Detaillierte Beschreibung
Derzeit liegt die Rate ipsilateraler Brusttumorrezidive (IBTR) nach brusterhaltender Operation und Strahlentherapie bei 10 % nach 10 Jahren bzw. 15 % nach 20 Jahren. IBTR ist ein unabhängiger Prädiktor für ein schlechtes Überleben mit einem um 3 bis 4,6 erhöhten Risiko für krebsbedingten Tod.
In einer heterogenen Population bleibt die lokale Kontrolle eine große therapeutische Herausforderung für diese Schübe, insbesondere solche, die eine bessere Prognose haben, nämlich spät auftreten und von niedrigem histologischem Grad sind. Bei diesen Schüben ist eine therapeutische Deeskalation möglich, um eine verstümmelnde und oft traumatische Mastektomie zu vermeiden.
Diese zweite konservative Operation hat jedoch eine hohe Rate an zweiten lokalen Rezidiven (19 bis 50 % nach 5 Jahren) aufgrund des Fehlens einer erneuten Bestrahlung, was durch das Problem der Verträglichkeit von zuvor bestrahltem Gewebe unmöglich gemacht wird.
Retrospektive oder prospektive Studien zur partiellen Brustbestrahlung (PBI) im adjuvanten Setting zeigen vielversprechende Ergebnisse, sowohl hinsichtlich der Verträglichkeit (Schonung von gesundem Gewebe) als auch der lokalen Kontrolle (74 % bis 100 % nach 5 Jahren). Zu den verwendeten Techniken gehören Brachytherapie, externe Strahlentherapie und intraoperative Strahlentherapie (IORT).
IORT ist jetzt Gegenstand eines erneuten Interesses an Brustkrebs. Es hat die Vorteile einer hochpräzisen Ballistik auf dem operierten Areal und der Schonung von gesundem Gewebe. Bisher wurden jedoch keine prospektiven Daten in der Indikation ipsilaterales Mammarezidiv publiziert.
Ein prospektives, multizentrisches, einarmiges Phase-II-Design wird die Durchführbarkeit einer wiederholten brusterhaltenden Operation in Kombination mit einer erneuten Bestrahlung mittels IORT nach einem lokalen Rezidiv eines Mammakarzinoms evaluieren.

- Histologisch nachgewiesenes nicht-lobuläres invasives Brustrezidiv
- Zeit von der Ganzbrustbestrahlung nach der initialen Lumpektomie >5 Jahre
- Unifokaler Tumor
- Rezidivierende Tumorgröße ≤ 2 cm
- Angemessene Brustgröße für eine zweite brusterhaltende Operation mit akzeptablem kosmetischem Ergebnis
- Bilaterale Brustmammographie innerhalb von 90 Tagen vor Studieneintritt
- Mamma-MRT innerhalb von 90 Tagen vor Studieneintritt
- Histologischer Grad I-II
- Östrogenrezeptor-positiver Tumor (ER+)
- Cerb2-negativer Tumor
- N0
- M0
- Vorherige Strahlentherapie innerhalb eines standardmäßigen Fraktionierungsplans
- Leistungsstatus (ECOG) 0-1
- Frauen ≥ 50 Jahre - Fehlen jeglicher psychologischer, familiärer, soziologischer oder geografischer Bedingungen, die die Einhaltung des Studien- und Nachsorgeplans beeinträchtigen könnten
- Angeschlossen an die französische Krankenversicherung
- Schriftliche und unterschriebene Einverständniserklärung.
Ausschlusskriterien:
- Multifokales und/oder multizentrisches Rezidiv
- Lobuläres Karzinom
- Östrogenrezeptor-negativer Tumor (ER-)
- Cerb2 (her2) überexprimiert - Brustkrebs
- Umfangreiche intraduktale Komponente (EIC) bei der Biopsie
- Lymphgefäßinvasion bei Biopsie
- N1-3-Status: Regionales zytologisches oder histologisch gesichertes Knotenrezidiv
- M1-Status: Metastasierende Erkrankung
- cT4 (Haut- oder Muskelbeteiligung) oder Morbus Paget der Brustwarze
- Vorherige Strahlentherapie, die innerhalb eines beschleunigten oder hypofraktionierten Zeitplans durchgeführt wurde
- Früherer bösartiger anderer Hautkrebs als Nicht-Melanom-Hautkrebs, es sei denn, der Patient ist seit mindestens 5 Jahren krankheitsfrei
- Patientinnen mit kleinem Brustvolumen, technisch unbefriedigend für eine zweite konservative Operation oder intraoperative Brustbestrahlung.
- Präoperative Chemotherapie oder Hormontherapie bei lokalem Rezidiv
- Bindegewebserkrankung oder Sklerodermie, die eine Strahlentherapie kontraindiziert
- Bekannte Mutation des BRCA1/2-Gens (Gentests sind nicht erforderlich)

Experimental: Chirurgie und intraoperative Strahlentherapie
Operation : Die Tumorektomie wird nach den aktuellen Standards durchgeführt, wobei "klare" Ränder erzielt werden. Die axilläre Lymphknotenkontrolle hängt von der anfänglichen Behandlung (klinisch und Ultraschall) dieser N0-Patienten ab, die von den Teams ausgewählt wird. Intraoperative Strahlentherapie (IORT): Nach der Exzision des Tumors wird IORT geliefert. Eine Einzeldosis von 20 Gy mal 50 kV Photonen (Intrabeam™) wird im Tumorbett verabreicht. Das Hinzufügen von IORT ändert das chirurgische Verfahren nicht. |
