Momenteel blijft het percentage ipsilaterale borsttumorrecidief (IBTR) na borstsparende chirurgie en radiotherapie respectievelijk 10% na 10 jaar en 15% na 20 jaar. IBTR is een onafhankelijke voorspeller van slechte overleving met een 3 tot 4,6 verhoogd risico op overlijden door kanker.
In een heterogene populatie blijft lokale controle een grote therapeutische uitdaging voor deze recidieven, vooral die waarvan wordt aangenomen dat ze een betere prognose hebben, namelijk laat optredend en van lage histologische graad. Therapeutische de-escalatie is mogelijk voor deze recidieven om een mutilerende en vaak traumatische borstamputatie te voorkomen.
Deze tweede conservatieve operatie heeft echter een hoog percentage tweede lokale terugval (19 tot 50% na 5 jaar) vanwege de afwezigheid van herbestraling, onmogelijk gemaakt door het probleem van tolerantie van eerder bestraalde weefsels.
Retrospectieve of prospectieve studies over partiële borstbestraling (PBI) in adjuvante setting rapporteren veelbelovende resultaten, zowel wat betreft tolerantie (gezond weefsel sparen) als lokale controle (74% tot 100% na 5 jaar). Gebruikte technieken zijn onder meer brachytherapie, uitwendige bestraling en intraoperatieve radiotherapie (IORT).
IORT is nu het onderwerp van hernieuwde belangstelling voor borstkanker. Het heeft de voordelen van zeer nauwkeurige ballistiek op het geopereerde gebied en het behoud van gezond weefsel. Tot op heden zijn er echter geen prospectieve gegevens gepubliceerd over de indicatie ipsilateraal borstrecidief.
Een prospectief, multicenter, eenarmig Fase II-ontwerp zal de haalbaarheid evalueren van herhaalde borstsparende chirurgie in combinatie met herbestraling met IORT na lokaal recidief van mammacarcinoom.

Inclusiecriteria:
- Histologisch bewezen niet-lobulaire invasieve borstrecidief
- Tijd vanaf bestraling van de gehele borst na de initiële lumpectomie >5 jaar
- Unifocale tumor
- Terugkerende tumorgrootte ≤ 2 cm
- Voldoende borstomvang voor een tweede borstsparende operatie met acceptabel cosmetisch resultaat
- Bilateraal borstmammogram binnen 90 dagen voorafgaand aan deelname aan het onderzoek
- Borst-MRI binnen 90 dagen voorafgaand aan deelname aan het onderzoek
- Histologische graad I-II
- Oestrogeen-receptor-positieve tumor (ER+)
- Cerb2-negatieve tumor
- N0
- M0
- Voorafgaande radiotherapie toegediend binnen een standaard fractioneringsschema
- Prestatiestatus (ECOG) 0-1
- Vrouwen ≥ 50 jaar - Afwezigheid van enige psychologische, familiale, sociologische of geografische omstandigheden die de naleving van het studie- en follow-upschema kunnen belemmeren
- Aangesloten bij de Franse ziektekostenverzekering
- Schriftelijk en ondertekend geïnformeerd toestemmingsformulier.
Uitsluitingscriteria:
- Multifocaal en/of multicenter recidief
- Lobulair carcinoom
- Oestrogeen-receptor-negatieve tumor (ER-)
- Cerb2 (her2) tot overexpressie gebracht - borstkanker
- Uitgebreide intraductale component (EIC) op biopsie
- Lymfevatinvasie op biopsie
- N1-3-status: regionaal cytologisch of histologisch bewezen klierrecidief
- M1-status: gemetastaseerde ziekte
- cT4 (huid- of spierbetrokkenheid) of de ziekte van Paget van de tepel
- Eerdere radiotherapie toegediend binnen een versneld of hypofractieschema
- Eerdere maligniteiten anders dan niet-melanome huidkanker, tenzij de patiënt ten minste 5 jaar ziektevrij is
- Patiënten met een klein borstvolume, technisch onvoldoende voor een tweede conservatieve operatie of intraoperatieve borstbestraling.
- Preoperatieve chemotherapie of hormoontherapie voor lokale terugval
- Bindweefselziekte of sclerodermie, contra-indicatie voor radiotherapie
- Bekende BRCA1/2-genmutatie (genetische tests zijn niet vereist)

Experimenteel: chirurgie en intra-operatieve radiotherapie
Chirurgie : Tumorectomie zal worden uitgevoerd volgens de huidige normen, waarbij "duidelijke" marges worden verkregen. De controle van de oksellymfeklieren zal afhangen van de initiële behandeling (klinisch en echografisch) van deze N0-patiënten, gekozen door de teams. Intra-operatieve radiotherapie (IORT): Na de excisie van de tumor wordt IORT afgeleverd. Een enkele dosis van 20 Gy bij 50 kV fotonen (Intrabeam™) wordt toegediend in het tumorbed. De toevoeging van IORT wijzigt de chirurgische procedure niet. |
