ODŮVODNĚNÍ: Léky používané v chemoterapii působí různými způsoby, aby zabránily dělení nádorových buněk, takže přestanou růst nebo zemřou. Kombinace více než jednoho léku může zabít více nádorových buněk.

ÚČEL: Randomizovaná studie fáze III k porovnání různých režimů kombinované chemoterapie u pacientů s pokročilým karcinomem močového měchýře.

Hlavní

  • Porovnejte přežití pacientů s kompletně resekovaným lokálně pokročilým karcinomem z přechodných buněk močového měchýře léčených adjuvantně doxorubicinem a gemcitabinem následovaným paklitaxelem a cisplatinou vs. adjuvantní cisplatinou a gemcitabinem.

  • Porovnejte profily toxicity těchto režimů u těchto pacientů.

PŘEHLED: Toto je randomizovaná studie. Pacienti jsou stratifikováni podle stavu primárního nádoru (<T4 vs. T4), počtu pozitivních lymfatických uzlin (0 nebo neznámé vs. 1-5 vs. >5) a počtu disekovaných uzlin (0-10 nebo neznámé vs > 10). Pacienti jsou randomizováni do jednoho ze dvou léčebných ramen.

  • Rameno I: Pacienti dostávají adjuvantní gemcitabin IV po dobu 30 minut ve dnech 1, 8 a 15 a cisplatinu IV po dobu 30-60 minut v den 1. Léčba se opakuje každé 4 týdny po dobu až 4 cyklů v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity.
  • Rameno II: Pacienti dostávají adjuvantní doxorubicin IV po dobu 45 minut a následně gemcitabin IV po dobu 2 hodin v den 1. Pacienti také dostávají filgrastim (G-CSF) subkutánně (SC) denně ve dnech 3-10. Léčba se opakuje každých 14 dní až ve 4 cyklech v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity. Počínaje 14 dny po ukončení léčby doxorubicinem a gemcitabinem dostávají pacienti paclitaxel IV po dobu 3 hodin a cisplatinu IV po dobu 20-30 minut v den 1. Pacienti také dostávají G-CSF SC denně ve dnech 3-10 nebo 4-11. Léčba se opakuje každých 14 dní až ve 4 cyklech v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity.

Pacienti jsou sledováni každé 3 měsíce po dobu 18 měsíců, každých 6 měsíců po dobu 18 měsíců a poté každý rok.

CHARAKTERISTIKA ONEMOCNĚNÍ:

  • Histologicky potvrzený přechodný buněčný karcinom močového měchýře

    • Jakékoli T, N+, M0 NEBO
    • T3a-4a, NO, M0

  • Nejméně 6 týdnů, ale ne více než 3 měsíce od předchozí kompletní resekce močového měchýře

    • Cystoprostatektomie plus disekce pánevních lymfatických uzlin u mužů
    • Cystektomie/totální abdominální hysterektomie/bilaterální salpingo-ooforektomie plus disekce pánevních lymfatických uzlin u žen
    • Negativní chirurgické okraje
  • Karcinom in situ povolen
  • Žádná parciální cystektomie nebo transuretrální resekce tumoru močového měchýře jako definitivní lokoregionální terapie
  • Karcinom prostaty detekovaný v resekovaném vzorku povolen, pokud pooperační PSA není vyšší než 0,05 mg/dl
  • Žádné metastatické nebo recidivující onemocnění při pooperačním RTG hrudníku a CT vyšetření břicha a pánve

  • 18 a více

Stav výkonu:

  • ECOG 0-1

Hematopoetický:

  • Absolutní počet neutrofilů ≥ 1 500/mm^3
  • Počet krevních destiček ≥ 150 000/mm^3

Jaterní:

  • Bilirubin ≤ 1,5násobek horní hranice normálu (ULN)
  • Alkalická fosfatáza ≤ 2krát ULN
  • AST ≤ 2krát ULN

Renální:

  • Kreatinin ≤ 1,5 mg/dl OR
  • Clearance kreatininu ≥ 60 ml/min

Kardiovaskulární:

  • Normální srdeční funkce podle anamnézy, fyzikálního vyšetření nebo rentgenového snímku hrudníku
  • Ejekční frakce levé komory alespoň 50 % podle radionuklidového ventrikulogramu nebo echokardiogramu, pro předchozí srdeční onemocnění
  • Žádné srdeční onemocnění třídy III nebo IV podle New York Heart Association
  • Žádné závažné srdeční arytmie včetně srdečního bloku prvního, druhého a třetího stupně

Jiný:

  • Negativní těhotenský test
  • Žádná nekontrolovaná infekce
  • Žádná jiná aktivní rakovina za posledních 5 let kromě nemelanomové rakoviny kůže nebo kurativního karcinomu in situ děložního čípku
