Actuellement, le taux de récidive homolatérale de la tumeur du sein (IBTR) après chirurgie conservatrice du sein et radiothérapie reste respectivement de 10 % à 10 ans et de 15 % à 20 ans. L'IBTR est un facteur prédictif indépendant de faible survie avec un risque accru de 3 à 4,6 de décès lié au cancer.
Dans une population hétérogène, le contrôle local reste un enjeu thérapeutique majeur de ces poussées, notamment celles considérées comme de meilleur pronostic, à savoir tardives et de bas grade histologique. Une désescalade thérapeutique est possible pour ces rechutes afin d'éviter une mastectomie mutilante et souvent traumatisante.
Cependant, cette deuxième chirurgie conservatrice présente un taux élevé de deuxième rechute locale (19 à 50 % à 5 ans) du fait de l'absence de réirradiation, rendue impossible par le problème de tolérance des tissus préalablement irradiés.
Des études rétrospectives ou prospectives sur l'irradiation partielle du sein (PBI) en situation adjuvante rapportent des résultats prometteurs, tant en termes de tolérance (sauvegarde des tissus sains) que de contrôle local (74 % à 100 % à 5 ans). Les techniques utilisées comprennent la curiethérapie, la radiothérapie externe et la radiothérapie peropératoire (IORT).
L'IORT fait aujourd'hui l'objet d'un regain d'intérêt pour le cancer du sein. Il présente les avantages d'une balistique de haute précision sur la zone opérée et de préservation des tissus sains. A ce jour, aucune donnée prospective n'a cependant été publiée dans l'indication de récidive mammaire homolatérale.
Une conception prospective, multicentrique et à un seul bras de phase II évaluera la faisabilité d'une chirurgie mammaire conservatrice répétée combinée à une réirradiation utilisant l'IORT après une récidive locale du carcinome du sein.

- Récidive mammaire invasive non lobulaire prouvée histologiquement
- Délai depuis la radiothérapie du sein entier après la tumorectomie initiale> 5 ans
- Tumeur unifocale
- Taille de la tumeur récurrente ≤ 2 cm
- Taille de poitrine adéquate pour une deuxième chirurgie mammaire conservatrice avec un résultat esthétique acceptable
- Mammographie mammaire bilatérale dans les 90 jours précédant l'entrée à l'étude
- IRM mammaire dans les 90 jours précédant l'entrée à l'étude
- Grade histologique I-II
- Tumeur positive aux récepteurs des œstrogènes (ER+)
- Tumeur Cerb2-négative
- N0
- M0
- Radiothérapie antérieure administrée dans le cadre d'un programme de fractionnement standard
- État des performances (ECOG) 0-1
- Femmes ≥ 50 ans - Absence de conditions psychologiques, familiales, sociologiques ou géographiques susceptibles d'entraver le respect de l'étude et du calendrier de suivi
- Affilié au régime de l'Assurance Maladie Française
- Formulaire de consentement éclairé écrit et signé.
Critère d'exclusion:
- Récidive multifocale et/ou multicentrique
- Carcinome lobulaire
- Tumeur négative aux récepteurs des œstrogènes (ER-)
- Cerb2 (her2) surexprimé - cancer du sein
- Composant intracanalaire étendu (EIC) sur biopsie
- Envahissement des vaisseaux lymphatiques à la biopsie
- Statut N1-3 : récidive ganglionnaire régionale prouvée cytologiquement ou histologiquement
- Statut M1 : Maladie métastatique
- cT4 (atteinte cutanée ou musculaire) ou maladie de Paget du mamelon
- Radiothérapie antérieure délivrée dans le cadre d'un calendrier accéléré ou hypo-fraction
- Malignité antérieure autre que le cancer de la peau autre que le mélanome, sauf si le patient n'a pas été atteint de la maladie depuis au moins 5 ans
- Patientes avec un petit volume mammaire, techniquement insatisfaisant pour une deuxième chirurgie conservatrice ou une irradiation mammaire peropératoire.
- Chimiothérapie ou hormonothérapie préopératoire en cas de rechute locale
- Maladie du tissu conjonctif ou sclérodermie, contre-indiquant la radiothérapie
- Mutation connue du gène BRCA1/2 (un test génétique n'est pas requis)

Expérimental: Chirurgie et Radiothérapie Intra-Opératoire
Chirurgie : La tumorectomie sera réalisée selon les normes en vigueur, en obtenant des marges "claires". Le contrôle ganglionnaire axillaire dépendra de la prise en charge initiale (clinique et échographique) de ces patients N0, choisie par les équipes. Radiothérapie Intra-Opératoire (IORT): Après l'excision de la tumeur, IORT sera livré. Une dose unique de 20 Gy par des photons de 50 kV (Intrabeam™) sera administrée dans le lit tumoral. L'ajout d'IORT ne modifie pas le geste chirurgical. |
