/CT para detectar a expressão de TROP2 de lesões tumorais em pacientes com tumores de mama e avaliar a eficácia da terapia medicamentosa ADC.

Critérios de inclusão:
- Idade entre 18 e 75 anos, masculino ou feminino, pontuação ECOG 0 ou 1 ponto (ver tabela no Anexo 2 para tabela de pontuação);
- A rotina sanguínea e a função hepática e renal atendem aos seguintes critérios: Rotina sanguínea: leucócitos ≥ 4,0×109 L ou neutrófilos ≥1,5×109 /L, PLT ≥ 100×109/L, Hb≥ 90 g/L; PT ou APTT ≤ 1,5 LSN; Função hepática e renal: T-Bil ≤ 1,5×ULT (limite superior do normal), ALT e AST≤ 2,5 ULN ou ≤ 5×ULN (indivíduos com metástase hepática), ALP ≤ 2,5 ULN (ALP ≤ 4,5 ULN se houver osso metástase ou metástase hepática); BUN ≤ 1,5×ULT, SCr ≤ 1,5×ULT;
- Pacientes com câncer de mama confirmado ou suspeito;
- Sobrevida esperada ≥12 semanas;
- Boa conformidade de acompanhamento;
- Mulheres em idade fértil (15 a 49 anos) deverão realizar teste de gravidez até 7 dias antes do início do teste e o resultado for negativo; Homens e mulheres férteis devem consentir com o uso de métodos contraceptivos eficazes para garantir a gravidez durante o período do estudo e dentro de 3 meses após o exame;
- Os pacientes sujeitos podem compreender totalmente e participar voluntariamente deste experimento e assinar o consentimento informado.
Critérios de exclusão:
- Função hepática e renal anormal grave;
- Mulheres grávidas, grávidas e lactantes;
- Não pode ficar deitado por meia hora;
- Incapaz de obter consentimento informado;
- Sofrer de claustrofobia ou outras doenças mentais;
- Pessoas que são alérgicas ao medicamento experimental ou aos seus excipientes na terapia experimental;
- Outras condições consideradas inadequadas para participação no ensaio pelo investigador.

Experimental: Diagnostic Validation Cohort
Patients with newly diagnosed or suspected breast cancer will undergo one [68Ga]Ga-TTP scan and one [18F]FDG scan within 1 week, in randomized order, with no anticancer treatment administered between the two imaging examinations.
This cohort is used to evaluate tumor targeting, compare diagnostic performance with [18F]FDG, and correlate imaging findings with pathology and TROP2 expression.

[18F]FDG is administered intravenously as the comparator PET radiotracer.
In the diagnostic validation cohort, participants undergo one [18F]FDG PET examination within 1 week of the [68Ga]Ga-TTP scan, with no treatment between scans.
In the ADC response assessment cohort, participants undergo [18F]FDG PET imaging at the same three time points as [68Ga]Ga-TTP: before ADC therapy, after 2 cycles(each cycle is 21 days), and at disease progression.
The planned administered activity is approximately 0.1-0.15
mCi/kg.
[68Ga]Ga-TTP is a TROP2-targeted PET radiotracer administered intravenously for PET imaging.
In the diagnostic validation cohort, participants undergo one [68Ga]Ga-TTP PET examination within 1 week of the comparator [18F]FDG scan.
In the ADC response assessment cohort, participants undergo [68Ga]Ga-TTP PET imaging before ADC therapy, after 2 cycles of treatment(each cycle is 21 days), and at disease progression.
The planned administered activity is approximately 0.05-0.1 mCi/kg.

Experimental: ADC Response Assessment Cohort
Patients with breast cancer who are planned to receive TROP2-directed ADC therapy will undergo paired [68Ga]Ga-TTP and [18F]FDG imaging at three time points: before treatment, after 2 cycles of ADC therapy(each cycle is 21 days), and at disease progression.
This cohort is used to assess longitudinal imaging changes and their relationship with treatment response.

[18F]FDG is administered intravenously as the comparator PET radiotracer.
In the diagnostic validation cohort, participants undergo one [18F]FDG PET examination within 1 week of the [68Ga]Ga-TTP scan, with no treatment between scans.
In the ADC response assessment cohort, participants undergo [18F]FDG PET imaging at the same three time points as [68Ga]Ga-TTP: before ADC therapy, after 2 cycles(each cycle is 21 days), and at disease progression.
The planned administered activity is approximately 0.1-0.15
mCi/kg.
