Akupresura ušního bodu k léčbě neuropatie indukované chemoterapií

Navrhovaná randomizovaná kontrolní studie vyhodnotí aurikulární bodovou akupresuru (APA) u neuropatie vyvolané chemoterapií (CIN), přičemž pečlivě zváží bodovou specifitu a účinky placeba integrací self-report opatření, psychofyzických měření (QST), endogenních biomarkerů (cytokiny) a neuropatie. -zobrazování ke zkoumání účinnosti APA a základního mechanismu(ů).

Detailní popis

Chemoterapií indukovaná neuropatie (CIN) – bolest, necitlivost nebo mravenčení distribuované v rukou a nohou – vytváří přetrvávající symptomy ovlivňující citlivost a rovnováhu u pacientů, kteří přežili rakovinu. Až 50 % pacientů, kteří přežili rakovinu, stále trpí CIN 6 let po léčbě. Bylo zjištěno, že duloxetin, jediný lék doporučený Americkou společností klinické onkologie, je lepší než placebo, ale zlepšil CIN pouze o 0,73 bodu (škála 0-10). Nebyla stanovena žádná účinná léčba CIN kromě cvičení s velikostí účinku <0,508. Opioidy zmírňují bolest CIN, ale dlouhodobé užívání se důrazně nedoporučuje kvůli nadměrnému užívání opioidů.

Výzkumníci navrhují testovat aurikulární bodovou akupresuru (APA), inovativní a škálovatelné řešení vyvinuté z aurikulární akupunktury. APA je neinvazivní (bezjehlová) a aktivní léčba pro pacienty s bolestí, zatímco akupunktura je invazivní (pomocí jehel) a pasivní léčba (aplikovaná licencovaným lékařem). V APA jsou malá semínka nalepena na konkrétní ušní body zkušeným poskytovatelem a pacienti tlačí na semena, aby stimulovali ušní body třikrát denně, vždy tři minuty, celkem devět minut denně. Kritéria pro zařazení:

  • pacientů s rakovinou ve věku ≥ 18 let
  • dostali lék v jedné z následujících kategorií: na bázi platiny, vinca alkaloidy, bortezomib, eribulin a/nebo taxany
  • dokončili chemoterapii tři měsíce nebo déle před zařazením
  • máte CIN v důsledku neurotoxické chemoterapie na rakovinu nebo máte již existující periferní neuropatii jiné etiologie, která se zhoršila po chemoterapii
  • mají jednu z průměrných intenzit bolesti, necitlivosti nebo brnění na končetinách předchozí týden v důsledku CIN ≥ 4 na 11bodové numerické škále.

Kritéria vyloučení:

  • použití zkoumané látky pro kontrolu bolesti současně nebo během posledních 30 dnů
  • použití implantovatelného systému pro dodávání léků, např. Medtronic SynchroMed®
  • předchozí blokáda celiakálního plexu nebo jiná neurolytická léčba bolesti
  • jiné identifikované příčiny bolestivé parestézie existující před chemoterapií (např. ozařování nebo maligní plexopatie, lumbální nebo cervikální radikulopatie,)

Milligram Morphine Equivalent (MME) will be determined by using an equivalency factor to calculate a dose of morphine equivalent to the ordered opioid.
